    POLICY UPDATE CHANGES

The policy added coverage criteria for relapsed or refractory B-cell acute lymphoblastic leukemia.

The mantle cell lymphoma criteria were revised to require at least one measurable lesion and prior treatment with a covalent BTK inhibitor.

The policy changed infection screening language to require that members have been or will be screened for HBV, HCV, and HIV.

The ALL criteria distinguish relapsed or refractory Philadelphia chromosome-negative disease from Philadelphia chromosome-positive disease requiring prior tyrosine kinase inhibitor therapy.

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Therapy Overview

Tecartus (brexucabtagene autoleucel) is a CD19-directed genetically modified autologous T-cell immunotherapy. A patient’s T cells are harvested and genetically modified outside the body to express a chimeric antigen receptor, expanded, and then reinfused to recognize and eliminate CD19-expressing target cells. The policy addresses adults with relapsed or refractory mantle cell lymphoma and B-cell acute lymphoblastic leukemia.
