    Coverage Criteria for Paricalcitol (generic)

    Initial approval criteria

Covered when ALL of the following are met:

Member has CKD stage: Member has Stage 3 (GFR 30-59), Stage 4 (GFR 15-29), or Stage 5 (GFR <15 or dialysis)
iPTH meets stage-specific thresholds: Member has one of the following iPTH values: >70 pg/mL if Stage 3; >110 pg/mL if Stage 4; >300 pg/mL if Stage 5 on dialysis
Laboratory limits: Member has corrected serum calcium <9.5 mg/dL AND serum phosphorus <4.6 mg/dL
Prior therapy: Member has tried and failed or has contraindications to Rocaltrol (calcitriol)

Coverage Criteria for Paricalcitol (generic)Laboratory Thresholds and DefinitionsWhat Providers Must DoBackgroundClinical Definitions

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage is NOT provided when any of the laboratory or disease-stage limits required for approval are not met. Specifically, requests will be denied if the member's intact parathyroid hormone (iPTH) value is below the stage-specific threshold (< = threshold for that CKD stage), or if either the corrected serum calcium or serum phosphorus exceed the allowable limits. The required lab cutoffs are corrected calcium < 9.5 mg/dL and serum phosphorus < 4.6 mg/dL. Additionally, the member must have chronic kidney disease Stage 3, 4, or 5; absence of an eligible CKD stage will render the request non‑covered.

Use of paricalcitol is unsupported when the member does not meet the policy's CKD stage, laboratory, or prior‑therapy requirements. Specifically, the member must have Stage 3 (GFR 30–59), Stage 4 (GFR 15–29), or Stage 5 (GFR <15 or on dialysis), and an iPTH that meets the stage‑specific threshold (>70 pg/mL for Stage 3; >110 pg/mL for Stage 4; >300 pg/mL for Stage 5 on dialysis). The member must also have documented corrected calcium < 9.5 mg/dL and serum phosphorus < 4.6 mg/dL, and evidence of a trial and failure of, or a contraindication to, Rocaltrol (calcitriol). Requests lacking any of these requirements are not supported.

    Laboratory Thresholds and Definitions

    iPTH thresholds by CKD stage
    Stage 3 iPTH threshold> 70 pg/mL
    Stage 4 iPTH threshold> 110 pg/mL
    Stage 5 (on dialysis) iPTH threshold> 300 pg/mL
    Corrected serum calcium
    Corrected serum calcium requirement< 9.5 mg/dL
Source contextRequired as part of laboratory limits for approval
Related denial triggerNo coverage if corrected calcium exceeds 9.5 mg/dL
    Serum phosphorus
    Serum phosphorus requirement< 4.6 mg/dL
Source contextRequired as part of laboratory limits for approval
Related denial triggerNo coverage if serum phosphorus equals or exceeds 4.6 mg/dL

    What Providers Must Do

    Prior Authorization

Prior authorization required; approval length Lifetime

Paricalcitol (generic) requires prior authorization. When all coverage criteria are met, approval length is Lifetime (through 12/31/2039).

    Step Therapy

Step therapy: trial of calcitriol required

Member must have tried and failed, or have a documented contraindication to, Rocaltrol (calcitriol) before paricalcitol will be approved.

    Documentation Required

    Required documentation for prior authorization

Prior authorization requests must include documentation of the member's CKD stage, a recent iPTH value meeting the stage‑specific threshold, corrected serum calcium and serum phosphorus values within required limits, and evidence of trial and failure of—or contraindication to—calcitriol (Rocaltrol) as applicable.

    • CKD stage (Stage 3, 4, or 5)
    • iPTH value meeting: >70 pg/mL (Stage 3); >110 pg/mL (Stage 4); >300 pg/mL (Stage 5 on dialysis)
    • Corrected calcium < 9.5 mg/dL
    • Serum phosphorus < 4.6 mg/dL
    • Documentation of trial and failure or contraindication to Rocaltrol (calcitriol)
    Denial Risk

    Denial triggers and documentation gaps

Requests will be denied/unsupported if the member does not meet the CKD stage criteria, the stage‑specific iPTH thresholds, or if corrected calcium or phosphorus exceed the stated cutoffs, or if required documentation (including trial/contraindication to calcitriol) is not provided.

    • Member not Stage 3–5 as specified
    • iPTH below required threshold for the member's CKD stage
    • Corrected calcium ≥ 9.5 mg/dL
    • Serum phosphorus ≥ 4.6 mg/dL
    • Missing documentation of trial and failure or contraindication to calcitriol (Rocaltrol)

    Background

Paricalcitol is a vitamin D analog used to manage secondary hyperparathyroidism in patients with chronic kidney disease. This guideline defines eligibility for prior authorization by CKD stage and laboratory thresholds: eligible members have Stage 3–5 CKD with stage‑specific elevated iPTH (Stage 3 >70 pg/mL; Stage 4 >110 pg/mL; Stage 5 on dialysis >300 pg/mL), and must have corrected calcium <9.5 mg/dL and serum phosphorus <4.6 mg/dL. Prior authorization also requires documented trial and failure of, or contraindication to, calcitriol (Rocaltrol).
