OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Documentation the patient is not receiving the requested medication in combination with another biologic drug or with a targeted synthetic oral small molecule drug.12 months

Approval requires at least 12 months on therapy prior to reauthorization.

    Program-level Coverage Requirements

Program-level coverage criteria and operational requirements

Authorization duration and reauthorization: Approval for biologic and targeted immunomodulator therapies is limited to 12 months and requires annual reauthorization.12 months

Effective 1/1/26; annual reauthorization criteria added.

Duplication prevention: Coverage criteria include provisions to prevent duplicative targeted immunomodulator therapy (concurrent overlapping targeted agents).N/A

    Added to prevent duplicative therapy.

Preferred product and prerequisites: Formulary preferences and biologic prerequisites updated (e.g., Simlandi preferred adalimumab; Rinvoq, Xeljanz, and Cimzia added to pJIA prerequisites as of noted effective dates).N/A

Samples, free goods, or similar offerings of the requested medication do not qualify as evidence of an established clinical response and will not be considered for prior authorization. Providers should document clinically appropriate, verifiable trials of therapy in the medical record rather than relying on sample or promotional supplies when requesting coverage.

Documentation that a patient has a needle phobia or that administration is inconvenient does not qualify as a medically acceptable contraindication to injectable products. This limitation was specifically removed from prior language effective 1/1/2024 and remains part of the policy's exclusion criteria.

Documentation that a member is a social drinker does not qualify as a medically acceptable contraindication or clinical inappropriateness to methotrexate therapy. Social alcohol use alone should not be used as the rationale to deny methotrexate or to justify alternative therapies without additional clinical justification.

The Pharmacy Medical Necessity Guidelines are intended to guide coverage decisions but do not guarantee payment. Claims adjudication remains subject to member eligibility and benefits on the date of service, coordination of benefits, referral/authorization requirements, utilization management rules, adherence to plan policies and procedures, and claims editing logic; for self‑insured plans, the benefit document governs in case of discrepancy.

    Prior Authorization

Prior authorization required for Actemra SC

Prior authorization is required for Actemra (tocilizumab) subcutaneous for Harvard Pilgrim Health Care and Tufts Health Plan commercial products. Fax submission contact is provided in the policy materials for these commercial products.

    Prior Authorization

Authorization required; approval limited to 12 months

Prior authorization is required for biologic and targeted immunomodulator therapies per the Pharmacy Medical Necessity Guidelines. Approvals are limited to 12 months and require annual reauthorization.

    Step Therapy

Step therapy: required prior medication trials

Documented failure of specified traditional DMARD(s) is required and, for many indications, documented failure of two listed biologic or targeted agents is required prior to approval.

    Background and Drug Information

Tocilizumab (Actemra) is an interleukin‑6 (IL‑6) receptor antagonist used in multiple rheumatologic and related conditions including giant cell arteritis, juvenile idiopathic arthritis, rheumatoid arthritis, and systemic sclerosis‑associated interstitial lung disease. The drug is available as a subcutaneous formulation and is prescribed by appropriate specialists (for example, rheumatologists or neurologists depending on indication).

These guidelines align authorization with FDA‑approved indications and standard specialty prescriber expectations and require documentation of prior therapy trials, appropriate prescriber specialty, and absence of concurrent biologic or targeted synthetic oral small‑molecule therapy when applicable. Authorization durations and program-level requirements (such as the 12‑month approval interval and annual reauthorization) are also specified within the policy framework.

    Standard administrative conditions.

    • pJIA: failure of one traditional DMARD and two of the listed biologic/targeted agents (cimzia, enbrel, rinvoq, simlandi, Xeljanz for polyarticular JIA).
    • RA: failure of one traditional DMARD and two of the listed agents (cimzia, enbrel, rinvoq, simlandi, simponi, Xeljanz).
    • SSc‑ILD: failure of two agents among azathioprine, cyclophosphamide, mycophenolate mofetil.
    Billing Rule

Preferred agents and duplicate therapy prevention

Formulary preferences and biologic prerequisites have been updated; Simlandi is now the preferred adalimumab product and criteria include provisions to prevent duplicative targeted immunomodulator therapy.

    • Preferred product change: Simlandi replaced Humira as preferred adalimumab (effective 3/1/26).
    • Policy now includes criteria to prevent concurrent overlapping targeted immunomodulator therapy.
    Documentation Required

    Required documentation to support authorization

Providers must document the patient’s diagnosis, age, prescriber specialty, prior medication trials with specific drug names, dates, and duration of trials, and that the patient is not receiving the requested medication in combination with another biologic or targeted synthetic oral small molecule.

    • For corticosteroid requirement in giant cell arteritis: document drug, date and duration of systemic corticosteroid trial (or contraindication).
    • For traditional DMARD and biologic trials: document drug names, dates, and duration of each trial.
    Documentation Required

    Documentation and annual reauthorization requirements

Providers must supply clinical justification consistent with the Pharmacy Medical Necessity Guidelines; the plan may require documentation at authorization and annual reauthorization to support continued coverage.

    • Reauthorization requires documentation the member has been on the medication for at least 12 months and evidence of clinically significant improvement or stabilization.
    Denial Risk

Denial risk for missing prior or concomitant therapy documentation

Failure to document required prior therapy trials or to show the absence of concomitant use with another biologic or targeted small molecule may result in denial of the request.

    • Giant cell arteritis: lack of documented systemic corticosteroid trial (drug/date/duration) when required may trigger denial.
    • JIA/RA/SSc‑ILD: missing documentation of required DMARDs or biologic/targeted agent trials may trigger denial.
    • Concurrent use documented with another biologic or targeted synthetic oral small molecule is not allowed and may lead to denial.
    Denial Risk

Administrative and eligibility triggers for denial

Coverage determinations are made case‑by‑case and payment is subject to member eligibility, benefit terms, coordination of benefits, referral/authorization rules, and utilization management; discrepancies for self‑insured plans are governed by the benefit document.

    • Claims payment is also subject to claims editing logic and adherence to plan policies and procedures.
PurposePharmacy Medical Necessity Guidelines are developed to determine coverage for plan benefits by providing concise clinical coverage criteria for selected therapeutic classes or drugs.
Basis for recommendationsCurrent literature review, consultation with practicing physician experts, FDA and government agency policies, and national accreditation standards.
Use and limitationsGuidelines inform coverage decisions on a case‑by‑case basis but do not guarantee payment; claims adjudication remains subject to member eligibility, benefits, and plan procedures.
MaintenanceGuidelines are revised and updated annually or more frequently when new evidence requires revisions.
Applicability to self‑insured plansIf a discrepancy exists between a guideline and a self‑insured member's benefit document, the benefit document governs.
