OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Policy Background

Lidocaine patches are topical local anesthetics used to provide localized pain relief. The policy identifies post-herpetic neuralgia as an FDA-approved use for 5% patches and ZTlido; cancer-related neuropathy and diabetic neuropathy are identified as compendial approved off-label uses. OTC 4% and 3.5% patches are indicated for temporary relief of minor localized pain.

    Step Therapy

    Preferred Topical Therapy Trial Required

Before requesting the product, document an adequate trial and failure of the preferred formulary topical dosage form, unless the member has a documented allergy or clinical contraindication to the formulary products that is not expected with the requested product.

    • Preferred topical options may include OTC 4% patches, cream, or gel.
    • Document the allergy or contraindication when a formulary trial is not appropriate.
    Step Therapy

    ZTlido and Gen7T Alternative Requirement

For ZTlido and Gen7T patches, document that the member tried and failed all formulary/preferred alternatives and generic non-formulary drugs with a matching indication before use of the requested therapy.

    • Requirement applies to ZTlido and Gen7T patches only.
    • The alternatives must include formulary/preferred options and generic non-formulary drugs with a matching member indication.
    Documentation Required

    Supporting Medical Records

Submit supporting medical records and any requested clinical documentation for coverage review. Records may include patient records, test results, and credentials of the provider ordering or performing the drug or service.

    • Provide patient records and relevant test results.
    • Supply provider credentials when requested.
    Documentation Required

    Diagnosis and Formulary Trial Documentation

Document a qualifying diagnosis of post-herpetic neuralgia, cancer-related neuropathy, or diabetic neuropathy, together with the required preferred topical therapy trial and failure or an eligible allergy or contraindication.

    • Document the diagnosis in the medical record.
    • Document adequate trial and failure of a preferred formulary topical dosage form, or the qualifying allergy/clinical contraindication.
    Documentation Required

    Additional ZTlido or Gen7T Requirements

For ZTlido or Gen7T, submit documentation supporting at least one additional qualifying pathway beyond the diagnosis and preferred topical therapy requirements.

    • Document that all formulary/preferred alternatives and matching-indication generic non-formulary drugs were tried and failed; or
    • Document FDA-labeled contraindication, serious side effects, likely reduced effectiveness, or likely harm from all such alternatives; or
    • Document that the member is currently receiving the requested medication and is at medical risk if therapy changes.
    Documentation Required

    Continuation Response Documentation

For continuation requests, document improvement in pain and confirm that the member has no intolerable adverse effects or drug toxicity.

    • Prescriber attestation or clinical reviewer finding may support the absence of intolerable adverse effects or toxicity.
    • Document positive clinical response as demonstrated by improvement in pain.

    Unsupported block type: denial_risk亡

    Documentation Required

    Additional ZTlido or Gen7T Pathways

For ZTlido or Gen7T requests, document at least one qualifying additional pathway: failure of all specified alternatives, contraindication or serious adverse effects/expected harm from all specified alternatives, or medical risk from changing current therapy.

    • The documentation must address both formulary/preferred alternatives and generic non-formulary drugs with a matching indication.
    • A current-therapy exception requires documentation of medical risk if therapy changes.
    Denial Risk

    Insufficient Supporting Documentation

Requests may be denied when submitted documentation does not support medical necessity, non-investigational status, eligibility under the member’s benefits, or appropriate billing and practice.

    • Additional documentation may be requested, including patient records, test results, and provider credentials.
    • Payment may be denied or other action taken when records do not support the coverage determination or indicate inappropriate or excessive billing.
    Denial Risk

Other Uses Are Experimental or Investigational

All other uses of lidocaine patch are considered experimental or investigational and are subject to the applicable off-label policy.

    • Non-policy indications are not supported as covered uses under this policy.
    Denial Risk

    Contraindicated Hypersensitivity

Coverage may be denied when the member has hypersensitivity to lidocaine, any component of the formulation, or another local anesthetic of the amide type.

    • Screen for hypersensitivity to lidocaine or formulation components.
    • Screen for hypersensitivity to another amide-type local anesthetic.
    Billing Rule

    Improper Billing or Noncovered Benefit

Claims may be rejected or payment may be recovered when lidocaine patch is billed incorrectly or is not a covered benefit. Listing or submitting a code does not guarantee coverage; providers must use industry-standard coding practices and verify benefit coverage.

    • Codes in the policy are for reference and may not be all-inclusive or applicable to every state or line of business.
    • Deleted or ineffective codes may not be eligible for reimbursement.
    • The payer may reject or deny the claim and recover payment when billing is inappropriate or the service is not a covered benefit.
    Compendial approved off-labeled uses
DefinitionCancer-related neuropathy and diabetic neuropathy are identified as compendial approved off-labeled uses of lidocaine patch.
    Use 1Cancer-related neuropathy
    Use 2Diabetic neuropathy

This criterion applies only to ZTlido and Gen7T patches.
