OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Code services accurately

Submit the service using the most appropriate codes in effect on the submission date and include applicable covered diagnosis and procedure codes.

    Denial Risk

    Use applicable covered codes

Claims submitted without applicable covered procedure or diagnosis codes may be denied as not covered. Reimbursement is not allowed for services billed for conditions or diagnoses that are not covered under the policy.

    Denial Risk

Visualization technologies are not medically necessary

Services billed for cervicography, spectroscopy or optical detection systems, or speculoscopy are considered not medically necessary for any indication, including cervical cancer screening, and may be denied as not covered. CPT 58999 is identified for reporting these noncovered technologies.

    Clinical Background

Visualization technologies have been proposed as adjuncts to, or replacements for, Pap cytology, HPV DNA testing, and colposcopy. Colposcopy provides an illuminated, magnified examination of the cervix and is primarily used to evaluate abnormal cervical screening results or abnormal gross findings, often with a colposcopically directed biopsy; however, colposcopy alone has not been shown to be an effective cervical cancer screening tool. Evidence for cervicography and spectroscopy remains limited or insufficient for routine clinical adoption, despite some studies reporting promising diagnostic performance.

    Terms and Technology Definitions

    Cervicography
DefinitionA visual screening method using a specially designed 35-mm camera to photograph the cervix.
PreparationThe cervix is photographed after application of a 3% to 5% acetic acid wash.
EvaluationThe film is sent to a laboratory for processing and evaluation.
    Spectroscopy / Optical Detection System
DefinitionAn optical detection approach that emits light from a probe onto the cervix.
Assessment basisDifferences in tissue optical properties are used to categorize tissue as normal or diseased.

Approaches include fluorescence spectroscopy, white light elastic backscatter spectroscopy, infrared spectroscopy, Raman spectroscopy, image analysis of visible images, or combinations of these methods.
    Colposcopy
DefinitionA diagnostic procedure using a colposcope, a dissecting microscope with various magnification lenses.
View providedThe procedure provides an illuminated, magnified view of the cervix, vagina, vulva, or anus.
Primary goalThe primary goal is to identify precancerous and cancerous lesions so that they may be treated early.
    Speculoscopy
DefinitionAn examination that applies acetic acid to the cervix and uses magnified vision and a chemiluminescent light to detect abnormalities.
Examination principleThe examination uses the same principle as colposcopy by applying acetic acid to the cervix.
Detection methodMagnified vision and chemiluminescent light are used to detect abnormalities.
    Speculite
DefinitionA low-energy, diffuse, blue-white chemiluminescent light.
Light propertiesThe light is cold and independent of temperature because it is produced through chemiluminescence.
Placement and useThe light is affixed to the top of a speculum for vaginal and cervical illumination.
    Health Equity
Health equityThe highest level of health for all people.
Health inequityAn avoidable difference in health status or distribution of health resources.
Underlying conditionsHealth inequity is due to the social conditions in which people are born, grow, live, work, and age.
    Social Determinants of Health
DefinitionConditions in the environment that affect a wide range of health, functioning, quality-of-life outcomes, and risks.
Community conditionsExamples include safe housing, transportation, and neighborhoods; racism, discrimination and violence; and access to clean air and water.
Economic and access factorsExamples include education, job opportunities and income; access to nutritious foods and physical activity opportunities; and language and literacy skills.
