OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Clinical Background

Myalept (metreleptin) is a recombinant human leptin analog used as an adjunct to diet and as replacement therapy for complications of leptin deficiency in patients with congenital or acquired generalized lipodystrophy. Generalized lipodystrophy is characterized by loss or insufficient distribution of adipose tissue and may be associated with severe metabolic abnormalities.

    Prior Authorization

    Myalept REMS Prior Authorization Requirements

Myalept is available only through the restricted Myalept REMS Program because of risks including anti-metreleptin antibodies that may neutralize endogenous leptin and/or Myalept and the risk of lymphoma.

    • Prescribers must be certified with the Myalept REMS Program.
    • Pharmacies must be certified and may dispense Myalept only after receiving the Myalept REMS Prescription Authorization Form for each new prescription.

    • Submit relevant patient records, test results, and provider credentials when requested.
    • When dosing is based on weight, body surface area, or another member-specific measurement, include that measurement.
    • Documentation must support medical necessity, non-investigational use, and coverage within the member’s benefits.
    Documentation Required

    Diagnosis and Confirmed Leptin Deficiency

Document congenital or acquired generalized lipodystrophy and confirm leptin deficiency using a fasting Linco assay.

    • Examples of congenital generalized lipodystrophy include Berardinelli-Seip syndrome; an example of acquired generalized lipodystrophy is Lawrence syndrome.
