    Coverage Criteria for Cervical Microdiscectomy

Initial Primary Cervical Microdiscectomy — Radiculopathy

Initial primary cervical microdiscectomy is considered medically necessary when performed for EITHER of the following conditions when ALL of the associated criteria have been met:

Initial radiculopathy criteria: Symptoms: BOTH significant daily pain causing clinically significant functional impairment (e.g., inability to perform household chores, prolonged standing) AND unremitting radicular pain to the shoulder girdle and/or upper extremity resulting in disability; Physical exam: ANY of dermatomal sensory deficit, motor deficit (e.g., biceps, triceps weakness), reflex changes, shoulder abduction relief sign, nerve root tension sign (e.g., Spurling's maneuver), or unremitting radicular pain without concordant objective findings; Prior nonoperative care: Less than clinically meaningful improvement with at least TWO of the following (unless contraindicated): prescription-strength analgesics/steroids/gabapentinoids/NSAIDs for six (6) weeks; provider-directed exercise program for six (6) weeks; or epidural steroid injection(s)/selective nerve root block(s) performed at the same level(s) as the requested surgery; Imaging: MRI/CT demonstrates neural structure compression at the requested level(s) concordant with the individual's symptoms and physical exam and caused by one of: herniated disc(s) (including retained or recurrent), synovial/arachnoid cyst, central/lateral/foraminal stenosis, or osteophytes; Other: Absence of unmanaged significant mental and/or behavioral health disorders (e.g., major depressive disorder, chronic pain syndrome, secondary gain, opioid and alcohol use disorders).

Initial Primary Cervical Microdiscectomy — Myelopathy

Initial primary cervical microdiscectomy is considered medically necessary when performed for EITHER of the following conditions when ALL of the associated criteria have been met:

Initial myelopathy criteria: Symptoms: ANY of upper or lower extremity weakness, numbness, or pain; fine motor dysfunction (e.g., difficulty buttoning or handwriting, clumsiness of hands); gait disturbance; new-onset bowel or bladder dysfunction; or frequent falls; Physical exam: ANY of abnormal grip and release test, ataxic gait, hyperreflexia, Hoffmann sign, Babinski sign, tandem walking ataxia, inverted brachioradialis reflex, increased muscle tone or spasticity, clonus, or myelopathic hand; Imaging: MRI/CT shows findings concordant with the individual's symptoms and physical exam that are caused by cervical spinal cord compression or cervical spinal stenosis.

Repeat Cervical Microdiscectomy at the Same Level — Radiculopathy

Repeat cervical microdiscectomy at the same level is considered medically necessary when performed for EITHER of the following conditions when ALL of the associated criteria have been met:

Repeat radiculopathy criteria: Timing: Greater than 12 weeks since the prior cervical microdiscectomy; Symptoms: BOTH significant daily pain causing clinically significant functional impairment AND unremitting radicular pain to the shoulder girdle and/or upper extremity resulting in disability; Physical exam: ANY of dermatomal sensory deficit, motor deficit (e.g., biceps, triceps weakness), reflex changes, shoulder abduction relief sign, nerve root tension sign (e.g., Spurling's maneuver), or unremitting radicular pain without concordant objective findings; Prior nonoperative care: Less than clinically meaningful improvement with at least TWO of the following (unless contraindicated): prescription-strength analgesics/steroids/gabapentinoids/NSAIDs for six (6) weeks; provider-directed exercise program for six (6) weeks; or epidural steroid injection(s)/selective nerve root block(s) performed at the same level(s) as the requested surgery; Imaging: Post-operative MRI/CT demonstrates neural structure compression at the requested level(s) concordant with the individual's symptoms and physical exam and caused by one of: herniated disc(s) (retained or recurrent), synovial/arachnoid cyst, central/lateral/foraminal stenosis, or osteophytes; Other: Absence of unmanaged significant mental and/or behavioral health disorders (e.g., major depressive disorder, chronic pain syndrome, secondary gain, opioid and alcohol use disorders).

Repeat Cervical Microdiscectomy at the Same Level — Myelopathy

Repeat cervical microdiscectomy at the same level is considered medically necessary when performed for EITHER of the following conditions when ALL of the associated criteria have been met:

Repeat myelopathy criteria: Symptoms: ANY of upper or lower extremity weakness, numbness, or pain; fine motor dysfunction (e.g., difficulty buttoning or handwriting, clumsiness of hands); gait disturbance; new-onset bowel or bladder dysfunction; or frequent falls; Physical exam: ANY of abnormal grip and release test, ataxic gait, hyperreflexia, Hoffmann sign, Babinski sign, tandem walking ataxia, inverted brachioradialis reflex, increased muscle tone or spasticity, clonus, or myelopathic hand; Imaging: Post-operative MRI/CT shows findings concordant with the individual's symptoms and physical exam that are caused by cervical spinal cord compression or cervical spinal stenosis.

    Initial microdiscectomy indications

Covered when ALL of the following are met:

Primary indication: Individual has a clinical diagnosis of radiculopathy or myelopathy as defined by the symptom and physical exam criteria in this policy.
Concordant imaging: Preoperative or post-operative MRI/CT demonstrates findings concordant with the individual's symptoms and physical exam and caused by cervical spinal cord compression or cervical spinal stenosis.
Conservative care: For cervical radiculopathy, a 4–6 week trial of non-operative management is typically expected prior to elective surgery; exceptions apply for myelopathy or urgent/emergent conditions (e.g., progressive neurologic deficit).4-6 weeks

For requests involving urgent/emergent conditions, certain standard criteria may be omitted and definitive surgical treatment may proceed when documented imaging supports the indication. Examples of such urgent/emergent conditions include central cord syndrome, myelopathy or cord signal change on MRI from compression, progressive neurologic deficit documented on two separate exams, or neurocompressive pathology with severe motor weakness, rapidly progressive motor loss, or bowel/bladder incontinence/retention. When an urgent/emergent indication is claimed, provide the pertinent imaging findings as specified in the procedure section.

Cervical microdiscectomy performed solely for any of the following is considered not medically necessary: annular tears, concordant discography, MR spectroscopy results, or degenerative disc disease. Such sole-indication use should be identified and will not meet coverage criteria.

Several sections of the document provide reference material and bibliographic citations to support clinical context and decision-making. These reference-only sections do not themselves define additional coverage exclusions or create new medical necessity criteria; they are included for background and evidence support.

Any cervical microdiscectomy performed without meeting the General Guidelines and the applicable procedure-specific criteria (initial or repeat) is considered not medically necessary. Exceptions to standard requirements may apply for documented urgent/emergent conditions, but when those exceptions are not met, procedures lacking the required criteria and supporting documentation are not covered.

Within the cited reference and bibliography sections there are no explicit additional 'not medically necessary' criteria specified beyond those already listed in the policy. The primary not medically necessary triggers are described in the policy sections noted elsewhere.

Prior Authorization, Documentation, and Provider Requirements

    Prior Authorization

    Prior authorization required (see CMM-600.1)

Prior authorization applies to requests for cervical microdiscectomy and the determination of medical necessity is made on a case-by-case basis; providers must follow the timing and documentation requirements in CMM-600.1 when submitting requests.

    Note

Code inclusion does not guarantee coverage or PA

Inclusion of CPT codes in the policy's code table does not by itself indicate that a code is managed or requires prior authorization; prior authorization of a listed code is not a guarantee of payment and providers should refer to the applicable health plan for management details.

    Note

No explicit prior authorization codes in reference sections

The bibliographic/reference sections do not provide specific prior authorization codes or explicit prior authorization requirements for cervical microdiscectomy.

    Background

Cervical microdiscectomy is performed to relieve compression of neural elements causing radiculopathy or myelopathy. The procedure aims to decompress nerve roots or the spinal cord to address symptoms such as disabling radicular pain, motor or sensory deficits, and myelopathic signs. When progressive neurologic deficit, cord compression with signal change, or other urgent/emergent indications are present, some standard conservative-treatment requirements may be bypassed, but appropriate imaging documentation is still required.

    Definitions

Urgent/emergent conditions — warranting surgery without conservative management
Central cord syndromeIncluded as an urgent/emergent condition that warrants definitive surgical treatment without prior conservative care
Myelopathy or cord signal changes on MRI due to cord compressionUrgent indication — may bypass standard criteria
Progressive neurologic deficit documented on two separate physical examsPrompts definitive surgical treatment without prior conservative management
Severe neurocompressive pathology with motor weakness (grade ≤3/5), rapidly progressive motor loss, or bowel/bladder incontinence/retentionListed as urgent/emergent criteria
Severe debilitating pain/dysfunction causing incapacitationA condition otherwise meeting criteria with documentation of severe incapacitating pain may be treated urgently

Coverage Criteria for Cervical MicrodiscectomyCoding and Interval GuidancePrior Authorization, Documentation, and Provider RequirementsBackgroundDefinitions
Requirement for elective surgeryLess than clinically meaningful improvement after at least TWO conservative therapies (examples above) is required prior to elective surgery
    Step Therapy

Require failure of ≥2 conservative therapies for elective radiculopathy

For elective cervical microdiscectomy, the individual must show less than clinically meaningful improvement after at least two conservative therapies before surgery will be considered under the radiculopathy criteria.

    • Examples include 6 weeks of prescription-strength analgesics/steroids/gabapentinoids/NSAIDs
    • 6 weeks of a provider-directed exercise program
    • Epidural steroid injection(s) or selective nerve root block(s) at the same level(s)
    Note

4–6 week non-operative management expected before surgery

Most individuals with cervical radiculopathy are expected to undergo a 4–6 week course of non-operative management prior to surgery; exceptions apply for myelopathy or other urgent/emergent conditions.

    • Urgent/emergent conditions (e.g., progressive neurologic deficit, myelopathy) may bypass this expectation
    Step Therapy

    No step therapy requirements specified

No step therapy program requirements are described in the bibliography or reference sections of this policy.

    Documentation Required

Submit imaging (MRI/CT) showing concordant compression for requests

Elective and urgent requests must include imaging findings; for elective initial or repeat procedures, provide MRI or CT demonstrating neural structure or spinal cord compression concordant with symptoms.

    • For urgent/emergent conditions, imaging findings noted in the applicable procedure section(s) are required
    Documentation Required

Ensure clinical history, exam, and imaging are concordant

Documentation should demonstrate concordant history, symptoms, physical exam findings (including myelopathic signs when present), and imaging that correspond to the individual's clinical presentation.

    • Physical exam findings listed (e.g., Hoffmann sign, hyperreflexia, dermatomal deficits) should align with imaging
    • Post-operative MRI/CT must show findings concordant with symptoms for repeat procedures
    Note

References-only sections do not add submission requirements

The bibliographic/reference chunks contain citations only and do not specify documentation submission requirements for prior authorization beyond what is stated elsewhere in the policy.

    Denial Risk

Case-by-case determinations depend on CMM-600.1 compliance

Medical necessity determinations are made on a case-by-case basis and providers must adhere to the prior authorization timing and documentation requirements in CMM-600.1; failure to follow those requirements may result in denial.

    Denial Risk

Not medically necessary if General Guidelines or criteria not met

Cervical microdiscectomy performed without meeting the General Guidelines and the applicable procedure-specific criteria (initial or repeat) is considered not medically necessary and may be denied.

    • Procedures performed solely for annular tears, concordant discography, MR spectroscopy results, or degenerative disc disease are listed as not medically necessary
    Note

Bibliography does not define provider actions or denial triggers

The bibliography/reference portion of the policy does not specify provider submission actions or explicit denial triggers beyond those stated in the main policy sections.
