OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

      Dose verification required.

Coverage of Kresladi is not recommended for patients who have received prior gene therapy, for leukocyte adhesion deficiency type II or III, or for circumstances not listed in the Recommended Authorization Criteria.

Kresladi is not recommended for LAD-II or LAD-III because these disorders are caused by genetic variants other than ITGB2 and result in leukocyte migration defects unrelated to CD18; they are not expected to respond to Kresladi therapy.

Kresladi is not recommended after prior gene therapy because it has not been studied in patients who have undergone prior gene therapy.

    Disease and Therapy Background

Leukocyte adhesion deficiency-I (LAD-I) is a rare autosomal recessive inborn error of immunity involving defective leukocyte adhesion and migration during infection and inflammation.

    Definitions

    Leukocyte adhesion deficiency-I (LAD-I)
DefinitionA rare, autosomal recessive inborn error of immunity characterized by defective leukocyte adhesion, migration, and tissue recruitment during infection and inflammation.
Genetic causeCaused by pathogenic loss-of-function variants in the ITGB2 gene, which encodes the beta 2 integrin subunit CD18.
Disease characteristicsCommon clinical features include recurrent severe bacterial and fungal infections, impaired wound healing, and absence of pus at infection sites due to defective neutrophil recruitment.
    Severe LAD-I
CD18 criterionLess than 2% of expected polymorphonuclear neutrophil CD18 expression.
Combined expression criterionAt least 2% of expected polymorphonuclear neutrophil CD18 expression together with less than 2% of expected CD11a and/or CD11b expression.

    Lifetime Quantity Limit

    Kresladi quantity limit
Lifetime quantity limitOne dose per lifetime.
    AdministrationA single intravenous infusion.
    Minimum dose2.8 × 10^6 CD34+ cells/kg of body weight.

    Administration Site

    Billing Rule

    Administer the one-time intravenous infusion

Administer Kresladi by intravenous infusion as a one-time, single dose. The policy does not specify a required facility setting.
