OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    • At least one required prior therapy

      • Adequate trial of an adalimumab product for at least 3 months, with failure, inadequate response, significant side effects or toxicities, or a contraindication. At least 3 months
      • Adequate trial of Cimzia for at least 3 months, with failure, inadequate response, significant side effects or toxicities, or a contraindication. At least 3 months
    • Safety and dosing

      • Not receiving chronic immunosuppressant or immunomodulatory therapy, and no systemic medical condition resulting in significant compromised immune system function.
      • Documentation of testing for anti-JCV antibodies.
      • Prescribed at a dose within the manufacturer's dosing guidelines listed in the FDA-approved labeling.FDA-labeled dosing

    Initial Therapy - Multiple Sclerosis

      • Member is age 18 years or older.18 years or older
    • At least one preferred MS therapy

      • Inadequate response or intolerance to Betaseron (interferon beta-1b).

    Continuation Therapy - Crohn's Disease

    • Documentation or provider attestation confirms no evidence of progressive multifocal leukoencephalopathy (PML).No evidence of PML

Progressive multifocal leukoencephalopathy (PML) is an exclusion to initial therapy when present currently or historically. For continuation therapy, any evidence of PML prevents reauthorization. Provider documentation or attestation must address this safety requirement.

Initial therapy is excluded when the member is receiving chronic immunosuppressant or immunomodulatory therapy or has a systemic medical condition causing significant immune system compromise. Examples identified in the policy include 6-mercaptopurine, azathioprine, cyclosporine, methotrexate, TNF-alpha inhibitors, and—under the multiple sclerosis criteria—interferon beta-1a, interferon beta-1b, glatiramer acetate, or fingolimod.
