Source: NASS Coverage Policy Recommendations cited in document

This coverage policy is intended to guide medical necessity and prior authorization decisions for Cigna benefit plans administered by eviCore. Plan-specific benefit documents supersede this policy; some customers’ plans may contain exclusions or terms that differ from the standard guidance provided here. Coverage determinations require review of the applicable benefit plan, relevant laws/regulations, and the specific clinical facts of the case.

The policy requires an absence of unmanaged significant mental and/or behavioral health disorders (for example, major depressive disorder, chronic pain syndrome, secondary gain, opioid or alcohol use disorders) before authorization for non-urgent posterior cervical fusion. Providers should document that such conditions are either not present or have been actively managed, because unmanaged behavioral health disorders may preclude meeting medical necessity criteria and can be a basis for denial.

For repeat posterior cervical fusion when pseudoarthrosis is suspected, post-operative imaging demonstrating nonunion must be performed at least six (6) months after the prior cervical fusion. The policy specifies that pseudoarthrosis must be shown on imaging obtained no earlier than six months following the prior fusion to meet coverage criteria.

Posterior cervical fusion is considered not medically necessary when performed solely for indications that lack correlation with imaging or objective findings. Examples include isolated signs or symptoms without concordant imaging, annular tears, disc bulge without neural impingement or cord compression, degenerative disc disease alone, concordant discography or MR spectroscopy results, and isolated facet fusion using certain standalone allograft devices.

Performing a posterior cervical column osteotomy (PCO), cervical three-column osteotomy, or vertebral column resection without meeting the procedure-specific osteotomy criteria and the General Guidelines is considered not medically necessary. Authorization requires that the specific osteotomy indications and all applicable general criteria be satisfied.

Procedures undertaken for indications explicitly listed as contraindications (per NASS) are considered not medically necessary. Contraindicated examples include treatment of discogenic axial neck pain without neurological symptoms, asymptomatic spinal stenosis without MRI intrinsic cord signal change and no instability, and isolated foraminal stenosis treated in the absence of instability.

General prerequisites for authorization include: documented plain cervical spine x-rays including flexion/extension lateral views demonstrating concordant pathology when required; evidence of an adequate course of conservative, provider-directed non-surgical management when instability is non-urgent; documentation of nicotine-free status (either never-smoker or >= 6 weeks abstinence verified by objective cotinine testing); and documentation that significant mental/behavioral health disorders are absent or have been addressed.

When instability does not meet urgent or emergent thresholds, the policy requires documentation of a reasonable course of conservative treatment before surgical stabilization. Acceptable conservative measures include rest, appropriate medications, and cervical immobilization (e.g., cervical collar); the treating provider must document the trial, duration, and lack of clinical improvement prior to authorization consideration.

For non-urgent instability, procedures are not supported if the individual has not first attempted a reasonable, medically appropriate conservative therapy trial or when imaging and clinical findings are not concordant. Absent required imaging (including flexion/extension views) or documentation of conservative treatment response, the request may be denied as not meeting medical necessity.

Coverage may be denied when posterior cervical fusion (with or without decompression) is requested but the case does not meet the General Guidelines and the specific procedure criteria (initial, repeat, or post-arthroplasty). Failure to provide required imaging, documentation of conservative therapy, behavioral health status, or nicotine-free verification are common reasons for non-coverage.

Contraindication examples cited in the policy include: discogenic axial neck pain without neurological symptoms and asymptomatic spinal stenosis without MRI intrinsic cord signal change and no instability. These scenarios are listed by NASS as contraindications to posterior cervical fusion and are not supported for coverage under this policy.

Procedures performed for indications that are explicitly listed as contraindications in the policy (see exclusions) are considered not medically necessary and may be denied. This includes surgeries performed for discogenic axial neck pain without neurologic signs and asymptomatic stenosis without cord signal change or instability.

Prior Authorization, Documentation, and Provider Requirements

    Prior Authorization

    Prior authorization required per plan

Coverage determinations use this policy to inform medical necessity and prior authorization decisions; providers must follow the plan's prior authorization requirements and the terms of the applicable benefit plan document.

    Prior Authorization

    Prior authorization when meeting criteria

Initial posterior cervical fusion (with or without decompression) is considered medically necessary only when all associated procedure-specific criteria are met and should be supported in the prior authorization request.

    Prior Authorization

PA for repeat/revision fusion requires criteria documentation

Prior authorization is required for repeat or revision posterior cervical fusion and must include documentation that all medical necessity criteria for the specific repeat/revision indication (e.g., pseudoarthrosis, implant failure, failed disc arthroplasty) are met.

    Note

PA determined by applicable plan; code listing not a management directive

Prior authorization requirements (including whether a listed code requires PA) are determined by the applicable health plan; inclusion of codes in the policy does not imply management or guarantee payment.

    Documentation Required

Authorization requires indication and risk-mitigation documentation

Prior authorization should evaluate the indication against contraindications and may require documentation of preoperative risk mitigation such as treatment of psychological comorbidities and biochemical verification of nicotine abstinence.

    • Authorization may require evidence of addressing unmanaged significant mental/behavioral health disorders
    • Authorization may require objective cotinine testing to validate nicotine-free status
    Step Therapy

    Urgent/emergent exception to non-surgical prerequisites

For confirmed urgent or emergent conditions, several non-surgical prerequisites (plain x-rays, conservative trials, smoking cessation proof, absence of unmanaged behavioral health disorders, time frames for repeat procedures) are NOT required prior to surgery.

    Step Therapy

Conservative therapy required for non-urgent instability

For instability that does not meet urgent/emergent thresholds, documentation of an adequate conservative treatment trial (e.g., rest, medication, cervical collar) and failure to improve is required prior to authorization.

    Step Therapy

Conservative therapy duration varies; pseudoarthrosis requires 6 months trials

Conservative therapy prerequisites vary by indication; for symptomatic pseudoarthrosis with unremitting neck pain the policy requires 6 months of prescription-strength analgesics/steroids/gabapentinoids/NSAIDs and a 6-month provider-directed exercise program prior to authorization.

    • Pseudoarthrosis (unremitting neck pain): prescription analgesics/steroids/gabapentinoids/NSAIDs for 6 months
    • Pseudoarthrosis: provider-directed exercise program for 6 months
    Step Therapy

Required conservative therapy trials before authorization

Required conservative therapy prior to authorization includes trials of prescription-strength analgesics/steroids/gabapentinoids/NSAIDs and provider-directed exercise (each generally ≥6 weeks), and for radiculopathy at least two conservative modalities which may include epidural steroid injections.

    • Prescription analgesics/steroids/gabapentinoids/NSAIDs trial (typically ≥6 weeks)
    • Provider-directed exercise program (typically ≥6 weeks)
    • Radiculopathy: at least two conservative measures, possibly including epidural steroid injections
    Note
    Documentation Required

Cross-reference: see CMM-600.1 for PA timing and documentation

For additional timing and documentation requirements related to prior authorization, refer to CMM-600.1: Prior Authorization Requirements.

    Documentation Required

Required plain x-rays and nicotine testing

Plain x-rays of the cervical spine including flexion/extension lateral views must be performed and documented; when required, nicotine-free status must be documented and validated by objective cotinine testing (serum, urine, or saliva).

    • Obtain and document plain x-rays including flexion/extension lateral views
    • Validate nicotine-free status by cotinine testing when required
    Documentation Required

Document RA or instability-prone condition and concordant imaging

When rheumatoid arthritis or other conditions with increased incidence of instability are cited, provide documentation of the diagnosis plus imaging evidence of subluxation and/or spinal cord compression and flexion/extension x-rays.

    • Document RA or other instability-prone diagnosis
    • Provide imaging showing subluxation and/or spinal cord compression and flexion/extension x-rays
    Documentation Required

Required imaging, nicotine validation, and prior-treatment documentation

Provide imaging evidence (plain x-rays including flexion/extension lateral views; MRI/CT where specified), documentation of nicotine-free status validated by cotinine testing, clinical exam concordant with symptoms, and records showing prior conservative treatment and timing since any prior fusion or arthroplasty.

    • Plain x-rays with flexion/extension lateral views
    • MRI/CT when required by the specific indication
    • Objective cotinine test results validating nicotine-free status
    • Clinical exam notes matching imaging and prior conservative therapy records
    • Documentation of interval since prior fusion or disc arthroplasty (≥6 months when specified)
    Documentation Required

Required clinical documentation: symptoms, exam, and prior treatments

Clinical documentation should demonstrate symptom history, physical exam findings concordant with imaging, prior non-operative treatments and their durations, and absence of unmanaged significant mental/behavioral health disorders.

    • History of present illness and symptom severity
    • Physical exam findings concordant with imaging
    • Prior non-operative treatment modalities and durations
    • Assessment/treatment of significant mental/behavioral health disorders
    Documentation Required

Cotinine testing to document nicotine abstinence and report cutoffs

When nicotine abstinence is required, verify abstinence using biochemical cotinine testing (serum, saliva, or urine) and document test results using established cutoff ranges (serum 3.0–20 ng/mL; saliva 10–44 ng/mL; urine 10 ng/mL).

    • Acceptable specimen types: serum, saliva, urine
    • Report cotinine values and compare to cutoffs: serum 3.0–20 ng/mL; saliva 10–44 ng/mL; urine 10 ng/mL
    Denial Risk

Medical necessity determined case-by-case; unmet criteria may lead to non-coverage

Determination of medical necessity is made on a case-by-case basis; failure to meet the applicable General Guidelines or procedure-specific criteria may lead to non-coverage or denial.

    Denial Risk

Absence of nicotine-free documentation may lead to denial

Lack of documentation of nicotine-free status — either never-smoker documentation or objective cotinine testing showing abstinence for at least six (6) weeks — may trigger denial of authorization.

    Denial Risk

Imaging thresholds for instability required; absence risks non-approval

Absence of imaging demonstrating instability that meets defined thresholds (subluxation/translation >3.5 mm or sagittal angulation >11° on static or flexion/extension views) may result in non-approval for urgent/emergent instability indications.

    Denial Risk

Missing required documentation and risk factors increase denial risk

Failure to provide required documentation — including objective cotinine testing validating nicotine-free status, required imaging (including flexion/extension views), or evidence of conservative therapy when indicated — may increase the risk of denial.

    • Missing flexion/extension plain x-rays
    • No cotinine test results when abstinence is required
    • No record of prior conservative treatment where required
    Denial Risk

Not meeting criteria may result in denial of fusion

Posterior cervical fusion performed without meeting the General Guidelines and the applicable procedure-specific criteria (initial, repeat, or following failed disc arthroplasty) is considered not medically necessary and may be denied.

    Denial Risk

Procedures for non-indicated sole indications may be denied

Operations performed solely for indications listed as non-indications (e.g., signs/symptoms without correlation to imaging, annular tears, disc bulge without neural impingement, degenerative disc disease, isolated facet fusion) are considered not medically necessary and may be denied.

    • Do not request authorization for surgery when only non-indicated findings are present
    Denial Risk

Procedures for contraindicated indications are not medically necessary

Procedures performed for indications explicitly listed as contraindications (per NASS), such as discogenic axial neck pain without neurologic symptoms or asymptomatic spinal stenosis without MRI intrinsic cord signal change and no instability, will be considered not medically necessary.

    Background

Posterior cervical fusion is a surgical stabilization procedure performed to treat cervical spinal instability and deformity, and to address traumatic fractures, infection, neoplasm, or progressive neurologic compromise. It is intended to restore alignment, stabilize the cervical spine, and decompress neural elements when indicated.

    Definitions and Thresholds

    inv-66: PCO definition
PCO definitionPosterior cervical column osteotomy (PCO): Smith-Peterson (SPO) or Ponte osteotomy performed for non-fixed cervical kyphotic deformity
Correction per segmentRequires 5°–10° correction per spinal segment
Relation to fusionConsidered in addition to fusion when fusion alone cannot achieve correction
    inv-67: Three-column osteotomy (PSO/VCR) definition
Three-column osteotomy definitionCervical three-column osteotomy (PSO) or vertebral column resection (VCR) performed to correct fixed cervical kyphotic deformity
IndicationReserved for fixed deformity not correctable by fusion alone
    inv-68: Instability (urgent/emergent) definition
Instability (urgent/emergent) definitionPlain x-rays showing subluxation/translation >3.5 mm OR sagittal plane angulation >11° between adjacent segments on static or dynamic flexion/extension views
Imaging requirementPlain x-rays including flexion/extension lateral views
Clinical implicationMeets urgent/emergent criteria for definitive surgical treatment
    inv-69: Nicotine-free status definition
Nicotine-free status definitionNever-smoker OR abstinent ≥ 6 weeks prior to planned surgery validated by objective cotinine testing (serum, urine, or saliva)
ValidationCotinine testing verified within normal range for method and lab
Alternate proofDocumented never-smoker status accepted without testing
    inv-70: Urgent/Emergent Instability definition
Urgent/Emergent InstabilityPlain x-rays demonstrating subluxation/translation >3.5 mm or sagittal plane angulation >11° between adjacent segments on static or dynamic flexion/extension views
PurposeDefines imaging threshold for urgent/emergent surgical indications
    inv-71: Pseudoarthrosis definition
Pseudoarthrosis definitionPersistent nonunion at a previously fused cervical level demonstrated on post-operative imaging performed at least six months after the prior fusion
Associated criteriaRequires concordant symptoms (neck pain, radiculopathy, or myelopathy) and exam/imaging concordance
    inv-72: Myelopathy definition
Myelopathy definitionClinical syndrome with weakness, numbness, fine motor dysfunction, gait disturbance, bowel/bladder dysfunction, or frequent falls with concordant exam findings and imaging showing cord compression
Exam examplesHyperreflexia, Hoffmann sign, Babinski, ataxic gait, clonus, increased tone
    inv-73: Radiculopathy definition
Radiculopathy definitionUnremitting radicular pain to the shoulder girdle and/or upper extremity causing disability, with possible dermatomal sensory or motor deficits or positive nerve root tension signs
Diagnostic concordanceImaging must show neural compression concordant with symptoms
    inv-74: Cotinine cutoff values
    Cotinine cutoff values (serum)3.0–20 ng/mL
    Cotinine cutoff values (saliva)10–44 ng/mL
    Cotinine cutoff values (urine)10 ng/mL
