Coverage Criteria for SSRI/SNRI Duplicate Therapy
Initial/General Coverage Criteria
Covered when ALL of the following are met
No controlled clinical studies support concurrent use.
Documentation must demonstrate clinical justification for concurrent therapy if requested (concurrent therapy is generally not supported).
Concomitant use of two selective serotonin reuptake inhibitors (SSRIs) or an SSRI with a serotonin‑norepinephrine reuptake inhibitor (SNRI) is excluded as medically necessary. This policy prohibits therapeutic duplication of agents in these classes; members are limited to the use of a single SSRI or a single SNRI at any one time.
Therapeutic duplication of SSRIs and SNRIs is considered not medically necessary. There are no controlled clinical studies supporting concurrent use of two SSRIs or an SSRI plus an SNRI, and such combinations will not be covered.

Clinical Background
Concurrent use of SSRIs and SNRIs can produce additive adverse effects, with a primary safety concern being serotonin syndrome.
