Coverage Criteria for Panniculectomy (Ohio Only)

    Medically necessary (reconstructive) panniculectomy

Covered when the InterQual CP: Procedures, Panniculectomy, Abdominal criteria are met.

InterQual-based coverage: Member meets all applicable clinical criteria in InterQual CP: Procedures, Panniculectomy, Abdominalrefer to InterQual

InterQual is the determining clinical criteria referenced by this policy

Not medically necessary / Cosmetic indications

The procedure is considered cosmetic and not medically necessary in the following explicit situations:

Cosmetic or not covered indications: Performed for any condition that does not meet InterQual CP: Procedures, Panniculectomy, Abdominal criteria; performed in conjunction with abdominal or gynecologic surgery (e.g., hernia repair, bariatric surgery, C-section, hysterectomy) unless the member meets InterQual CP: Procedures, Panniculectomy, Abdominal criteria; performed for primarily cosmetic purposesN/A

Explicitly listed as cosmetic in policy

Coverage for panniculectomy is determined by clinical criteria rather than by the procedure label alone. This policy references InterQual CP: Procedures, Panniculectomy, Abdominal as the primary source for determining whether a panniculectomy is medically necessary (reconstructive); the procedure is covered only when the member meets the applicable InterQual criteria. Procedures performed to improve appearance rather than to treat a functional or medical impairment are considered cosmetic and are not covered. When panniculectomy is performed concurrently with other abdominal or gynecologic procedures (for example, hernia repair, bariatric surgery, cesarean section, or hysterectomy), coverage is allowed only if the combined procedure(s) meet InterQual criteria; otherwise the concurrent panniculectomy is considered cosmetic and not medically necessary.

Panniculectomy procedures themselves are not subject to U.S. Food and Drug Administration (FDA) device regulation; devices and instruments used during surgery may require FDA clearance or approval, but FDA approval alone does not determine coverage. Coverage decisions for members subject to this policy must also follow applicable federal, state (Ohio Administrative Code), and contractual benefit plan requirements; in the event of any conflict, those legal or contractual terms govern. UnitedHealthcare uses InterQual as the primary clinical criteria set and may use other UnitedHealthcare medical policies or guidelines when InterQual lacks applicable criteria.

Operationally, when a panniculectomy is performed primarily for cosmetic reasons or for any indication that does not meet InterQual CP: Procedures, Panniculectomy, Abdominal criteria, the service is considered not medically necessary and is subject to denial. Providers should obtain any required prior authorization and ensure the medical record fully documents the clinical indications that would satisfy InterQual criteria, recognizing that documentation alone does not guarantee coverage.

    BMI — risk threshold
    Threshold> 50 kg/m2 is associated with increased wound complications (odds ratio, 3.19; 95% CI, 1.02–9.96; p = 0.046)
Observed wound complication rateWound complications occurred in 11.5% of patients and were associated with prolonged hospital stay (odds ratio, 4.65; 95% CI, 1.99–10.86; p < 0.001)
Clinical implicationHigh BMI (particularly >50 kg/m2) increases perioperative wound morbidity and should be considered when assessing surgical risk and eligibility

Provider Requirements, Prior Authorization, and Documentation

    Prior Authorization

Obtain prior authorization using InterQual criteria

Coverage determinations for panniculectomy use the InterQual® CP: Procedures, Panniculectomy, Abdominal criteria. Obtain prior authorization per plan processes and include documentation showing the member meets the InterQual criteria.

    • Prior authorization is required and should reference InterQual CP: Procedures, Panniculectomy, Abdominal.
    • Submit supporting medical records with the prior authorization request to demonstrate InterQual criteria are met.
    Note

Follow federal, state (OAC), and contractual requirements

When deciding coverage, reference applicable federal, state (Ohio Administrative Code [OAC]), or contractual requirements; these govern if they differ from this policy. Prior authorization requirements are determined by the applicable benefit plan and state/contractual rules.

    • Check federal, state (OAC), and contractual benefit terms before relying on this policy.

    Background and Clinical Context

An abdominal panniculus is an apron-like overhang of excess skin and subcutaneous fat of the lower abdomen that commonly develops after massive weight loss. It can cause recurrent skin irritation, fungal or bacterial infections in skin folds, rashes, difficulty with hygiene, and interference with activities of daily living. While panniculectomy removes the redundant skin and subcutaneous tissue to address these functional problems, it typically does not include repair of the abdominal musculature and therefore differs from abdominoplasty procedures that may include fascial plication or umbilical transposition.

    Definitions

    Panniculectomy — definition and scope
DefinitionRemoval of hanging excess skin and subcutaneous fat in a transverse or vertical wedge
Exclusions (not included)Does not include muscle plication, neoumbilicoplasty (umbilical transposition), or flap elevation
Scope noteMay be performed as a functional procedure; a cosmetic abdominoplasty can be performed concurrently but is distinct from panniculectomy
    Functional panniculectomy — definition
DefinitionSurgical removal of a redundant panniculus to correct functional impairment (e.g., recurrent panniculitis, chronic back pain, interference with activities of daily living) after failure of nonsurgical management
IndicationsConsidered when symptoms persist despite an adequate trial of nonsurgical management and when the panniculus causes measurable functional impairment

  1. In case of conflict, federal, state (OAC), or contractual requirements take precedence over this policy.
  2. Note

Coverage governed by applicable laws; documentation may be required

Benefit coverage is determined by federal, state, or contractual requirements and applicable laws; medical records documentation may be required to assess whether the member meets clinical criteria but does not guarantee coverage.

    • Medical records may be requested to determine if InterQual criteria are met.
    • Documentation alone does not ensure that the service will be covered.
    Step Therapy

Document trial of nonsurgical management per ASPS

Document an adequate trial of nonsurgical management when panniculectomy is being considered for functional correction, consistent with ASPS guidance.

    • Record conservative treatments tried and duration, and persistent functional impairment despite nonsurgical management.
    Documentation Required

Include complete medical history, exam, and diagnostic test results

The patient's medical record must contain documentation that fully supports medical necessity, including relevant medical history, physical examination, and results of pertinent diagnostic tests or procedures; records should be legible, maintained in the chart, and made available upon request.

    • Include relevant medical history and physical exam findings demonstrating functional impairment or indications that meet InterQual criteria.
    • Attach results of pertinent diagnostic tests or procedures used to support the request.
    • Ensure documentation is legible and retrievable if requested for review.
    Documentation Required

Ensure medical records fully support medical necessity

Medical records documentation used for reviews must fully support the medical necessity for the requested services; documentation may be required to assess whether the member meets clinical criteria and must be made available upon request.

    • Provide clear, legible records that directly support InterQual criteria for panniculectomy.
    • Understand that providing records for review does not guarantee coverage.
    Denial Risk

Avoid requests for primarily cosmetic panniculectomy

Procedures performed for primarily cosmetic purposes or that do not meet InterQual criteria are considered cosmetic and not medically necessary and may be denied.

    • Do not submit requests for panniculectomy for mainly aesthetic reasons without demonstrating medical necessity per InterQual.
    Denial Risk

Concurrent panniculectomy with other abdominal/gynecologic surgery requires InterQual criteria

Concurrent panniculectomy performed with abdominal or gynecologic surgery (e.g., hernia repair, bariatric surgery, C-section, hysterectomy) is considered cosmetic and not medically necessary unless the member meets the InterQual CP: Procedures, Panniculectomy, Abdominal criteria.

    • If planning concurrent procedures, include documentation showing the member meets InterQual criteria for panniculectomy to avoid denial.
    • Be aware that evidence shows increased wound morbidity with concurrent procedures, which may affect coverage decisions.
    Denial Risk

Incomplete or missing records may cause denial or indeterminate coverage

Lack of adequate, legible medical records that support medical necessity may result in denial or an inability to determine coverage; make records available upon request to support review.

    • Ensure all requested documentation is submitted with the authorization request to prevent administrative denials.
    • Maintain legible, complete records of history, exam, diagnostic tests, and conservative therapy trials.
Relation to cosmetic proceduresFunctional panniculectomy is focused on restoring function; cosmetic abdominoplasty may be performed at the same time but does not define functional indication
