OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage and Medical Necessity Criteria

    Medically Necessary Criteria

Liposuction for lipedema is considered medically necessary when ALL of the following are met:

Overall requirement: All listed criteria A through H must be satisfied per policy
A: Functional impairment: Physical functional impairment (difficulty ambulating or performing activities of daily living)
B: Pain/tenderness: Pain and tenderness on palpation in affected areas
C: Stemmer sign: Negative Stemmer sign unless the individual has coexisting lymphedema (Stemmer sign is negative if the skin can be lifted at the base of the second toe or second finger)
D: Subcutaneous nodules: Presence of subcutaneous nodules of adipose tissue in affected areas
E: Pitting edema absent: Absence of pitting edema unless coexisting lymphedema is present
F: Conservative therapy failure: Failure to respond to conservative treatment for at least three consecutive months, including compression therapy, manual lymphatic drainage, and documented participation in a physician-supervised weight loss program>= 3 months

Policy revised from six to three months for conservative therapy requirement

G: Documented chronic complications: Medical records and photographs documenting at least one chronic and persistent complication refractory to conservative therapy for at least three consecutive months>= 3 months

Examples include persistent pain, swelling, functional impairment, or bruising

H: Lack of improvement with elevation: Lack of improvement in swelling from elevation of the limbs in lipedema-affected areas
Other signs: Tendency to bruise easily in lipedema-affected areas without apparent cause

It is the policy of Centene-affiliated health plans that liposuction for the treatment of lipedema is considered medically necessary only when all specified criteria are met. These required criteria include: documented physical functional impairment (for example, difficulty ambulating or performing activities of daily living); pain and tenderness on palpation in affected areas; negative Stemmer sign unless coexisting lymphedema is present; presence of subcutaneous nodules of adipose tissue; absence of pitting edema unless coexisting lymphedema exists; documented failure to respond to conservative therapy including at least three consecutive months of compression therapy, manual lymphatic drainage, and participation in a physician-supervised weight loss program; medical records and photographs showing chronic, persistent complications refractory to conservative therapy for the required duration; characteristic distribution of adipose changes (such as bilateral, symmetrical accumulation or disproportionate lower-extremity hypertrophy relative to the trunk, or the "cuff" phenomenon); lack of improvement with limb elevation; and a tendency to bruise easily in affected areas. Providers requesting authorization must demonstrate that all of these policy-specified elements are satisfied.

Liposuction for lipedema is not medically necessary for any indications other than those explicitly specified in this policy. Requests for procedures that do not meet every required policy criterion will not be considered medically necessary.

    Authorization, Documentation, and Operational Requirements

    Prior Authorization

    Prior Authorization Required

Prior authorization is required for liposuction procedures for lipedema. Submit requests with complete clinical documentation demonstrating that all policy-specified medical necessity criteria are met. A list of applicable CPT codes should be included with the authorization request.

    • Prior authorization required for listed CPT codes (e.g., 15877, 15878, 15879).
    • Requests must include sufficient clinical documentation to demonstrate all policy criteria are met.
    Documentation Required

    Conservative Therapy Prerequisite

All requests must document an adequate trial of conservative therapy prior to consideration of liposuction. Conservative treatment must include compression therapy, manual lymphatic drainage (part of complex decongestive therapy), and a physician-supervised weight loss program for the durations specified in the policy.

    • Conservative therapy trial of at least three consecutive months including: compression therapy; manual lymphatic drainage (CDT); and documented participation in a physician-supervised weight loss program.

    Key Definitions

    Stemmer sign
Definition (Stemmer sign)Physical exam sign used to distinguish lymphedema from lipedema; negative Stemmer sign is when the skin at the base of the second toe or second finger can be lifted
Interpretation supporting lipedemaNegative Stemmer sign (skin can be lifted) supports lipedema unless coexisting lymphedema is present
Contrast with lymphedemaA positive Stemmer sign (inability to lift skin at base of digit) is characteristic of lymphedema, which may coexist in advanced lipedema
    Complex Decongestive Therapy (CDT)
Core components of CDTManual lymphatic drainage, compression therapy, skin care, and therapeutic exercise
PurposeConservative approach to manage symptoms of lipedema and improve lymphatic function and tissue health

    Clinical Background

Lipedema is a chronic, progressive disorder characterized by bilateral, symmetric accumulation of adipose tissue in the extremities, most commonly affecting women and frequently misdiagnosed as lymphedema or obesity. Clinical features include disproportionate fat deposition of the limbs with relative sparing of hands and feet (the "cuff" phenomenon), subcutaneous nodules, easy bruising, pain and tenderness on palpation, and a typically negative Stemmer sign unless lymphedema coexists. Conservative measures such as complex decongestive therapy, compression, manual lymphatic drainage, and weight management may provide symptom relief, but when symptoms persist and meet the policy criteria despite conservative care, suction-assisted lipectomy is considered as a treatment option to reduce pain, improve function, and address deformity.

  1. Document duration and response to each conservative modality; evidence of continued symptoms and functional impairment after the trial.
  2. Denial Risk

Required Clinical Documentation and Denial Risk

Requests lacking documentation that all required clinical criteria are met may be denied. Required documentation includes medical records and photographs showing chronic/persistent lipedema complications refractory to conservative therapy, evidence of functional impairment (difficulty ambulating or performing activities of daily living), pain/tenderness on palpation, negative Stemmer sign (unless coexisting lymphedema), subcutaneous nodules, absence of pitting edema (unless coexisting lymphedema), bilateral/symmetrical fat distribution findings, and tendency to bruise.

    • Medical records and photographs documenting at least one chronic, persistent complication refractory to conservative therapy for at least six consecutive months.
    • Documentation of failure to respond to three consecutive months of conservative treatment.
    • Explicit documentation of each required clinical finding (functional impairment; pain/tenderness; negative Stemmer sign or explanation if lymphedema coexists; subcutaneous nodules; absence of pitting edema or explanation if lymphedema coexists; lack of improvement with elevation; tendency to bruise).
    • If documentation is incomplete or criteria are not all met, the request may be denied.
    Documentation Required

    Insufficient Clinical Criteria Documentation

If submitted records do not clearly show that every policy-specified criterion is satisfied, including conservative therapy trial and the required clinical signs, the authorization may be denied. Providers should attach operative/procedure coding (CPT) and current clinical notes and photos to avoid delay or denial.

    • Insufficient clinical criteria documentation (missing or incomplete evidence of functional impairment, pain/tenderness, negative Stemmer sign, subcutaneous nodules, absence of pitting edema, refractory complications, or conservative therapy trial) may result in denial.
    • Include relevant CPT codes (e.g., 15877, 15878, 15879) with the request and supply complete medical records and photographs.
Role before surgeryCDT and related conservative measures must be attempted and documented (see conservative therapy duration) prior to consideration of liposuction
