OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage Criteria for Trigger Point Injections

    inv-01: Medically Necessary Indications

Covered when ALL of the following are met:

Initial Coverage Criteria: Trigger identified by history and palpation with local tenderness and/or weakness, restricted motion, or referred pain when compressed; presence of a taut band on palpation of an accessible muscle with marked point tenderness at one point along the length of the muscle; AND the pain has persisted for a minimum of 3 months; AND documented failure or contraindication to physical therapy or chiropractic care with documentation of a minimum of 4 weeks of therapy at least 2 times per week (minimum 8 visits) within one year of the request, with therapy directed to the body area to be treated (requirement not applicable to Medicare and Medicaid business segments) or documentation from an evaluating physical therapist/chiropractor within 4 weeks if the member cannot tolerate therapy; AND documented failure or contraindication to pharmacologic therapy with documentation of use of at least two classes from: NSAIDs; opiates; non-opioid analgesics; anti-epileptic medications for chronic pain; antidepressants for chronic pain; ASA or ASA derivatives; muscle relaxants; steroids (e.g., prednisone or Medrol), or documented contraindications to each of these drug classes.

    All conditions required

    inv-02: Repeat/Continuation Therapy

    Repeat injections are covered when:

Repeat injections: There is documented prior response to trigger point injections with at least 50% improvement in pain and functional status lasting at least 6 weeks, and symptoms have returned with associated decreased functional status.>=50% for >=6 weeks

    All conditions required

The following services are not included as part of trigger point injections: dry needling, injection with glucose or saline, and acupuncture. Contract-specific limitations or exclusions may also apply to these services; providers should verify member benefit language prior to submission.

Coverage for experimental, investigational, or unproven treatments, services, and procedures is specifically excluded under the member's certificate with Geisinger Health Plan. Unproven services performed outside of an approved clinical trial are also excluded. This policy does not override or expand exclusions listed in the member's certificate; consult MP015 for the evaluation process for experimental or investigational services.

This policy explicitly reiterates that dry needling, injections with glucose or saline, and acupuncture are excluded from the services described as trigger point injections. Confirm any additional contract-specific limitations before proceeding with treatment or claims submission.

    Provider Responsibilities and Documentation Requirements

    Prior Authorization

Prior authorization and coding note — 3-site limit; CPT 20552/20553

Authorization is limited to three injection sites in one session; when a site is injected it counts as one injection service regardless of number of needle passes. Use CPT 20552 (one or two muscles) or 20553 (three or more muscles) when submitting procedure coding and verify Medicare-specific coding rules if applicable.

    • Limit: ≤3 injection sites per session
    • When a given site is injected it is considered one injection service regardless of number of injections administered
    • CPT codes for billing: 20552, 20553
    Prior Authorization

Prior authorization may be required — check member contract and Geisinger resources

Prior authorization or precertification may apply depending on the member's contract and line of business. Check the member-specific benefit document and Geisinger’s provider prior authorization resources before scheduling.

    • Line-of-business benefits and limitations supersede this policy

Required Conservative Therapy and Medication Trials

    inv-19: Formal PT/chiropractic or documented inability to tolerate plus ≥2 medication classes tried/contraindicated — 4 weeks (2x/week) except Medicare/Medicaid

Covered when ALL of the following conservative treatment requirements are met (unless Medicare or Medicaid line of business):

Conservative treatment and medication trials: Formal physical therapy or chiropractic care for a minimum of 4 weeks at least 2 times per week (minimum 8 visits) within one year prior to the request, directed to the body area to be treated, OR documented inability to tolerate such therapy with documentation from an evaluating physical therapist or chiropractor within 4 weeks of the request; AND documented trials of at least two classes of medications from the following: NSAIDs; opiates; non-opioid analgesics; anti-epileptic medications used for chronic pain; antidepressants used for chronic pain; ASA or ASA derivatives; muscle relaxants; steroids (e.g., prednisone or Medrol), or documented contraindications to each required class.4 weeks (2x/week) except Medicare/Medicaid

Applies to non-Medicare/Medicaid business segments; minimum 8 visits

    Session and Site Frequency Limits

inv-21: Trigger point injections — usual maximal frequency not more often than three sessions in three months
Usual maximal frequencyNot usually more often than three sessions in a three month period
Definition of a sessionWhen a given site is injected, it will be considered one injection service regardless of the number of injections administered
Repeat injection response thresholdRepeat injections are considered medically necessary if prior documented improvement ≥50% in pain and function lasting at least 6 weeks
inv-22: Trigger point injections — authorization limited to 3 injection sites in one session

    Key Definitions

inv-17: Trigger point / Trigger point injection definition — clinical description of trigger points
Clinical descriptionTrigger points are focal, discrete areas of inflammation in skeletal muscle occurring from acute or chronic muscle strain and produce characteristic pain patterns for the affected muscle
Exam findings to documentIdentification by history and palpation with local tenderness, taut band, marked point tenderness on an accessible muscle, and referred pain when compressed
Therapeutic substancesInjections involve administration of local anesthetics, steroids and/or anti-inflammatory drugs
inv-18: Trigger Point Injections (CPT 20552, 20553) — definition of codes for one/two vs three or more muscles
CPT 20552Injection(s); single or multiple trigger point(s), one or two muscle(s)
    CPT 20553

    Clinical Background

Trigger points are focal, discrete areas within skeletal muscle that arise from acute or chronic muscle strain and produce characteristic localized and referred pain patterns. Trigger point injections, when indicated, deliver local anesthetics, steroids, and/or anti-inflammatory agents directly into the identified trigger point (typically a taut band with focal tenderness) to reduce pain and improve function.

    Services Not Covered

Not covered services under this policy include: Dry needling; Injection with glucose or saline; and Acupuncture. Additionally, experimental, investigational, or unproven trigger point injection procedures are excluded per the member's certificate and MP015.

    Imaging Requirements (if applicable)

    Note

    Imaging requirements

No specific imaging prerequisites are listed in the policy for trigger point injections; include imaging only if required by the member's benefit document or clinical judgment.

    • Policy does not specify imaging requirements for trigger point injections
    • Follow line-of-business benefit document or applicable LCD/NCD if imaging is required

  1. Prior authorization requirements posted at Geisinger provider prior authorization web page
  2. Step Therapy

Conservative care and medication trials required — PT/chiropractic (4 weeks, 2x/week)

Document prior conservative care: formal physical therapy or chiropractic for a minimum of 4 weeks at least 2 times per week (minimum 8 visits) within one year prior to the request, except for Medicare and Medicaid; if the member cannot tolerate therapy, submit PT/chiropractor documentation within 4 weeks stating inability to tolerate.

    • Therapy must target the body area to be injected; home exercise programs do not substitute
    • Requirement not applicable to Medicare and Medicaid business segments
    • If unable to tolerate therapy, provide evaluating PT/chiropractor note dated within 4 weeks
    Note

Confirm line-of-business applicability and policy hierarchy

If applicable, follow line-of-business rules and Medicare LCD/NCD guidance referenced by this policy; for PA Medicaid this policy applies as written.

    • Line-of-business specific benefit documents supersede policy
    • For Medicare, consult applicable LCDs/NCDs (e.g., referenced Novitas LCD)
    Documentation Required

Required clinical documentation — trigger point exam, chronicity, functional/pain measures

Include clinical exam findings that identify the trigger point (taut band, focal tenderness, referred pain with compression), document pain chronicity ≥3 months, and report functional and pain response measures for initial and repeat injection requests.

    • Trigger point identification on exam (taut band, marked point tenderness, referred pain)
    • Pain duration ≥3 months
    • Document functional status and pain improvement (≥50% for ≥6 weeks for repeat injections)
    Billing Rule

Coding and documentation note — record CPT 20552/20553 and align with benefits

Document the specific CPT code performed (20552 or 20553) and ensure coding aligns with the member’s benefit document and any applicable Medicare coding rules when submitting claims or authorization requests.

    • CPT 20552 — single or multiple trigger points, one or two muscles
    • CPT 20553 — single or multiple trigger points, three or more muscles
    • Verify Medicare coverage/coding rules as CMS may restrict covered CPT/HCPCS codes
    Denial Risk

Missing conservative therapy or medication trials — risk of denial

Requests lacking documentation of the required conservative therapy (minimum 4 weeks PT/chiropractic at 2x/week, ≥8 visits within one year) or of trials/failures/contraindications to at least two medication classes risk denial.

    • Failure to document required therapy or medication trials may result in denial
    • Medication classes to document trials or contraindications: NSAIDs, opiates, non-opioid analgesics, anti-epileptics for chronic pain, antidepressants for chronic pain, ASA/derivatives, muscle relaxants, steroids
    Denial Risk

Excluded services risk — experimental/investigational exclusions per member certificate

Services that are experimental, investigational, or otherwise excluded in the member’s certificate (including unproven services outside an approved clinical trial) are not covered and may be denied.

    • Policy does not expand coverage to services excluded by the member certificate
    • Examples of not-covered procedures per policy: dry needling, injection with glucose or saline, acupuncture
