OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Clinical Background

Sublingual and oral immunotherapy expose patients to controlled, small amounts of allergen extract to induce immune tolerance and reduce allergic symptoms. Sublingual immunotherapy (SLIT) delivers allergen extract under the tongue to modify the immune response and is indicated for pollen-induced allergic rhinitis in specified age ranges. Oral immunotherapy, such as peanut allergen powder, is taken orally to increase tolerance to food allergens and mitigate reactions from accidental exposures; it is used in conjunction with allergen avoidance and is not a substitute for emergency treatment.

FDA approvals for these therapies specify product-specific age ranges, required confirmatory testing (skin testing or in vitro specific IgE), and prescriber qualifications (allergist/immunologist). Coverage determinations in this guideline are made according to those product-specific criteria and required documentation.

    Definitions and Product Descriptions

Sublingual immunotherapy (SLIT) — definition / description
DefinitionAdministration of an allergen extract under the tongue to induce immune tolerance and reduce allergic symptoms (sublingual immunotherapy).
MechanismContains small amounts of allergen extract to decrease immune sensitivity and reduce the natural allergic response, resulting in symptom reduction.
FDA age range (examples)Products like Oralair and Odactra are indicated for patients 5 to 65 years of age (product-specific).
Confirmatory testingIndications require confirmation by positive skin testing or in vitro testing for allergen-specific IgE per product labeling.
Clinical useUsed to treat allergic rhinitis (with or without conjunctivitis) due to specific allergens represented by the product formulation.

  1. Oralair: documentation of failure, insufficient response, or intolerance to ≥2 generic oral antihistamines, nasal antihistamines, or nasal corticosteroids
  2. Documentation Required

    Documentation Requirements

Required documentation must accompany all prior authorization requests. Include age verification, confirmatory testing results, prescriber specialty, and origin of the prescription.

    • Age: documentation the member is within the age ranges specified by product (Odactra and Oralair: 5–65 years; Palforzia: initial dosing/up‑dosing rules and age per product labeling)
    • Confirmatory testing: Odactra — in vitro IgE to D. farinae or D. pteronyssinus OR skin testing to licensed house dust mite extracts; Oralair — positive skin test or in vitro pollen-specific IgE for listed grasses within the past 2 years; Palforzia — documented history of allergic reaction to peanuts AND positive peanut-specific IgE (in vitro or skin test)
    • Prescriber: documentation the medication is prescribed by, or based on the recommendations/consult of, an allergist or immunologist
    • Prescription origin: note if prescribed for initial dose escalation, up‑dosing, or maintenance as applicable
    Denial Risk

Failure to Meet Product-Specific Criteria — Denial Risk

Requests that do not meet product-specific criteria (age limits, required confirmatory testing, prescriber specialty, or required prior trials) are at risk for denial. Ensure all required items are clearly documented to avoid delays or adverse determinations.

    • Denial risk if age criteria not met (e.g., Odactra/Oralair: 5–65 years)
    • Denial risk if confirmatory testing is absent or outdated (e.g., Oralair: tests >2 years old)
    • Denial risk if prescriber is not an allergist or immunologist or if consult/recommendation documentation is missing
    • Denial risk if required prior trials (≥2 agents) are not documented or if intolerance/insufficiency is not shown
Palforzia — product definition / indication summary
ProductPalforzia (Peanut, Arachis hypogaea, allergen powder-dnfp).
IndicationOral immunotherapy indicated to mitigate allergic reactions, including anaphylaxis, that may occur with accidental peanut exposure in patients with confirmed peanut allergy.
