Coverage Criteria for Posterior Tibial Nerve Stimulation (PTNS)

    Initial Therapy (Medically Necessary)

Covered when ALL of the following are met

Initial PTNS criteria: 1) The plan member has been evaluated by a specialist (usually a urologist or urogynecologist) who determined the patient is a candidate for PTNS; 2) The medical record documents compliance with and failure of a trial of symptom-appropriate behavioral therapy of sufficient length (e.g., bladder training, pelvic floor muscle training, fluid management); 3) The medical record documents compliance with and failure of at least one antimuscarinic medication administered for 4 to 8 weeks (failure may include lack of efficacy or intolerance); 4) The plan member is willing to attend weekly in‑office 30-minute treatment sessions and keep a bladder/voiding diary to monitor symptoms and efficacy.initial course = 12 weekly 30-minute sessions

Behavioral therapies may be combined with pharmacologic management.

    Maintenance Therapy Authorization

Covered when ALL of the following are met after initial course

Maintenance criteria: Member achieved greater than 50% decrease in OAB symptoms (for example: decreased urinary urgency, decreased urge incontinence, decreased frequency and/or nocturia) during the initial 12-week PTNS course>50% symptom reduction

Monthly maintenance treatments will be authorized for members who meet this threshold; continued treatment is not medically necessary if the member fails to achieve >50% decrease after the initial 12 weeks.

Implanted integrated neurostimulation system (Percutaneous implantation) coverage condition

Coverage for implanted integrated neurostimulation system codes is limited and conditional:

IDE-enrolled Medicare Advantage members: Member must be enrolled in the Category B IDE study G150178 (PROTECT) to qualify for coverage of percutaneous implantation, revision/removal, and programming T-codes for the integrated neurostimulation system

Applies to Medicare Advantage plan members only (including Fallon Medicare Plus and NaviCare HMO SNP); Fallon Health follows applicable CMS NCDs/LCDs and will require documentation of IDE enrollment.

    Supported diagnoses

Diagnostic support for outpatient PTNS services:

Supported first-listed outpatient diagnoses: PTNS services are supported when the first-listed outpatient diagnosis is one of the ICD-10-CM codes that best describe the patient's condition, for example: N32.81 (Overactive bladder), N39.41 (Urge incontinence), R35.0 (Frequency of micturition), or R39.15 (Urgency of urination)

ICD-10-CM code N32.81 should be reported for overactive bladder syndrome.

Any posterior tibial nerve stimulation use outside the indications and conditions described in this policy is excluded. This specifically includes uses for fecal incontinence and neurogenic bladder dysfunction. Implanted posterior tibial nerve stimulation devices (for example, the Protect PNS device) and transcutaneous tibial nerve stimulation systems are considered experimental/investigational and not medically necessary under this policy.

Not all services discussed in this policy are covered for every Fallon Health product or employer group. Coverage is governed by the member’s specific benefit plan and any applicable state or federal mandates; when the policy and a member’s benefit plan conflict, the benefit plan provisions will govern. Providers should consult the member’s Evidence of Coverage to confirm whether a service is covered for that product or employer group.

Transcutaneous tibial nerve stimulation (TTNS) systems (for example, ZIDA Wearable Neuromodulation Unit and Vivally System) and their associated supplies are designated experimental/investigational and not medically necessary. The policy lists the related HCPCS codes considered not medically necessary: E0736, E0737, A4545 (and notes that E0731 is not covered when used to report a ZIDA control sock). Implanted posterior tibial nerve stimulation devices are likewise considered experimental/investigational and not medically necessary outside applicable Category B IDE coverage.

The policy history documents prior clarifications and updates, including a historical clarification that the service was considered experimental for Commercial Plans. The history also records regulatory and coverage updates over time (for example, CMS approval of a Category B IDE for the Protect PNS device) and clarifications related to Medicare Advantage and other plan types; consult the Policy History for details when assessing prior determinations and applicability to specific lines of business.

    Step Therapy

Step therapy: behavioral therapy + at least one antimuscarinic

    Background and Scope

Overactive bladder (OAB) typically presents with urinary urgency, often with increased daytime frequency and nocturia, and may include urge urinary incontinence. Posterior tibial nerve stimulation (PTNS) is a minimally invasive neuromodulation technique that delivers low-voltage electrical stimulation to tibial nerve fibers via a percutaneous needle electrode. The standard initial PTNS course is one 30-minute session per week for 12 weeks, with individualized maintenance thereafter for responders. The policy distinguishes PTNS from transcutaneous tibial nerve stimulation (TTNS) and notes that TTNS systems and implanted PTNS devices are considered experimental/investigational and not medically necessary except as specified (for example, Category B IDE coverage for certain Medicare Advantage members).

    Definitions and Key Terms

PTNS — Posterior tibial nerve stimulation
DefinitionPosterior tibial nerve stimulation (PTNS): percutaneous insertion of a needle-electrode near the tibial nerve with a 30-minute stimulation session delivered via low-voltage generator and surface grounding electrode.
Typical needle placementNeedle-electrode inserted approximately two inches cephalad to the medial malleolus, with patient-controlled stimulation for 30 minutes.
Treatment setting and scheduleMinimally invasive, office-based procedure; treatments usually given once weekly for 12 consecutive weeks (initial course).
TTNS — Transcutaneous tibial nerve stimulation
DefinitionTranscutaneous tibial nerve stimulation (TTNS): noninvasive wearable systems that deliver neuromodulation via surface electrodes embedded in garments or socks (examples: ZIDA, Vivally).

Coverage Criteria for Posterior Tibial Nerve Stimulation (PTNS)Coding Guidance and Permitted CodesProvider Requirements, Documentation, and Billing Action ItemsBackground and ScopeDefinitions and Key Terms

Members must have trialed and failed symptom‑appropriate behavioral therapy and at least one antimuscarinic medication (administered for 4 to 8 weeks) before PTNS; clinicians may individualize duration or combination of therapies.

    • Behavioral therapies may be combined with pharmacologic management.
    • Failure of an antimuscarinic may be lack of efficacy or intolerance to adverse effects.
    Note

    Other provider actions: not stated

No additional provider action is stated in this section of the document.

    Documentation Required

    Required documentation for initial PTNS

Medical record must document specialist evaluation (typically urologist or urogynecologist), compliance with and failure of symptom‑appropriate behavioral therapy, compliance with and failure of at least one antimuscarinic medication administered for 4–8 weeks, and willingness to attend weekly in‑office sessions and keep a bladder/voiding diary.

    • Initial course defined as one 30‑minute PTNS session per week for 12 weeks.
    • Documented willingness to attend weekly treatments and keep diary is required.
    Documentation Required

    Required clinical and enrollment documentation

Documentation must include the first‑listed outpatient ICD‑10‑CM diagnosis code that best describes the patient’s condition (e.g., N32.81, N39.41, R35.0, R39.15); for implanted devices, include record of enrollment in the Category B IDE study when applicable.

    • ICD‑10‑CM code N32.81 should be reported for overactive bladder syndrome.
    • For Medicare Advantage members, include PROTECT (G150178) IDE enrollment when submitting T‑codes.
    Denial Risk

Denial triggers for maintenance and experimental technologies

Requests for PTNS maintenance after the initial 12‑week course will be denied if the member did not achieve a >50% decrease in OAB symptoms; TTNS devices, associated supplies, and implanted PTNS devices are considered experimental/investigational and not medically necessary.

    • Maintenance treatments authorized only if >50% symptom reduction achieved during initial course.
    • HCPCS codes E0736, E0737, A4545 and implanted Protect PNS procedures are listed as experimental/not medically necessary.
    Billing Rule

First‑listed outpatient diagnosis must be reported

Claims must report the appropriate first‑listed (outpatient) diagnosis code that best describes the patient’s condition for which the service was performed; failure to report an appropriate first‑listed diagnosis supporting medical necessity may lead to denial.

    Denial Risk

Implant T‑codes covered only for IDE‑enrolled Medicare Advantage members

Percutaneous implantation of the integrated neurostimulation system (CPT T‑codes 0587T–0590T) is covered for Medicare Advantage members only when the member is enrolled in the Category B IDE study G150178 (PROTECT); billing these T‑codes for members not enrolled in G150178 may result in denial.

    • Applies to Fallon Medicare Plus and NaviCare HMO SNP (dual eligible).
    • Fallon Health follows CMS NCDs/LCDs and covers Category B IDEs for Medicare Advantage members as described.
ZIDA Wearable Neuromodulation System (control sock with embedded electrodes) and Vivally System (wearable ankle garment with integrated electrodes and EMG sensing).
Primary distinction from PTNSTTNS delivers stimulation noninvasively through surface electrodes rather than via a percutaneous needle electrode used in PTNS.
    Initial course
Initial course definitionOne 30-minute PTNS session per week for 12 weeks (initial treatment phase).
Alternative/accelerated protocolTreatment variations may include an accelerated protocol of 3 times per week for 4 weeks.
Post-initial maintenanceFollowing the initial treatment phase, maintenance frequency is individualized; typically one treatment every 2 to 3 weeks or monthly if responder criteria met.
TTNS enabling home-based therapy with objective confirmation and digital integration
CapabilityEnables home-based therapy with objective confirmation of tibial nerve activation via integrated EMG sensing and wearable delivery platforms.
Digital integrationSome TTNS systems integrate digital health platforms for symptom and behavioral tracking and remote monitoring of therapy progress by providers.
Potential benefitsMay improve treatment adherence by addressing time, cost, and accessibility barriers versus in-office PTNS; long-term outcomes require further study.
Category B IDE study enrollment (PROTECT, G150178)
Enrollment requirementMedicare Advantage members must be enrolled in the Category B IDE study G150178 (PROTECT) to qualify for coverage of specified percutaneous implantation, revision/removal, analysis, and programming T-codes for the integrated neurostimulation system.
Applicable membersApplies to Medicare Advantage plan members including Fallon Medicare Plus and NaviCare HMO SNP (dual eligible).
Coverage limitationCoverage of implant-related CPT T-codes is conditional on documented enrollment in the PROTECT study; billing outside enrollment may result in denial for those members.
