    Coverage Criteria

    Indication-specific coverage criteria

Covered when ALL of the following are met for each indication pathway as specified below.

Heart failure (reduced ejection fraction, systolic): 1) Diagnosis of reduced ejection fraction (systolic) heart failure AND 2) Submission of medical records (e.g., chart notes) confirming intolerance or failure of an ACE inhibitor.

    Approval length: Lifetime (12/31/2039).

Hypertension: 1) Diagnosis of hypertension AND 2) Both of the following: a) Patient has tried maximum tolerated doses of both losartan and irbesartan; AND b) Patient has tried maximum tolerated doses of at least one medication from three of the following classes: calcium channel blockers (amlodipine, nifedipine, diltiazem, verapamil), beta blockers (metoprolol, carvedilol, atenolol), alpha blockers (prazosin, terazosin, doxazosin), thiazides (chlorthalidone, HCTZ), other (clonidine, spironolactone).

    Approval length: Lifetime (12/31/2039).

Migraine prevention: Diagnosis of migraine prevention as listed in the approval criteria.

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Approval length: Lifetime (12/31/2039).

Coverage for generic candesartan and generic candesartan/hydrochlorothiazide (HCT) is provided when the specific, indication-based criteria below are met. Approvals are granted only after review of the documentation required for each clinical pathway. The policy defines separate criteria for reduced ejection fraction (systolic) heart failure, hypertension, and migraine prevention; each pathway must meet all listed requirements to be approved. Approval length is lifetime when criteria are satisfied.

    Provider Actions & Authorization Requirements

    Prior Authorization

Prior authorization required; lifetime approval when criteria met

Prior authorization is required for Generic Candesartan and Generic Candesartan/HCT. When the approval criteria are met for heart failure, hypertension, or migraine prevention, approval length is Lifetime (12/31/2039). Medical records documenting prior therapy and intolerance/failure (when specified) must be submitted with the request.

    Step Therapy

    Step therapy requirements by indication

Step therapy must be documented per indication pathway: for heart failure (reduced ejection fraction) the request must document trial of — or intolerance to — an ACE inhibitor; for hypertension the patient must have tried maximum tolerated doses of both losartan and irbesartan and have tried maximum tolerated doses of at least one medication from three additional antihypertensive classes.

    • Heart failure pathway: trial or documented intolerance/failure of an ACE inhibitor (submit supporting records).
    • Hypertension pathway: trial of losartan and irbesartan at maximum tolerated doses; plus trials at maximum tolerated doses of at least one medication from three classes (calcium channel blockers, beta blockers, alpha blockers, thiazides, or other listed agents).
    Documentation Required

    Required medical records (chart notes)

Submit medical records (for example, chart notes) that confirm intolerance or therapeutic failure of the prior agent(s) when required by the indication (e.g., intolerance or failure of an ACE inhibitor for reduced ejection fraction heart failure).

    • Examples: clinic progress notes, hospitalization records, medication trial history documenting doses and reason for discontinuation, adverse event documentation.
    Denial Risk

Denial risk if documentation or prior therapy not met

Failure to provide the required, indication-specific documentation or to show required prior therapy trials may result in denial of the prior authorization request; if documentation is incomplete, provide the missing chart notes or medication trial records.

    • Denial triggers include missing documentation of reduced ejection fraction for heart failure, absence of ACE inhibitor trial/intolerance documentation (heart failure), or lack of documented trials of losartan and irbesartan plus three antihypertensive class trials (hypertension).
    • If denied for insufficient documentation, resubmit with the requested medical records confirming trials, maximum tolerated doses, and reasons for intolerance or failure.

    Background

Candesartan is an angiotensin II receptor blocker (ARB) used for conditions including systolic (reduced ejection fraction) heart failure, hypertension, and migraine prevention. In the heart failure pathway, candesartan is intended as an alternative for patients with intolerance or failure of an ACE inhibitor and requires submission of medical records documenting that intolerance or failure. For hypertension, the policy requires trials of other ARBs (specifically losartan and irbesartan at maximum tolerated doses) plus trials of at least one medication at maximum tolerated dose from three additional antihypertensive classes (for example calcium channel blockers, beta blockers, alpha blockers, thiazides, or other listed agents) before candesartan will be approved.
