OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria for AABPM

    Covered Indications

Covered when ALL of the following are met as an adjunct diagnostic tool in members unable to obtain accurate BP with conventional methods and one of the following indications:

    General coverage requirement

    • Indications (at least one required)

      • Suspected white coat hypertension: Office blood pressure >130/80 mm Hg on at least three separate clinic/office visits with two separate measurements made at each visit; AND documented evidence of two blood pressure measurements <130/80 taken outside the clinic/office with the diagnosis of hypertension remaining in question; AND no evidence of end-organ damage.
      • Suspected masked hypertension: Average office systolic BP 120–129 mm Hg or diastolic BP 75–79 mm Hg on two separate clinic/office visits with at least two measurements per visit, AND at least two out-of-office BP measurements >130/80 mm Hg.
      • Episodic hypertension symptoms: Symptoms (excessive sweating, palpitations, apprehension) suggesting episodic hypertension when office BP measurements are repeatedly normal.
      • Episodic hypotension/syncope: Symptoms consistent with episodic hypotension and/or syncopal events.
      • Nocturnal angina present.
      • Refractory hypertension: Hypertension that is refractory to optimized medication therapy.

    (no additional top-level items)

Automated ambulatory blood pressure monitoring (AABPM) is not covered when the established coverage criteria are not met. Specific routine monitoring exclusions include use of AABPM to establish the clinical diagnosis of hypertension, to predict end-organ damage, to evaluate responses to pharmacologic antihypertensive therapy, and for monitoring normal or pre-hypertension. Additional not-covered scenarios include diagnosing malignant hypertension, monitoring members with irregular cardiac rhythm, monitoring pregnant members who do not meet established criteria, and use in children under 18 years of age.

Coverage for experimental or investigational treatments, services, and procedures is specifically excluded under the member's certificate with Geisinger Health Plan. Unproven services provided outside an approved clinical trial are also excluded; this policy does not expand coverage to items or services that are explicitly excluded in a member’s certificate.

Additional not-covered specifics include use of AABPM for routine monitoring to establish a hypertension diagnosis, for prediction of end-organ damage, and for evaluation of response to antihypertensive therapy. The policy also designates AABPM as not covered for monitoring in cases of irregular cardiac rhythm, for pregnant members who do not meet coverage criteria, for diagnosing malignant hypertension, and for members younger than 18 years.

Provider Actions, Documentation & Prior Authorization

    Prior Authorization

Prior authorization & coding: AABPM covered only if criteria met; A4670 referenced

Automated ambulatory blood pressure monitoring (AABPM) is covered only when the policy's coverage criteria are met. The policy's coding guidance lists HCPCS code A4670 as the associated device code for an automatic blood pressure monitor.

    • AABPM coverage is contingent on meeting the documented indications and criteria in the policy.
    • HCPCS A4670 is listed as the automatic blood pressure monitor code in the policy coding section.
    Prior Authorization

Prior authorization may be required — check Geisinger listings

Prior authorization or pre-certification may be required depending on the member's contract; providers should consult Geisinger's prior authorization listings for specific requirements.

    • Check the Geisinger prior authorization listings at https://www.geisinger.org/health-plan/providers/ghp-clinical-policies for current requirements.
    • Precertification lists are found in the member's contract-specific benefit document.

    Background

Automated ambulatory blood pressure monitoring (AABPM) uses fully automated, noninvasive devices that obtain repeated blood pressure measurements over a typical monitoring period of 24 hours. AABPM is intended as an adjunct diagnostic tool to assess blood pressure variability related to daily activities and environmental stressors when conventional office measurements are unreliable or insufficient to resolve diagnostic uncertainty.

    Note

Provider action (placeholder) — actionable items to complete

Provider action item placeholder: include any required pre-visit steps, device scheduling, or referrals as applicable to this patient per contract and clinical indication.

    • Document indication that patient is unable to obtain accurate office BP with conventional methods.
    • Ensure scheduling and device provision follow the policy's limitations (one session, up to 3 days per calendar year) when applicable.
    Note

Provider action (placeholder) — post-test actions

Provider action item placeholder: capture any follow-up, communication of results, or care-plan adjustments required after AABPM per clinical findings and member benefits.

    • Provide interpretation and incorporate AABPM findings into the treatment plan when coverage criteria are met.
    • Communicate results to the member and document any medication changes or referrals.
    Documentation Required

Required clinical documentation for AABPM requests

Clinical documentation must include specific office and out-of-office blood pressure measurements supporting the indication (e.g., suspected white coat or masked hypertension) and relevant symptom descriptions.

    • Office BP: number of visits and at least two separate measurements per visit (e.g., office BP >130/80 on ≥3 visits for suspected white coat hypertension).
    • Out-of-office readings: documented evidence of at least two readings outside the clinic/office supporting the indication (e.g., two readings <130/80 for white coat or ≥130/80 for masked hypertension).
    • Symptom documentation when relevant: descriptions of episodic hypertension symptoms (excessive sweating, palpitations, apprehension), episodic hypotension/syncope, nocturnal angina, or refractory hypertension treatment history.
    Prior Authorization

Prior authorization notice — consult Geisinger resources

Prior authorization and/or pre-certification requirements for services or items may apply; providers should reference Geisinger's online prior authorization resources for details.

    • Prior authorization requirements are posted at Geisinger's provider policies web page.
    • Precertification lists may also be found in the member's contract-specific benefit document.
    Denial Risk

Denial risk: lack of required criteria will result in non-coverage

Services for AABPM will not be covered when the policy's established criteria are not met; see the exclusions list for not-covered indications.

    • Not covered when established criteria are not met.
    • Not covered for routine monitoring to establish a diagnosis of hypertension, predicting end-organ damage, evaluating response to therapy, monitoring irregular rhythm, pregnancy not meeting criteria, or in children <18.
    Billing Rule

Coverage contingent on member-specific benefits — codes listed do not imply coverage

Inclusion of a procedure or device code in this policy does not guarantee coverage; coverage is determined by the member-specific benefit plan and applicable laws.

    • Verify coverage against the member's benefit plan document before providing or billing for services.
    • Medicare members may be subject to separate Medicare coding rules and coverage determinations.
    Note

Line-of-business and contract terms supersede policy

Eligibility, contract-specific benefits, limitations, and exclusions take precedence; line-of-business benefit statements supersede this policy.

    • For Medicare, applicable LCDs and NCDs will supersede this policy.
    • For PA Medicaid business segment, this policy applies as written.
