An implantable cardioverter-defibrillator with a built-in ST-segment monitoring feature, also called an ICD-based ischemia monitor, is not medically necessary for any indication in adults or children because of insufficient evidence of therapeutic value.

Cardioverter-defibrillator therapy is not medically necessary when another disease process clearly and severely limits estimated life expectancy to less than one year.

ICD use is investigational and not medically necessary for clinical conditions other than those listed in this policy or when the specific criteria are not met.

ICD-based ischemia monitors and built-in ST-segment monitoring features are not medically necessary for any indication because insufficient evidence demonstrates therapeutic value.

Cardioverter-defibrillators are not medically necessary when another disease process clearly and severely limits estimated life expectancy to less than one year.

Unlisted clinical conditions, unmet policy criteria, and excluded device features are not covered as medically necessary under this policy.

Use of a subcutaneous or transvenous ICD is not medically necessary when the specific policy criteria are not met.

Covered ICD surveillance includes face-to-face or remote monitoring of device behavior; investigation of post-event shock, syncope, malfunction, or device failure; programming evaluation and adjustment; and modification or disabling of the device before surgery or other procedures. Remote interrogation is a single 90-day service, while in-person interrogation may be reported for each day performed.

    Authorization and Documentation Requirements

    Prior Authorization

Clinical Evidence Required for ICD Authorization

Before submitting an ICD or S-ICD request for prior authorization, provide clinical evidence demonstrating that the member meets the applicable medical-necessity criteria. FDA-approved transvenous ICD and S-ICD therapy is medically necessary only when the policy criteria are satisfied.

    • Include the applicable indication, clinical history, diagnostic findings, prior therapies, and other evidence supporting the requested device.
    • The reviewing medical director retains clinical judgment; benefit-plan terms govern coverage when they differ from this guideline.
    Documentation Required

    Submit Provider Documentation

The treating physician or primary care provider must submit the clinical evidence and documentation to ConnectiCare. Without this information, ConnectiCare cannot properly review the prior authorization request.

    • Submit documentation from the treating physician or primary care provider with the authorization request.

    Policy Background

An implantable cardioverter defibrillator monitors cardiac rhythm, recognizes ventricular fibrillation or ventricular tachycardia, and delivers an electrical shock to terminate the arrhythmia. By treating life-threatening ventricular arrhythmias, it reduces the risk of sudden cardiac death. Transvenous systems use leads placed through the veins, while subcutaneous systems use an electrode beneath the skin.

    Key Device and Clinical Definitions

    Transvenous implantable cardiac defibrillator (ICD)
Device purposeMonitors heart rate, recognizes ventricular fibrillation or ventricular tachycardia, and delivers an electrical shock to terminate these arrhythmias
Generator placementGenerator is placed in the subcutaneous tissue of the chest wall
Lead placementTransvenous leads are threaded intravenously into the endocardium
    Subcutaneous implantable cardiac defibrillator (S-ICD)
Device placementDefibrillator is implanted under the skin on the side of the chest below the armpit
Electrode placementElectrode is implanted under the skin along the rib margin to the breastbone

  12. Documentation Required

    Support the Applicable Clinical Criteria

Support the applicable clinical criteria with documentation of the indication and all required clinical elements for the requested ICD or S-ICD. The record should establish the relevant arrhythmic history, ventricular function, disease-specific findings, therapies, and exclusion of reversible causes where required.

    • For adult transvenous ICD primary prevention, document the qualifying condition and findings, such as prior myocardial infarction at least 40 days earlier, NYHA class, LVEF threshold, cardiomyopathy, hypertrophic cardiomyopathy risk factors, LMNA mutation, long-QT syndrome, Brugada syndrome, CPVT, cardiac sarcoidosis, giant cell myocarditis, Chagas disease, LV non-compaction, or ARVD, as applicable.
    • For adult transvenous ICD secondary prevention, document a life-threatening ventricular arrhythmic event, such as sustained VT, and that reversible causes—including acute ischemia, drug toxicity, and electrolyte abnormalities—were excluded.
    • For pediatric transvenous ICD requests, document the applicable qualifying event or condition, including prior cardiac arrest, symptomatic sustained VT with congenital heart disease, congenital heart disease with unexplained syncope plus ventricular dysfunction or inducible ventricular arrhythmias, or specified cardiomyopathy, genetic, or channelopathy risk factors.
    • Document required ventricular-function and treatment details, including LVEF, exclusion of reversible causes, and refractory status after at least 3 months of maximally titrated ACE inhibitor, beta blocker, and diuretic therapy when applicable.
    • For an S-ICD, document that the member meets the applicable transvenous ICD criteria and does not have symptomatic bradycardia, incessant or frequently recurring VT reliably terminated by antitachycardia pacing, or prior endocarditis or infection associated with a conventional ICD.
    Denial Risk

    Criteria Not Met

Use of a subcutaneous or transvenous ICD is investigational and not medically necessary when the clinical condition is not listed in the policy or when the specific applicable criteria are absent or unmet.

    • Document a listed indication and each required criterion before requesting authorization.
    • Requests for conditions outside the listed indications are subject to non-coverage.
    Denial Risk

Cardioverter-defibrillator therapy is not medically necessary when another disease process clearly and severely limits the member’s estimated life expectancy to less than one year.

    • Assess and document significant comorbid disease and the estimated life expectancy before implantation.
    Denial Risk

    ST-Segment Monitoring Feature

ICDs with a built-in ST-segment monitoring feature, also called ICD-based ischemia monitors, are not medically necessary for any indication in adults or children because evidence of therapeutic value is insufficient.

    • Do not request coverage for an ICD-based ischemia-monitoring feature as a medically necessary service.
    Denial Risk

    S-ICD Eligibility Restrictions

An S-ICD is medically necessary only when the member meets the applicable transvenous ICD clinical criteria and has none of the policy’s disqualifying conditions.

A subcutaneous or transvenous ICD is investigational and not medically necessary for clinical conditions other than those listed in the policy or when the specific criteria for the requested indication are not met.

    • Coverage requires a policy-listed indication supported by the submitted clinical evidence.
    • The investigational/non-medically-necessary limitation applies to both S-ICD and transvenous ICD use.
    Denial Risk

    ST-Segment Monitoring Feature

ICD-based ischemia monitors with a built-in ST-segment monitoring feature are not considered medically necessary for any adult or pediatric indication because evidence of therapeutic value is insufficient.

    • The non-coverage limitation applies regardless of the member’s age or underlying indication.
    Denial Risk

Life Expectancy Less Than One Year

Cardioverter-defibrillators are not considered medically necessary when another disease process clearly and severely limits estimated life expectancy to less than one year.

    • A request may be denied when severe comorbid disease limits expected survival to under one year.
FunctionElectrodes sense cardiac rhythm and deliver countershocks through the subcutaneous tissue; the system does not use transvenous leads
