    Investigational intradiscal procedures

    Intradiscal procedures are considered investigational.

    Denial Risk

IDET/IDTA codes carry an experimental designation

Percutaneous intradiscal electrothermal annuloplasty is designated experimental/investigational under CPT codes 22526 and 22527. Claims for IDET/IDTA reported with these codes may be denied.

    • 22526: single-level percutaneous intradiscal electrothermal annuloplasty, including fluoroscopic guidance.
    • 22527: one or more additional levels, reported with the primary procedure code.
    Denial Risk

    Clinical Background

Intradiscal procedures are minimally invasive techniques intended to address discogenic pain associated with annular disruption, contained herniated discs, annular tears or fissures, and internal disc derangement. Depending on the technique, treatment may involve placing a probe or catheter into the disc to deliver radiofrequency or electrothermal energy, injecting agents into the nucleus pulposus or annulus, decompressing disc material through percutaneous or endoscopic methods, supplementing disc tissue with an allogeneic cellular or tissue-based product, or using nucleoplasty to ablate disc tissue. The policy considers these procedures investigational because the supporting evidence is limited, conflicting, or insufficient to establish improved health outcomes; for example, one randomized study reported successful outcomes in 29% of percutaneous-discectomy patients compared with 80% of microdiscectomy patients.

    Procedure Definitions

    Intradiscal procedures
Procedure categoryMinimally invasive surgical procedures for symptomatic individuals with discogenic pain attributed to annular disruption of a contained herniated disc.
Clinical indicationsMay be used to seal annular tears or fissures or destroy nociceptors to relieve pain.
Methods includedIncludes percutaneous intradiscal probe placement with radiofrequency or electrothermal energy, injection of agents into the disc, and percutaneous decompression using direct or indirect visualization.
    Thermal intradiscal techniques
Technique categoryIntradiscal procedures using one or more probes or catheters to deliver thermal energy.
Delivery systemThe probes or catheters use a resistance coil or another delivery-system technology and may be flexible or rigid.

    Imaging Requirements

    Billing Rule

    Apply the required imaging guidance

Use the imaging guidance specified for the procedure. Automated percutaneous discectomy is performed under fluoroscopic guidance; endoscopic techniques use image guidance; and IDET/IDTA requires catheter insertion under fluoroscopy.

    • APD/APLD: fluoroscopic guidance.
    • Endoscopic techniques: image guidance for placement of the working channel.
    • IDET/IDTA: fluoroscopy for spinal-catheter insertion.
    Billing Rule

Follow code-specific fluoroscopic or CT guidance

Report the procedure with the imaging guidance specified in the applicable code description. The listed procedures require fluoroscopic, CT, or other indirect image guidance as applicable, including fluoroscopy or CT for percutaneous decompression, laminotomy/laminectomy, and cellular or tissue-based disc injections.
