Coverage Criteria for Trigger Point Injections

    inv-01: Initial Trigger Point Injections

Covered when ALL of the following are met:

Initial TPI criteria: 1) New (acute or subacute) localized pain occurring in the last 3 months; 2) Member refractory or intolerant of conservative therapies for at least 1 month, including at least one of: bed rest, active exercise, ultrasound, range of motion, heating or cooling treatments, massage; 3) TPIs are part of an overall conservative management plan including at least one of: physical therapy, occupational therapy, physician supervised home exercise program (HEP), manipulative therapy; 4) Pharmacotherapies are being administered including at least one of: NSAIDs, muscle relaxants, non-narcotic analgesics, antidepressants; 5) Member has a diagnosis appropriate for TPI and ALL of the following are documented in the medical record: evaluation with history and physical leading to trigger point diagnosis; reasons for selecting TPI; affected muscle(s); muscle(s) injected and number of injections; frequency of injections required; name of medication used; results of any prior treatment; corroborating evidence that the injection is medically necessary.

From policy section D.I.A (Initial Injections)

    inv-02: Subsequent Trigger Point Injections

Covered when ALL of the following are met:

Subsequent TPI criteria: 1) Repeated injections are reasonable and medically necessary; 2) No more than 8 dates of service per calendar year per member may be reimbursed; 3) Injections may be repeated only with documented positive results to the most recent TPI at the same anatomic site, including at least 50% improvement in pain, functioning, and activity tolerance.<=8 dates per calendar year

    From policy section D.II

Localization techniques to image or otherwise identify trigger point anatomic locations are not indicated and will not be covered for payment when associated with trigger point injection procedures. Providers should not submit charges for imaging solely to localize trigger points for the purpose of performing TPIs.

Dry needling is not an acceptable alternative to trigger point injections performed by an appropriately licensed clinician. Claims for dry needling in lieu of documented TPIs will be denied as not an approved substitute under this policy.

Use of imaging localization techniques and dry needling in place of documented trigger point injections are explicitly not covered and are not acceptable alternatives to meet medical necessity for TPI. Documentation substituting these approaches for a documented TPI will not satisfy policy requirements for coverage.

    Procedure and Coding Guidance

Procedure/Coding guidance (no specific CPT codes listed)mixed
    No codes listed
    inv-07: Repeat injection success threshold
Repeat injection success threshold>= 50% improvement in pain, functioning, and activity tolerance
When requiredDocumentation must show positive results from the most recent injection at the same anatomic site
PurposeJustifies medical necessity for subsequent trigger point injections

    Provider Requirements and Documentation

    Prior Authorization

Obtain authorization only when policy criteria and documentation are complete

Trigger point injections are considered medically necessary only when the policy criteria are met; providers must ensure the medical record documents the required elements before requesting authorization.

    • Ensure all documented elements listed under policy section 5 (evaluation/history/physical, reason for TPI, affected muscles, muscles injected and number, frequency, medication name, prior treatment results, corroborating evidence).
    • Confirm conservative therapy trial and other coverage criteria are met prior to authorization request.
    Step Therapy

Document ≥1 month trial of conservative therapy before initial TPI

Confirm the member has been refractory or intolerant of conservative therapies for at least 1 month and that TPIs are incorporated into an overall conservative management plan before initial injection.

    • Conservative therapies include at least one of: bed rest; active exercise; ultrasound; range of motion; heating/cooling treatments; massage.

    Conservative Care Requirements Before TPI

inv-19: Documented prior conservative therapy trial and inclusion of TPI within a conservative care plan

Conservative therapy requirement: Member must be refractory or intolerant of conservative therapies for at least 1 month, with at least one therapy from: bed rest, active exercise, ultrasound, range of motion, heating or cooling treatments, or massage. TPIs must be included as part of an overall conservative care plan including at least one of: physical therapy, occupational therapy, physician supervised home exercise program (HEP; defined as a 6-week program when used), or manipulative therapy.>= 1 month refractory to conservative therapies; HEP = 6 weeks

    From policy conservative treatment requirements

    Frequency and Quantity Limits

inv-20: Trigger point injections — Frequency limit
Maximum reimbursable dates of serviceNo more than 8 dates of service per calendar year per member
    Applies toSubsequent trigger point injections
RationaleLimits frequency of repeat injections to those that are reasonable and medically necessary

    Imaging and Localization Requirements

    Note

Imaging localization not required/covered for TPIs

No additional imaging is required or indicated to localize trigger points for TPI; such localization techniques are not covered.

    Definitions

    inv-15: Acute Pain
DefinitionPain that lasts less than 4 weeks.
ContextUsed to determine applicability of 'new' (acute or subacute) localized pain criteria for initial TPI
Policy sectionC. Definitions — Acute Pain
    inv-16: Subacute Pain
DefinitionPain that has lasted between 4 weeks and 12 weeks.
ContextClassified as subacute for evaluating candidacy for initial trigger point injections
Policy sectionC. Definitions — Subacute Pain

    Background

Myofascial trigger points are identified clinically as hyper‑irritable foci within skeletal muscle that produce referred pain and are diagnosed by history and directed physical examination rather than by laboratory or imaging tests. Trigger point injections deliver local anesthetic and/or corticosteroid into the affected muscle as part of a comprehensive, conservative pain management plan after failure or intolerance of initial conservative therapies.

    Services Not Covered

Not covered: Localization imaging techniques for trigger points and dry needling as an alternative to trigger point injections. Both the use of imaging to localize trigger points and substitution of dry needling for documented TPIs are explicitly excluded from payment under this policy.
