Provider Actions — Authorization, Documentation, and Supplier Requirements

    Prior Authorization

    Prior authorization & rental guidance

Coverage for specialized manual wheelchairs is contingent on meeting the stated medical necessity criteria and supplier/assessment requirements. Rental of a standard manual wheelchair may be covered temporarily for the duration needed to repair a member-owned manual wheelchair that meets coverage guidelines.

    • Prior authorization required when coverage depends on meeting medical necessity and supplier/assessment requirements.
    • Rental rule: temporary rental of a standard manual wheelchair may be covered during repairs to a member‑owned device that meets coverage criteria.
    Step Therapy

    Conservative measures required (walker/cane trial)

Before considering coverage for a standard manual wheelchair, the limitations must not be safely resolved with the use of a properly fitted walker or cane.

    • Document attempts and why a walker or cane is not sufficient to resolve the mobility limitation.

    • Include evaluator's licensure/certification and statement that the evaluation documents medical necessity and required specialized features.
    • Supplier documentation must identify the RESNA-certified ATP and describe their direct, in-person participation.

    Background

Mobility impairment can result from congenital conditions, injury, or disease and may be temporary or permanent. Wheelchairs range from standard manual chairs to specialized bases and enhancements designed to meet needs related to stature, weight, activity level, or postural support. Clinical evaluation and proper fitting are essential to ensure the selected wheelchair meets functional needs and safety within the home environment.

    Not Covered

Not covered under this policy are manual wheelchairs intended for exclusive outdoor use and custom manual wheelchairs when the anticipated need is less than three (3) months. Additionally, when a manual wheelchair base is not covered, any associated accessories are likewise not covered. These exclusions align with the policy’s requirement that devices meet documented medical necessity for use in the home setting.
