Monitor payer policy activity
This policy governs ConnectiCare coverage and prior authorization for balloon sinuplasty, including use with functional endoscopic sinus surgery (FESS) and as a stand-alone procedure. It applies to providers submitting clinical evidence for medical-necessity review; member benefit-plan terms may further limit coverage.
The policy clarified on March 10, 2023 that balloon sinuplasty is medically necessary when performed with FESS.
Endoscopy was added as a confirmation modality for chronic rhinosinusitis on September 10, 2021.
Recurrent acute sinusitis was added as a covered indication and endoscopy was removed as an evaluation option on August 14, 2020.
OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.
