OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    • MCG S-864 pathway: The member meets the outlined criteria in MCG S-864, Mastectomy, Complete, with Tissue Flap Reconstruction.
    • Congenital deformity: A congenital deformity affects the member’s physical and psychological wellbeing.
    • Severe fibrocystic breast disease limits the member’s function.
    • Unintentional trauma or injury: The reconstruction is required because of unintentional trauma or injury.
    • Unintentional postsurgical complication: An unintentional complication occurred after breast surgery for a nonmalignant condition, such as pain, irritation, bleeding, discharge, or a complication causing difficulty with lactation.

    Post-Reconstruction Complications

Treatment of physical complications following breast reconstruction is medically necessary.

Physical complications after reconstruction: Treatment of physical complications, including lymphedema, following breast reconstruction is medically necessary.

See the Women’s Health and Cancer Rights Act for additional information.

Breast reconstruction procedures considered experimental, investigational, or unproven for the specific indication are not covered.

Procedures considered cosmetic in nature are not covered, including procedures intended to address natural changes associated with aging or weight loss or gain.

Lipectomy for donor-site symmetry is not covered.

Suction lipectomy or ultrasonically assisted suction lipectomy (liposuction) is not covered when performed to correct surgically induced donor-site asymmetry, such as trunk or extremity asymmetry resulting from one or more flap breast reconstruction procedures.

Coverage of nonsurgical alternatives is limited to external breast prostheses and mastectomy bras following mastectomy or breast-conserving surgery. All other indications are considered not medically necessary.
