Prior authorization is required for GLP-1 agents and will only be approved for FDA‑approved indications related to type 2 diabetes when supporting clinical documentation is provided. Providers must include medical record documentation of the patient’s type 2 diabetes diagnosis and evidence of prior trial and failure of metformin, metformin combination therapy, or insulin with the prior authorization submission; requests lacking this documentation will be denied.
Victoza (liraglutide) is excluded from coverage for Florida Blue commercial plans effective 2024-04-01. Current commercial members on Victoza will be allowed continued coverage through July 1, 2024. Note that Medicare coverage differs; Victoza is listed as Medicare-covered in the source document and exclusion applies to commercial plans only.
Requests for GLP-1 medications for indications other than type 2 diabetes — including weight loss or weight management — are considered not medically necessary and will be denied. Providers should inform patients that GLP-1 use for weight loss is not covered and must be paid out-of-pocket. Submissions to Prime Therapeutics/myPrime for weight-loss indications will result in denial and should be avoided to reduce administrative burden.
