    Coverage Criteria

    Mechanical Insufflation-Exsufflation (E0482) - Medically Necessary

Covered when ALL of the following are met:

Mechanical insufflation-exsufflation criteria: 1) Presence of neuromuscular or chest wall disease (eg, amyotrophic lateral sclerosis, congenital muscular dystrophies, Duchenne muscular dystrophy, multiple sclerosis, post-poliomyelitis, spinal cord injury, spinal muscle atrophy); 2) The condition causes significant impairment of chest wall and/or diaphragmatic movement resulting in inability to clear retained secretions; 3) Inadequate response or intolerance to chest percussion and postural drainage; 4) No bullous emphysema, pneumomediastinum, or pneumothorax.

High Frequency Chest Compression (E0483) - Medically Necessary

Covered when ANY of the following clinical criteria is met:

HFCWO criteria: 1) Cystic fibrosis with failure, intolerance, or contraindication to home chest physiotherapy, or when home chest physiotherapy cannot be provided; 2) Diagnosis of bronchiectasis confirmed by high-resolution, spiral, or standard CT characterized by either: a) daily productive cough for at least 6 continuous months, or b) frequent (eg, more than 2 per year) exacerbations requiring antibiotic therapy.

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Mutual Exclusion / Limitations

Device exclusivity: It is not reasonable and necessary for a member to use both a high frequency chest compression device and a mechanical insufflation-exsufflation device.

Mechanical insufflation-exsufflation devices are covered only for the specific indications listed under the mechanical insufflation-exsufflation criteria. Mechanical insufflation-exsufflation devices for any indication not listed are not covered or reimbursable. Additionally, the policy specifies that the Volara device is not approved for outpatient use; outpatient claims for the Volara device may be denied.

High-frequency chest compression (HFCWO) is medically necessary only for the diagnoses and clinical presentations enumerated in the policy. Chronic bronchitis and chronic obstructive pulmonary disease (COPD) without a confirmed diagnosis of bronchiectasis do not meet criteria for HFCWO and therefore are not eligible for coverage under this policy.

    Provider Actions & Requirements

    Prior Authorization

Prior authorization required for E0482 and E0483

Coverage and prior authorization apply to HCPCS codes E0482 (mechanical insufflation-exsufflation) and E0483 (high-frequency chest compression); authorization will be granted only when the applicable clinical criteria for the requested device are met.

    • HCPCS E0482 — mechanical insufflation-exsufflation device: covered when all listed clinical criteria are met (see policy).
    • HCPCS E0483 — high frequency chest compression device: covered when any listed clinical criteria are met (see policy).
    Step Therapy

Conservative therapy required before device use

Conservative chest physiotherapy must be tried and found ineffective or not tolerated before authorization: members need an inadequate response or intolerance to conventional chest percussion and postural drainage prior to mechanical insufflation-exsufflation; for HFCWO, failure, intolerance, or contraindication to home chest physiotherapy is required for cystic fibrosis.

    • Mechanical insufflation-exsufflation (E0482): member has an inadequate response or intolerance to chest percussion and postural drainage.
    • HFCWO (E0483) for cystic fibrosis: failure, intolerance, or contraindication to home chest physiotherapy, or it cannot be provided.
    Documentation Required

CMN required for continued HFCWO rental

For continued rental of a high-frequency chest wall oscillation (HFCWO) device in a residential setting after the trial period, include a Certificate of Medical Necessity (CMN) attesting to device effectiveness during the trial and prior rental periods and documenting any change in therapy and recommendation for additional rental or purchase.

    • CMN must attest to effectiveness of the device during the trial period and every previous rental period.
    • If applicable, CMN must specify any change in duration or frequency of therapy and recommend additional rental or purchase.
    Denial Risk

Mutually exclusive device coverage — do not use both devices

Concurrent use of both a high frequency chest compression device and a mechanical insufflation-exsufflation device is considered not reasonable and necessary and may be denied.

    • Do not request or bill for both device types for the same member concurrently; coverage is limited to one airway clearance device type as indicated.
    Denial Risk

Volara device not approved for outpatient use

The Volara device is not approved for outpatient use; outpatient claims for the Volara may be denied.

    • Do not submit outpatient claims for the Volara device; use is restricted per policy.

    Background

The airways normally clear secretions by mucociliary transport and coughing; healthy individuals produce roughly 10–100 mL of airway secretions per day. Diseases that impair mucociliary function or cough strength — for example, neuromuscular disorders, chest wall disease, cystic fibrosis, bronchiectasis, and advanced COPD — can lead to retained secretions and increased risk of infection and respiratory compromise. Mechanical insufflation-exsufflation devices assist patients with weak cough by delivering positive pressure to the airway followed by rapid negative pressure to simulate an effective cough and facilitate expectoration. High-frequency chest compression (HFCWO) devices use an inflatable vest connected to an air-pulse generator that rapidly inflates and deflates the vest (up to about 20 times per second), compressing and releasing the chest wall to mobilize secretions; HFCWO is indicated in settings such as cystic fibrosis or CT-confirmed bronchiectasis when other conservative measures fail or are not possible.

    Definitions

inv-16: High Frequency Chest Compression Device
DefinitionAn inflatable vest connected by tubes to a small air-pulse generator that rapidly inflates and deflates the vest, compressing and releasing the chest wall up to 20 times per second.

MechanismRapid inflation/deflation produces external chest compression to mobilize secretions
    inv-17: Mechanical Insufflation-Exsufflation Device
DefinitionA device with a facemask that covers the nose and mouth, allowing air to be pumped into the lungs and then rapidly evacuated to facilitate the expulsion of secretions.
Primary componentsFacemask (nose and mouth); insufflation/exsufflation airflow generator
MechanismCycles positive pressure insufflation followed by rapid negative pressure exsufflation to augment cough and clear secretions
