Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    • Continued medical necessity: The documentation demonstrates treatment progress and the need for continued treatment, including that treatment cannot be rendered at a lower level of care.Review at least every 30 days

Review frequency is based on level of care, treatment progress, and documentation provided by the SUD facility.

    Prior Authorization

    Continued Stay Authorization Review

    • DSAMH may authorize multiple days, but each treatment day must meet medical necessity based on the facility's documentation.
    • Reviews are completed at least every 30 days, with frequency based on level of care, treatment progress, and documentation provided.
    • DSAMH SUD UR responds with an authorization or denial within two business days of submission.

    • EOB, proof of insurance verification, or proof of lack of insurance at referral.
    • Documentation that treatment cannot be provided at a lower level of care.
    • Treatment goals; progress and group notes since the last review; and documentation of psychoeducation and community resources, if available.

    Late CSR Submission

CSR and supporting clinical documentation may be submitted up to seven days before or up to seven days after the last covered day. Material submitted seven or more days after the last covered day is late and will not be accepted.

    • Late submission results in a denial of authorization for failure to submit the CSR for review.
    • No exceptions will be made.
    Denial Risk

    Incomplete CSR

An incomplete CSR will result in a denial of authorization. The form must include every required CSR element and the supporting clinical information needed for review.

    • Missing the number of days requested, new CSR start date, or authorization type identified as CSR makes the submission incomplete.
    • The CSR must also include treatment progress, justification for continued treatment, and treatment goals.
