    Daytime Ventilation Considerations

The policy identifies the following daytime ventilation circumstances.

Daytime ventilation indications: Daytime ventilation may be required because of worsening daytime hypercapnia with need for mouthpiece ventilation or because daytime use of a battery-operated unit is required to reduce hypercapnia or dyspnea.

    Hypoventilation syndrome

E0466 is medically appropriate for hypoventilation syndrome when the listed criteria are met.

Hypoventilation syndrome E0466 criteria: The individual has a hypoventilation syndrome diagnosis and BPAP was tried and failed with documented attempted daily use and persistent hypercapnia.Awake venous blood gas PCO2, end-tidal PCO2, or transcutaneous PCO2 ≥50 mm Hg

    Dual or multifunction ventilators

E0467 and E0468 are medically necessary when BOTH criteria are met.

    Dual or multifunction ventilator criteria

    • Home mechanical ventilator qualification: The individual meets criteria for a home mechanical ventilator.
    • Integrated function need: Documentation supports the need for one or more functions, such as an oxygen concentrator, cough stimulator, suction pump, or nebulizer, and the individual has not received any one of these devices individually.

      • Extended ventilatory support: Ventilatory support is required for more than 8 hours per 24-hour period.>8 hours per 24 hours
      • Alarms and internal battery: Alarms and an internal battery are required because the individual cannot effectively breathe independently for more than a few hours and unrecognized interruption of support is likely to cause a life-threatening condition if the individual or caregiver cannot otherwise be alerted.
      • Failure of advanced BPAP with iVAPS

        • Unmet iVAPS outcome: The individual cannot achieve normalization or stabilization of PaCO2, a 20% reduction in PaCO2 from baseline, or improvement in at least one symptom associated with chronic hypercapnia.PaCO2 normalization <46 mm Hg; or 20% PaCO2 reduction; or symptom improvement

    • Equipment condition and effect: The DME no longer functions adequately and inadequate function interferes with activities of daily living.
    • Expected repair result: Repair is expected to make the equipment fully functional as defined by the manufacturer.

    Replacement of non-warranty equipment

Replacement of a medically necessary home NIPPV device or component not under warranty is medically appropriate when EITHER pathway is met.

    Replacement pathway

    • Non-repairable or uneconomical repair: The DME no longer functions adequately and is non-repairable, or the cost of repair exceeds the replacement cost.
    • Clinical change: A change in the patient's condition makes the present unit nonfunctional and improvement is expected with a replacement unit.

    Re-qualification after initial trial

Individuals who do not meet continuation-of-coverage criteria during the initial 90-day trial may re-qualify when the following process is completed.

Re-qualification after failed trial: The treating physician performs a clinical reevaluation to determine the reason for failure to respond to ventilation therapy, documents factors such as unresolved symptoms or improper device fit, and provides re-education on proper equipment use or mask refitting as appropriate.Initial 90-day trial

Repair or replacement is ineligible for coverage when equipment is damaged or lost because of patient neglect, theft, or abuse. It is also ineligible when another available coverage source may apply, including homeowners, rental, automobile, or liability insurance.

Replacement of a properly functioning home NIPPV device, component, or accessory is not medically necessary when requested solely for advanced technology or improved aesthetics.

Devices used to clean or sanitize BPAP devices, including SoClean and SoClean 2 Go, are considered convenience items and are ineligible for coverage.

The monitoring feature or device described by HCPCS A9279 is inclusive to the positive airway pressure device. This includes stand-alone or integrated monitoring equipment and all related accessories, components, and electronics; it is not separately covered.

Replacement of a properly functioning home NIPPV device, component, or accessory is not medically necessary. A functioning unit is not eligible for replacement merely because newer technology is available or because a different appearance is preferred.

Accessories or components for home NIPPV devices that are considered not medically necessary or investigational based on peer-reviewed literature are also considered not medically necessary or investigational by the Health Plan.

    Background

This policy addresses home ventilatory support for individuals with neuromuscular or thoracic restrictive diseases that may weaken respiratory muscles, reduce vital capacity, or impair effective ventilation. BPAP qualification may be supported by impaired pulmonary function, inspiratory muscle weakness, nocturnal oxygen desaturation, or awake hypercapnia.

For E0466 under hypoventilation syndrome, document attempted daily BPAP use and failure together with qualifying persistent hypercapnia.

    Documentation Required

    Document integrated-function qualification

Document the need for one or more integrated functions and that the individual has not received any such device individually.

    Documentation Required

    Provide objective COPD testing

For COPD-related E0470 or E0471, provide the applicable ABG, sleep oximetry, and sleep apnea exclusion documentation.

    Documentation Required

    Document required additional functions

For E0467 or E0468, document the need for one or more additional integrated functions and confirm that the individual has not received those devices individually.

    • Examples include an oxygen concentrator, cough stimulator, suction pump, or nebulizer.
    Documentation Required

    Provide physician repair documentation

For repair of a medically necessary, non-warranty NIPPV device or component, physician documentation must include the date of DME initiation.

    Documentation Required

    Document repair eligibility

For repair of a medically necessary home NIPPV device or component not under warranty, include the date of DME initiation, manufacturer warranty information if applicable, and an attestation that the patient has been compliant with DME use and will continue to benefit from it.

    Documentation Required

    Document repair necessity and outcome

Document that the equipment no longer functions adequately, that inadequate function interferes with activities of daily living, and that repair is expected to make the equipment fully functional as defined by the manufacturer.

    Documentation Required

    Document replacement medical necessity

For replacement of a medically necessary home NIPPV device or component not under warranty, document either non-repairability or repair cost exceeding replacement cost, or a patient-condition change making the current unit nonfunctional with expected improvement from replacement.

    Not Covered Services and Situations

The policy uses a stepwise clinical framework: BPAP is considered when specified physiologic or symptomatic criteria are met, while more extensive non-invasive home mechanical ventilation is considered when BPAP is insufficient or when substantial daytime support is needed. Coverage decisions also distinguish medically necessary repair or replacement from equipment damage, preference-based upgrades, separately billed inclusive monitoring, convenience cleaning devices, and accessories that are not medically necessary or investigational.

  4. Vital capacity is less than 30%.<30%
  5. Worsening daytime hypercapnia requires mouthpiece ventilation.
  6. Daytime battery-operated ventilation is required to reduce hypercapnia or dyspnea.
