    POLICY UPDATE CHANGES

Policy explicitly defines separate medically necessary criteria for open SIJ fusion and percutaneous SIJ fusion/stabilization.

Introduced requirement that percutaneous SIJ fusion requires diagnostic confirmation via two image-guided intra-articular SIJ blocks with ≥50% pain reduction.

    6Conservative therapy duration (months)
≥50%Required pain reduction after SIJ block

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. Coverage Criteria for Sacroiliac Joint Fusion

Open SIJ fusion - medically necessary indications

Covered when ALL of the following are met

Open SIJ fusion medically necessary: 1. At least ONE of the following applies: a) post-traumatic injury of the SIJ (eg, following pelvic ring fracture); b) adjunctive treatment for sacroiliac joint infection (eg, osteomyelitis, pyogenic sacroiliitis)/sepsis; c) management of sacral tumor (eg, partial sacrectomy); d) part of a multisegmental long fusion construct for correction of spinal deformity (eg, idiopathic or neuromuscular scoliosis); e) prior percutaneous SIJ fusion has failed. 2. Recent (within 6 months) plain radiographs and/or cross-sectional imaging (CT or MRI) demonstrate localized SIJ pathology.

Must meet both the 'one of' complex-indication list and recent imaging requirement.

Percutaneous SIJ fusion/stabilization - medically necessary criteria

Covered when ALL of the following are met

Anatomic and symptomatic localization: 1. Presence of non-radiating pain that is caudal to the lumbar spine (L5) and localized over the posterior SIJ and consistent with SIJ pain. 2. SIJ pain rating of at least 5 on a 0–10 scale. 3. Localized tenderness with palpation of the posterior SIJ in the absence of tenderness of similar severity elsewhere (eg, greater trochanter, lumbar spine, coccyx) and no other obvious pain sources on exam. 5. Positive response to either the thigh thrust test OR the compression test. 6. Positive response to at least two of the following provocative tests: Gaenslen's test, distraction test, Patrick's (FABER) test. 9. Diagnostic imaging (plain radiographs and CT or MRI of the SIJ, plain radiographs of the ipsilateral hip, and CT or MRI of the lumbar spine) excludes destructive lesions, inflammatory arthropathy, ipsilateral hip osteoarthritis, or lumbar neural compression that would account for symptoms.pain >=5/10

Physical exam and imaging localization requirements.

Conservative therapy and injections: 4. Failure of conservative therapy demonstrated by: a) documentation of at least 6 months of active conservative therapy within the past 12 months OR documented inability to complete active therapy due to contraindication, increased pain, or intolerance; AND b) documentation of at least 6 months of inactive conservative therapy within the past 12 months. 10. Failure of a trial of at least two therapeutic intra-articular SIJ corticosteroid injections.6 months

Both active and inactive conservative therapy durations required, and two failed therapeutic injections.

Diagnostic confirmation: 7. Diagnostic confirmation that the SIJ is the pain generator by at least a 50% reduction in pain for the expected duration of the anesthetic following image-guided, contrast-enhanced intra-articular SIJ block using local anesthetic performed on two separate occasions.>=50%

Two separate image-guided, contrast-enhanced intra-articular SIJ blocks required.

Exclude generalized pain disorders: 8. Exclusion of generalized pain behavior or generalized pain disorders (eg, somatoform disorder, fibromyalgia) as the primary etiology of the member's pain.

Generalized pain disorders must be excluded prior to percutaneous SIJ fusion.

Open sacroiliac (SIJ) fusion is considered not medically necessary when performed for indications that are not explicitly listed as covered in this policy. Examples of excluded indications include mechanical low back syndrome, sacroiliac joint syndrome, degenerative sacroiliac joint, and situations where neural compression identified on MRI or CT correlates with the member's symptoms or indicates a more likely source of pain (for example radicular pain).

Percutaneous SIJ fusion is considered not medically necessary when the procedure is requested for indications not outlined as covered in this policy. Specific exclusions include systemic arthropathy (for example ankylosing spondylitis or rheumatoid arthritis), generalized pain behavior or disorders (eg, somatoform disorder, fibromyalgia), active infection, tumor, or fracture, acute traumatic SIJ instability, and cases where neural compression on MRI or CT correlates with the member’s symptoms or indicates another more likely pain source.

Any open or percutaneous SIJ fusion procedure requested for an indication outside the specific covered lists in this policy is considered not medically necessary and may be denied. Providers should ensure the requested indication matches the policy’s listed medically necessary criteria before submitting authorization requests.

    Diagnostic and Coding Requirements

inv-06: Diagnostic SIJ block pain reduction
Diagnostic pain reduction threshold>= 50% reduction in pain for the expected duration of the anesthetic following image-guided, contrast-enhanced intra-articular SIJ block
Number of diagnostic blocks requiredTwo separate image-guided, contrast-enhanced intra-articular SIJ blocks performed on separate occasions
Type of anesthetic response documentedPain reduction measured for the expected duration of the local anesthetic used
    inv-07: Pain severity threshold
Pain severity thresholdSIJ pain at least 5 on a 0-10 numeric pain scale
Localization requirement linked to thresholdPain must be non-radiating, caudal to L5, and localized over the posterior SIJ

Prior Authorization, Documentation, and Provider Responsibilities

    Prior Authorization

Prior authorization must document that all listed criteria are met

Prior authorization must show that every applicable coverage criterion for the requested procedure is met before performing percutaneous or open SIJ fusion. This includes documentation of required imaging, trials of conservative therapy, relevant physical exam findings, and diagnostic SIJ blocks when indicated.

    • Demonstrate recent (within 6 months) plain x‑rays and/or CT or MRI showing localized SIJ pathology (open fusion) or required SIJ/hip/lumbar imaging (percutaneous).
    • Provide evidence of required physical exam localization and positive provocative tests for percutaneous SIJ fusion.
    • Include results of diagnostic image‑guided, contrast‑enhanced intra‑articular SIJ blocks when the policy requires them.
    Step Therapy

Document 6 months of conservative therapy and failure of ≥2 therapeutic SIJ injections

Document failure of conservative therapy with both active and inactive modalities for the required durations and document failure of at least two therapeutic intra‑articular SIJ corticosteroid injections before percutaneous SIJ fusion will be considered.

    Definitions and Conservative Therapy Elements

    inv-13: Conservative therapy / HEP
Definition of Conservative TherapyA multimodality plan of care including active therapies (physical therapy, occupational therapy, physician‑supervised home exercise program, chiropractic care) and inactive therapies (rest, ice/heat, medical devices, TENS, pharmacotherapy)
HEP requirementsA 6‑week program with an exercise prescription/plan plus documented follow-up after completion, or documentation of inability to complete due to a stated physical reason (eg, increased pain, inability to perform exercises)
Timing for coverage considerationDocumentation of at least 6 months of active conservative therapy within the past 12 months (or documented inability to complete) and at least 6 months of inactive conservative therapy within the past 12 months
    inv-14: TENS
TENS definitionTranscutaneous Electrical Nerve Stimulator: a device delivering electrical current through skin electrodes to reduce pain perception by inhibiting afferent pain impulses and/or stimulating endorphin release

    Clinical Background

The sacroiliac joints connect the sacrum to the ilia and allow minimal motion; dysfunction of the SIJ can account for a substantial portion of low back and buttock pain and may be mistaken for discogenic or radicular pain. Causes include inflammatory arthropathy, infection, tumor, fracture, or other structural changes; these alternate etiologies must be excluded by imaging (plain radiographs and CT or MRI of the SIJ, ipsilateral hip radiographs, and lumbar spine imaging) prior to considering percutaneous SIJ fusion.

    Policy Revision History

    2026-06-01material_updateLatest

Policy clarified separate medically necessary criteria for open versus percutaneous SIJ fusion (distinct indication lists) and added a diagnostic confirmation requirement for percutaneous SIJ fusion (two image-guided intra-articular SIJ blocks with ≥50% pain reduction).
