    Clinical Background

Hemophilia A is an X-linked bleeding disorder caused by Factor VIII deficiency. It may lead to spontaneous or trauma-related bleeding into the joints, bleeding at other body sites, and hemophilic arthropathy. Factor VIII replacement products include extended- and standard-half-life recombinant products as well as plasma-derived products, including products with or without substantial von Willebrand Factor.

    Documentation Required

    Clinical Request Documentation

Clinical requests should identify the requested product, covered diagnosis or indication, prescribing provider, specialist prescription or consultation, and requested regimen. Include the applicable dosing details, including dose per kilogram, frequency, duration, and whether treatment is prophylactic, on-demand, perioperative, or immune-tolerance therapy.

    • Document the requested Factor VIII product and the diagnosis or indication, such as Hemophilia A or von Willebrand disease.
    • Identify the prescribing provider and hemophilia specialist involvement.
    • Document the requested dose, frequency, and duration and show that it matches an established regimen.
    Documentation Required

    Von Willebrand Disease Supporting Documentation

For Alphanate, Humate-P, and Wilate requests for von Willebrand disease under Employer Plans or Individual and Family Plans, document the applicable plan-specific qualification pathway and supporting clinical facts. Type 1 von Willebrand disease treatment of bleeding episodes does not apply to routine prophylaxis or surgery-related use.

    • Employer Plans and Individual and Family Plans: document that the patient meets at least one applicable pathway.
    • Type 1 von Willebrand disease: document a trial of injectable desmopressin (DDAVP), a severe bleeding phenotype, or very low von Willebrand factor levels.
    • Alternatively, document Type 2 or Type 3 von Willebrand disease; inadequate response to injectable desmopressin or safety concerns with its use; or, where applicable, that the patient is younger than 2 years.
    • For Type 1 disease, specify that the request is for treatment of a bleeding episode rather than routine prophylaxis or surgery-related use.
    Denial Risk

    Unlisted Use Is Not Covered

Use outside the policy's listed indications is not medically necessary and is not covered under this policy.

    • Applies to Adynovate, Eloctate, Esperoct, Jivi, Advate, Afstyla, Kogenate FS, Kovaltry, Novoeight, Nuwiq, Recombinate, Xyntha, Hemofil M, Koate, Alphanate, Humate-P, and Wilate.
    Denial Risk

    Criteria or Dosing Not Met

Coverage may be denied when the request does not meet the applicable indication, specialist-prescriber or consultation requirement, or established dosing criteria. Doses outside the established dosing in this policy require case-by-case review by a clinician, such as a Medical Director or Pharmacist.

    • The diagnosis or indication must be one listed in the policy, including Hemophilia A or the applicable von Willebrand disease criteria.
    • The agent must be prescribed by or in consultation with a hemophilia specialist when required.
    • The requested dose, frequency, and duration must fall within an approved regimen unless separately reviewed case by case.
    • Extended approval requires the patient to continue meeting the applicable criteria and dosing.
    Billing Rule

Billing Codes and Plan Terms Control

Claims submitted without covered code(s) under the applicable Coverage Policy may be denied as not covered. The customer's applicable benefit-plan document controls if it conflicts with this policy, and benefits are ultimately determined by that plan document absent a controlling mandate.

    • Use the most appropriate billing codes in effect on the submission date.
    • Reimbursement is not allowed for services billed for conditions or diagnoses not covered under the policy.
    • The applicable Group Service Agreement, Evidence of Coverage, Certificate of Coverage, Summary Plan Description, or similar plan document supersedes conflicting policy language.
    Denial Risk

Any Other Use Is Not Medically Necessary

Hemophilia Factor VIII products used for any use other than the policy's listed indications are considered not medically necessary. Such requests may be denied unless the policy is updated to address new published data.

    • Applies to all listed products, including Factor VIII products and Alphanate, Humate-P, and Wilate.
    Note

Type 1 von Willebrand Disease Criteria

For Type 1 von Willebrand disease treatment of bleeding episodes, the patient must meet at least one of the listed alternative pathways. These pathways do not apply to routine prophylaxis or surgery-related use.

    • The patient has tried injectable desmopressin (DDAVP injection).
    • The patient has a severe bleeding phenotype.
    • The patient has very low von Willebrand factor levels.
    • The patient has Type 2 or Type 3 von Willebrand disease.
    • The patient has a history of inadequate response to injectable desmopressin or safety concerns with its use.
    • For Employer Plans, the patient is younger than 2 years of age.
