    Other Indications / Off-label

    Other/Off-label uses

Other indications: 1) If the drug has had a label change within the last 6 months not yet reflected in this policy, follow the applicable formulary or non‑formulary policy for the relevant line of business; OR 2) If the requested use (e.g., diagnosis, age, dosing) is not listed in section III and criterion 1 does not apply, refer to the off‑label use policy for the relevant line of business (CP.CPA.09 for commercial, HIM.PA.154 for health insurance marketplace, CP.PMN.53 for Medicaid).

Non‑FDA approved indications not addressed in this policy are not authorized unless sufficient documentation of efficacy and safety per off‑label use policies or evidence of coverage documents

Non‑FDA approved indications that are not specifically addressed in this policy are not authorized unless the provider submits sufficient documentation of efficacy and safety in accordance with the applicable off‑label use policies (see CP.CPA.09 for commercial, HIM.PA.154 for health insurance marketplace, and CP.PMN.53 for Medicaid) or other evidence of coverage documents.

For Medicaid members, state Medicaid coverage provisions take precedence when they conflict with this clinical policy. Providers must consult the applicable state Medicaid manual for coverage rules that supersede this policy.

Use of omadacycline for non‑FDA approved indications is not authorized unless the request is supported by the off‑label use policies referenced above or other evidence of coverage documents demonstrating adequate efficacy and safety.

    Note

    Allow continuation of hospital‑initiated therapy

Continuation of omadacycline therapy is allowed when therapy was initiated in an acute care hospital from which the member was discharged; include documentation that therapy was hospital‑initiated when requesting continuation coverage.

    • This permits hospital outpatient continuation of therapy begun during an inpatient or acute care stay.

    Background

Omadacycline is a tetracycline‑class antibacterial indicated for adult patients with community‑acquired bacterial pneumonia (CABP) and acute bacterial skin and skin structure infections (ABSSSI). Use should be limited to infections proven or strongly suspected to be caused by susceptible organisms, consistent with the prescribing information and to reduce development of resistance.

    • HCPCS codes referenced in the policy include J0121 and J8499.
    • Step therapy bypass for Illinois HIM added per IL HB 5395 (see step therapy notes).
    • Providers must follow the Health Plan's PA submission procedures for required form fields (not specified in this excerpt).
    Step Therapy

Step therapy: failure of ≥2 formulary antibiotics required

Step therapy applies unless an Illinois HIM bypass is applicable: member must have failure of ≥2 formulary antibiotics active against the pathogen per culture & sensitivity (C&S) report (or failure of ≥2 formulary antibiotics indicated for the diagnosis if C&S not feasible), unless adverse effects or contraindications prevent use.

    • Preferred documentation: C&S report showing susceptibility to guide step therapy.
    • If C&S not feasible, document failure of ≥2 formulary antibiotics indicated for the diagnosis.
    Step Therapy

Step therapy bypass for Illinois HIM (IL HB 5395)

A statutory step therapy bypass was added for Illinois Health Improvement Matters (HIM) per Illinois HB 5395; as noted in the policy, the step therapy requirements do not apply to Illinois HIM requests effective 1/1/2026.

    • For Illinois HIM requests, the step therapy requirements in section I.b.ii do not apply as of 1/1/2026 per IL HB 5395.
    Documentation Required

    Required documentation to support PA

Providers must submit supporting clinical documentation with the PA request, such as office chart notes, laboratory results (including C&S when available), or other clinical information demonstrating the member meets all approval criteria.

    • Include C&S reports when available to demonstrate organism susceptibility, or documentation explaining why C&S is not feasible.
    • Include records of prior antibiotic trials and responses when relying on step therapy failure criteria.
    Note

Providers bound by policy terms; unauthorized use prohibited

Providers submitting claims and providing services for members are bound by the terms and conditions of this clinical policy; unauthorized copying, use, or distribution of the policy is prohibited.

    • Providers, members, and representatives are bound to the policy terms through their contracts or by submitting claims or providing services.
    Denial Risk

Denial risk if supporting documentation not provided

Failure to submit required documentation (for example, office notes, C&S or other lab results) demonstrating that the member meets all approval criteria may result in denial of the PA request.

    • Ensure documentation shows diagnosis, age, prior antibiotic failures or C&S results, dosing, and treatment duration when applicable.
    Note

Coverage subject to policy terms, exclusions, and legal requirements

Coverage decisions are subject to the terms, conditions, exclusions, and limitations of coverage documents and to state and federal requirements; if those requirements are not met, coverage may be affected or denied.

    • For Medicaid members, state Medicaid provisions take precedence when they conflict with this policy.
    • Coverage determinations do not constitute guarantees of payment and remain subject to contractual terms and applicable laws/regulations.
