- Home Care (Days 1-60) — Revenue Code 651: dollar amount listed for Days 1-60
- Home Care (Days 61+) — Revenue Code 651: dollar amount listed for Days 61 and after
- SIA (Hourly) — Revenue Code 651: dollar amount listed for SIA hourly
- Quarter Hour — Revenue Code 651: dollar amount listed for quarter-hour SIA
