OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage criteria and exclusions

    Initial therapy / Medical necessity

Covered when ALL of the following are met:

Main medical necessity criteria: Pain is at least 5 on a 0 to 10 rating scale that impacts quality of life or limits activities of daily living.
Absence of generalized pain behavior (e.g., somatoform disorder) or generalized pain disorders (e.g., fibromyalgia).
Failed a minimum of 6 months of intensive nonoperative treatment that includes medication optimization, activity modification, bracing, and active therapeutic exercise targeted at the lumbar spine, pelvis, sacroiliac joint, and hip (including a home exercise program).minimum 6 months

Includes at least 6 weeks of physical therapy unless documented intolerance.

Pain is caudal to the lumbar spine (L5 vertebra), localized over the posterior sacroiliac joint, and consistent with sacroiliac joint pain.
Physical examination demonstrates localized tenderness with palpation over the sacral sulcus (Fortin's point) in the absence of tenderness of similar severity elsewhere.
Positive response to a cluster of 3 provocative tests (e.g., thigh thrust, compression, Gaenslen, distraction, Patrick, posterior provocation).

    Imaging requirements

    • Imaging (pain radiographs and CT or MRI) of the sacroiliac joint excludes destructive lesions (e.g., tumor, infection) or inflammatory arthropathy.
    • Pelvis AP radiograph rules out concomitant hip pathology.
    • Lumbar spine CT or MRI rules out neural compression or other degenerative conditions that can cause low back or buttock pain.
    • Imaging of the sacroiliac joint indicates evidence of injury and/or degeneration.
Diagnostic SIJ injection: At least a 75% reduction in pain for the expected duration of the anesthetic following an image-guided, contrast-enhanced intra-articular sacroiliac joint injection on 2 separate occasions.>=75% on 2 separate occasions
Therapeutic injection trial: At least one therapeutic sacroiliac joint injection (e.g., corticosteroid) has been performed.
Applicability: These criteria apply specifically to minimally invasive sacroiliac joint fusion/stabilization using titanium triangular implants (e.g., iFuse).

    Experimental / Investigational

Investigational or not covered uses: Minimally invasive sacroiliac joint fusion/stabilization performed using devices other than titanium triangular implants is considered experimental/investigational. SIJ fusion for indications including but not limited to infection, tumor, or acute traumatic instability/fracture is considered experimental/investigational.

Per policy, these uses do not meet TEC criteria #2-5

Minimally invasive sacroiliac joint fusion/stabilization performed with devices other than titanium triangular implants (for example, cylindrical threaded devices or other non‑titanium systems) is considered experimental/investigational and is not covered under this policy. The policy notes that comparisons of these alternative devices with titanium triangular implants or with open surgery are currently insufficient to determine equivalent effectiveness, and efficacy for indications beyond those identified as medically necessary is unknown. Additionally, SIJ fusion for indications such as infection, tumor, or acute traumatic instability/fracture is considered investigational.

Coverage for minimally invasive sacroiliac joint fusion/stabilization varies by contract and line of business. Providers should confirm member‑specific benefits and limitations before applying the terms of this policy.

Minimally invasive SIJ fusion performed for reasons beyond the uses defined as medically necessary in this policy, or using non‑titanium triangular implants, is considered investigational and not medically necessary. The policy highlights that evidence is insufficient to support effectiveness of alternative devices or use outside the specified indications, so such procedures do not meet the policy's criteria for coverage.

Diagnostic thresholds and conservative therapy requirements

Diagnostic SIJ injection pain reduction threshold — ≥75% reduction on 2 separate occasions
Diagnostic pain reduction thresholdAt least 75% reduction in pain for the expected duration of the anesthetic following an image-guided, contrast-enhanced intra-articular SIJ injection
Required occasionsDemonstrated on 2 separate occasions (dual positive blocks)
Block type and guidanceImage-guided, contrast-enhanced intra-articular SIJ injections used diagnostically; no therapeutic intra-articular injections within 4 weeks prior to diagnostic blocks
Conservative therapy duration — Minimum 6 months of intensive nonoperative treatment (including ≥6 weeks PT)
Minimum durationPatients must have failed a minimum of 6 months of intensive nonoperative treatment
    Physical therapy requirement

Prior authorization, documentation, and provider responsibilities

    Prior Authorization

    Prior Authorization Available

Preauthorization may be required for this service. Providers should submit prior authorization requests online at provider.carefirst.com or call 1-866-773-2884 (1-866-PRE-AUTH). Failure to obtain required prior authorization may affect payment.

    • Submit requests online or by phone: provider.carefirst.com or 1-866-773-2884 (1-866-PRE-AUTH)
    Note

Coverage Varies — Verify Member Coverage

Coverage will vary by contract and by line of business. Providers must verify member-specific coverage and any prior authorization requirements with the member’s contract/line of business before scheduling or performing services.

    • Verify coverage and prior authorization requirements for each member prior to submitting requests

    Clinical background and epidemiology

Mechanical low back pain is common; approximately 5–10% of patients progress to chronic, treatment‑resistant pain, and among chronic low back pain patients, the sacroiliac joint is implicated in an estimated 10–30% of cases. SIJ pain can mimic lumbar or hip pathology, making diagnosis challenging. Image‑guided intra‑articular sacroiliac joint injections (diagnostic SIJ blocks) are used as the reference standard to confirm the SIJ as the pain source and to identify patients who may benefit from SIJ‑directed treatments.

    Key definitions

SIJ diagnostic block definition — image-guided anesthetic injection into the sacroiliac joint used diagnostically
DefinitionImage-guided anesthetic injection into the sacroiliac joint used diagnostically to confirm the SIJ as the source of pain
PurposeHelps identify patients likely to benefit from SIJ-directed treatment and is the reference standard for diagnosis
Performance considerationsShould be image-guided and contrast-enhanced; diagnostic blocks should not be performed under IV sedation unless specifically indicated
Fusion vs fixation — fusion encourages bone growth; fixation stabilizes with hardware
FusionProcedures that encourage bone growth to stabilize the joint (arthrodesis)
FixationStabilization using screws, rods, or other hardware without promoting bone growth
