    Anesthesiologist Assistant Coverage Criteria

    Anesthesiologist supervision and responsibilities

Coverage of anesthesia services by an AA requires all of the following criteria to be met.

    AA supervision and anesthesiologist responsibilities

    • Required anesthesiologist activities: The anesthesiologist performs a pre-anesthetic examination and evaluation; prescribes the anesthesia plan; personally participates in the most demanding aspects of the plan, including induction and emergence if applicable; ensures that procedures not personally performed are performed by a qualified AA; monitors anesthesia administration at frequent intervals; remains physically present within the operating suite and available for immediate personal diagnosis and treatment of emergencies; provides indicated post-anesthesia care; and performs no other services while supervising no more than four AAs concurrently, or fewer if required by state law.No more than 4 AAs concurrently, or the lower state-law limit

The anesthesiologist need not be personally present for all services but must remain within the operating suite and available to provide immediate assistance.

    AA licensure and educational qualifications

The AA must meet all applicable legal and educational qualifications.

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

AA qualifications: The AA complies with all applicable requirements of state law, including any state licensure requirements for nonphysician anesthetists, and is a graduate of a qualifying master's-level anesthesiologist assistant educational program.

The educational program must be established under the auspices of an accredited medical school, accredited by the Commission on Accreditation of Allied Health Educational Programs or its successor organization, and include approximately two years of specialized basic science and clinical education in anesthesia building on a premedical undergraduate science background.

    Coverage and Denial Conditions

    Denial Risk

Failure to Meet AA Coverage Requirements

Coverage may be denied when any AA coverage requirement is not met. The AA must work under the direct supervision of an anesthesiologist who bills for the anesthesia services, and the supervising anesthesiologist and AA must satisfy all applicable supervision, state-law, education, and qualification requirements.

    • The anesthesiologist must perform a pre-anesthetic examination and evaluation and prescribe the anesthesia plan.
    • The anesthesiologist must personally participate in the most demanding aspects of the anesthesia plan, including induction and emergence when applicable.
    • Procedures in the anesthesia plan not performed by the anesthesiologist must be performed by a qualified AA.
    • The anesthesiologist must monitor anesthesia administration at frequent intervals, remain within the operating suite and immediately available for personal diagnosis and treatment of emergencies, and provide indicated post-anesthesia care.
    • While supervising no more than four AAs concurrently, the anesthesiologist must perform no other services. If state law establishes a lower concurrent-supervision limit, the state limit applies.
    • The AA must comply with all applicable state-law requirements, including licensure requirements for nonphysician anesthetists.
    • The AA must be a graduate of a master's-level anesthesiologist assistant program established under an accredited medical school and accredited by the Commission on Accreditation of Allied Health Educational Programs or its successor.
    • The educational program must include approximately two years of specialized basic science and clinical anesthesia education building on a premedical undergraduate science background.
