OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria

Initial coverage for acute post-operative pain

Covered when all of the following are met:

    Covered acute post-operative pain

    • FDA status: The TENS device or supplies are FDA-approved or FDA-cleared.
    • Covered indication: Treatment is for acute post-operative pain.

    Continuation therapy beyond 30 days

Extended coverage requires all of the following:

    Extended acute post-operative pain coverage

    • Persistent pain: The beneficiary's acute post-operative pain lasts longer than 30 days.>30 days
    • Monthly medical-necessity documentation: A TRICARE-authorized provider documents medical necessity each month.Monthly
    • Timely prescription: The device was prescribed within the first 30 days from the day of surgery.Within 30 days after surgery

    Conductive garment medical necessity

Covered only when all of the following are met:

    Conductive garment coverage

    • Authorized prescription: A TRICARE-authorized provider prescribed the garment for use in delivering covered TENS treatment.
    • At least one qualifying condition

      • Documented skin condition: The beneficiary has a documented skin condition that precludes use of conventional electrodes, adhesive tapes, and lead wires.
      • Large area or many stimulation sites: A large area or many sites must be stimulated, making use of conventional electrodes, adhesive tapes, and lead wires impracticable.

Home-use TENS devices and supplies prescribed during physical therapy for acute, subacute, or chronic low back pain are excluded from coverage.

TENS devices and supplies used to treat chronic post-operative pain are excluded from coverage.

TENS devices and supplies are excluded for the treatment of any condition not listed under the covered-condition provision.

    Provider Documentation and Denial Risks

    Documentation Required

    Prescription Timing and Extended Treatment

For acute post-operative pain, the TENS device must be prescribed within the first 30 days after surgery. If treatment continues beyond 30 days, supporting documentation of the ongoing treatment must be maintained.

    • Prescription timing: within the first 30 days from the day of surgery.
    • Treatment continuing beyond 30 days requires documentation supporting continued medical necessity.
    Documentation Required

    Monthly Medical-Necessity Documentation

When acute post-operative pain lasts longer than 30 days, a TRICARE-authorized provider must document medical necessity each month. Cost-sharing may not extend beyond 90 days (three one-month rentals) from the day of surgery.

    • Monthly medical-necessity documentation is required for treatment beyond 30 days.
    • Maximum duration: 90 days (three one-month rentals) from surgery.

    Background

Transcutaneous electrical nerve stimulation (TENS) is a non-invasive pain-management technology that delivers electrical impulses through adhesive electrodes applied to painful areas of the body. TENS devices are small, wearable, and battery-operated; treatment may be administered by a clinician or self-administered at home.

    Documentation Required

    Conductive-Garment Documentation

Conductive garments for delivering covered TENS treatment require a prescription from a TRICARE-authorized provider and supporting documentation that conventional electrodes, adhesive tapes, and lead wires cannot be used or are impracticable.

    • The provider must prescribe the conductive garment for covered TENS treatment.
    • Document a beneficiary skin condition that precludes conventional electrodes, adhesive tapes, and lead wires; alternatively, document that the treatment area or treatment frequency makes conventional delivery impracticable.
    Denial Risk

Denial Risks: Indication, Documentation, and Quantity

Claims are at risk of denial when the device is not FDA-approved or cleared, the treatment indication is not covered, required prescription or medical-necessity documentation is missing, or billed quantities exceed policy limits.

    • Use only TENS devices and supplies approved or cleared by the U.S. Food and Drug Administration for conditions listed in paragraph 2.1.
    • For acute post-operative pain, verify the prescription was issued within 30 days after surgery and provide monthly medical-necessity documentation when treatment exceeds 30 days.
    • For conductive garments, include the authorized-provider prescription and required documentation supporting the garment's medical necessity.
    • Observe quantity and duration limits: two-lead units allow up to one A4595 per month and one A4557 per episode; four-lead units allow up to two A4595 per month and two A4557 per episode, generally not beyond 90 days from surgery.

    Excluded Pain Indications

TENS devices and supplies are excluded when prescribed during physical therapy for acute, subacute, or chronic low back pain, and are excluded for chronic post-operative pain effective July 1, 2026.

    • Physical-therapy home-use TENS for acute, subacute, or chronic low back pain is excluded effective June 1, 2020.
    • TENS for chronic post-operative pain is excluded effective July 1, 2026.
