OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Digital EEG Spike Analysis — Coverage Criteria

Digital EEG spike analysis coverage criteria

Covered when ALL of the following are met:

    ALL of the following

    • Digital EEG spike analysis (CPT 95957) is performed in conjunction with EEG for topographic voltage and/or dipole analysis.
    • The service is performed specifically for pre-surgical evaluation with video-EEG long-term monitoring.
    • AND the member has ONE of the following clinical indications:

      • Intractable epilepsy.
      • Intracranial injuries.
      • Concussion.
All other indications for digital EEG spike analysis are not covered and not reimbursable.
Claims for CPT 95957 must be supported by accurate and complete documentation submitted by the provider; lack of documentation may result in denial.
Operational notes: Policy does not guarantee authorization or payment; member benefits, eligibility, medical necessity, and contractual terms also apply. Mental Health Parity applies where relevant.

    Provider Responsibilities and Billing Guidance

    Documentation Required

Provide complete, accurate documentation or risk denial

The submitting provider must supply accurate and complete documentation to substantiate the coding on claims; failure to do so may result in denial if the documentation does not support the claim submission.

    • Documentation must substantiate the coding of the claim.
    • Incomplete or inaccurate documentation may lead to claim denial.

    Definitions and Terminology

    Digital EEG
    TermDigital EEG
DefinitionSystems that allow for the recording, analysis, and storage of EEG data; often include software tools that can detect spikes and other abnormal brain patterns and help visualize, quantify, and interpret the data.
Common useSupports spike detection and quantitative analysis used in diagnosis and pre-surgical evaluation for epilepsy.
    Ambulatory EEG
    TermAmbulatory EEG (aEEG)
DefinitionPortable recording of EEG outside the hospital or clinic setting; can include simultaneous video recording and often uses computer software to detect seizures and interictal epileptiform discharges (IED) to aid interpretation.
SettingsPerformed in the home or outpatient settings to capture events not seen during routine in‑lab monitoring.
    Dipole Analysis
    TermDipole Analysis
DefinitionLocalization and quantification of the sources of electrical activity in the brain to help identify the origins of conditions such as epilepsy.
Clinical roleUsed with topographic voltage analysis during pre‑surgical video‑EEG long‑term monitoring to localize epileptogenic foci.
    Epileptiform Activity
    TermEpileptiform Activity
DefinitionCortical hyperexcitability manifested as spikes, sharp waves, or spike‑and‑wave patterns that are associated with increased seizure risk and presence of an epileptic network.
Clinical significanceIdentification of epileptiform activity on EEG supports diagnosis of epilepsy and guides surgical evaluation and management.
