Covered when ALL of the following are met
Includes age-appropriate, FDA-approved COVID-19 vaccines for eligible individuals.
Obstructive utilization management includes prior authorization and step therapy when applied to these vaccines.
This coverage requirement does not apply to Medicare, certain self-funded arrangements, limited-scope benefit plans, or other excepted benefit plans. Carriers operating under those excluded plan types are not bound by the District of Columbia expectation to provide ACIP-recommended immunizations and administration without cost-sharing or obstructive utilization management as described for individual, small, and large group market plans.
Provider Obligations and Carrier Management Expectations
Prior authorization discouraged for ACIP‑recommended vaccines
