OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Provider Actions and Authorization Requirements

    Prior Authorization

    Prior Authorization Required

Evkeeza (evinacumab-dgnb) requires prior authorization under the Health Plan medical benefit. The request must satisfy all applicable initial-therapy criteria before approval.

    • Applies to practitioners providing the requested drug under the Health Plan’s medical benefit.
    • Requests that do not meet established criteria may be referred to a Medical Director for review, when required by the plan and request level.
    Step Therapy

    Required Praluent or Repatha Trial

Before initial Evkeeza authorization, document an adequate trial of the age-appropriate PCSK9 inhibitor with inadequate response, significant side effects/toxicity, or a contraindication.

    • Praluent: adequate trial of at least 3 months for members age 18 and older.
    • Repatha: adequate trial of at least 3 months for members age 10 and older.

    Background

Evkeeza (evinacumab-dgnb) is indicated as an adjunct to diet, exercise, and other LDL-C-lowering therapies for adult and pediatric patients age 1 year and older with homozygous familial hypercholesterolemia (HoFH).

Coverage is managed through prior authorization under the Health Plan medical benefit. The policy evaluates medical necessity for this limited-indication therapy and requires documented use of other LDL-C-lowering treatments, including an adequate age-appropriate trial of Praluent or Repatha before Evkeeza is considered.
