Adaptive Seating Systems - Medical Necessity

Covered when ALL of the following are met:

Prescriber and documentation: A signed prescription valid for 1 year is written by a physician, advanced practice registered nurse, or physician assistant with an established relationship and appropriate certifications or training following a completed face-to-face encounter.

Sitting safety and need: The beneficiary cannot safely sit in conventional seating due to a medical condition, modifications to the beneficiary's current seating cannot meet their needs, and at least one of the following is present: (a) significant head and/or trunk instability or weakness with decreased motor control; (b) requires external support to maintain an upright position and proper body alignment; (c) has no functional protective or righting reaction; (d) must be in an upright supported position for safe and effective feeding and would otherwise have to be held by the caregiver for feeding; (e) severe seizure activity; or (f) orthopedic condition resulting in significant bony fragility or significant fracture.

Therapist evaluation and trial: An independent physical therapist (PT) or occupational therapist (OT) experienced in an appropriate specialty completed an evaluation documenting the member can benefit from and safely use the requested item; a trial of the equipment was completed; caregiver education with demonstrated safe use was provided; improvement of function due to the equipment is documented; and other equipment used by the member is listed (eg, wheelchairs, standers, gait trainers, other seating).

Therapist must be independent from the equipment vendor

Home safety and care plan: The beneficiary can safely use the AdSS in the home setting, and any person-centered service plans for the beneficiary, when applicable, list use of the AdSS.

Item description: A complete description of the item requested is provided including manufacturer, model/style, size, all bundled components, any accessories or not-included components, and an itemization of all charges.

Accessories - Medical Necessity

Accessories are considered medically necessary when providing additional functional support than the chair alone and ANY of the following are met:

Accessory indications: Cannot maintain head control in an upright position; unable to perform a functional weight shift requiring relief to prevent pressure injuries; needs additional trunk support that is not provided by the chair alone; (other supported needs such as those related to changes in muscle tone, digestion/GERD management, or independence in transfers/repositioning should be documented as applicable).

Any one condition sufficient

Adaptive Seating Systems (AdSS) explicitly exclude certain items and situations. AdSS does not include adaptive desks or adaptive strollers, nor will any item be covered that fails to meet the policy's medical necessity criteria. Requests for equipment that duplicates the function of an existing AdSS will not be reviewed for medical necessity and are excluded.

Coverage of an AdSS requires selection of the lowest cost alternative that adequately addresses the beneficiary's health condition. The AdSS must demonstrably benefit the beneficiary and not be provided for caregiver, family member, or provider convenience. Waiver codes for adaptive seating are only considered after documentation that conventional DME options have been exhausted.

Items that are duplicative or primarily intended for caregiver or facility convenience are not covered. Specifically, equipment with the same function as an existing AdSS will be excluded, and any item that does not meet the policy's listed medical necessity criteria (including those serving only convenience needs) is not medically necessary.

Provider Requirements and Authorization

Prior Authorization

Prior authorization required — medical necessity review before purchase/delivery

A review of medical necessity must occur prior to purchase or delivery; prior authorization will be issued only when the request meets all listed clinical criteria, including a valid signed prescription, independent PT/OT evaluation with trial and caregiver education, and complete item details.

Authorization requires meeting ALL clinical criteria in the policy (prescriber, PT/OT evaluation and trial, home safety, item description).

Step Therapy

Waiver/step requirement — DME options must be exhausted first

Waiver codes for adaptive seating will only be considered after documentation that conventional DME options have been attempted and exhausted.

Provide evidence that lower-cost DME options were tried and found inadequate before requesting waiver codes.

Submit complete supporting documentation with the authorization request: a detailed item description, a signed prescription valid for one year, and an independent PT/OT evaluation including trial results and caregiver education.

Signed prescription valid for 1 year from an appropriate provider (physician, APRN, or PA) with a completed face‑to‑face encounter.
Independent PT/OT evaluation documenting benefit, safe use, completed trial, caregiver education with demonstrated safe use, functional improvement, and listing of other equipment used.
Documentation that the beneficiary can safely use the AdSS in the home and that person‑centered service plans list AdSS when applicable.

Denial Risk

Exclusions that trigger denial — duplicative items and excluded categories

Requests will be denied if the item is excluded by policy — including adaptive desks or adaptive strollers, duplicative equipment, or any item that does not meet the policy's medical necessity criteria.

Adaptive desks and adaptive strollers are explicitly excluded.
Duplicative equipment with the same function as an existing AdSS will not be reviewed and is excluded.
Items that do not meet the medical necessity criteria listed in this policy are not medically necessary and can be denied.

Background and Clinical Context

Conditions such as cerebral palsy, developmental delays, and Down syndrome can impair sitting posture, trunk and head control, and functional participation in daily activities. Adaptive Seating Systems are individualized devices designed to support posture, enable upright sitting, improve safety, and facilitate engagement in activities of daily living for individuals with these clinical impairments.
