OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria — Orthognathic Surgery

Medical necessity indications for orthognathic surgery

Covered when ALL of the following groups (A and B) are met as applicable:

Group A - Indications: Orthognathic surgery is medically necessary for correction of skeletal deformities of the maxilla and/or mandible associated with: (1) injury or trauma involving the oral cavity that require surgical intervention to restore normal body function; OR (2) correction of a congenital deformity identified at birth that is required to achieve normal body function and is not performed only to improve appearance (e.g., cleft lip/palate); OR (3) acquired anomalies following medically necessary surgical procedures when orthognathic surgery is required as part of reconstruction to restore masticatory function.
Group B - Morphologic and functional criteria: Correction of skeletal deformities (maxillary or mandibular) when clinical documentation supports these deformities cause significant dysfunction not correctable with dental/orthodontic therapy alone. One or more anatomic discrepancy criteria must be present AND documentation of one or more functional impairments must be present.Anatomic thresholds: see numeric criteria below; Functional impairments: persistent chewing/swallowing difficulty evaluated by SLP; repeated soft tissue injury during mastication; malnutrition/weight loss (>=10 lb or >=5% body weight over 6 months); SLP-documented speech impairment attributable to skeletal deformity; persistent pain >1 year despite conservative therapy (diet modification, NSAIDs if not contraindicated, occlusal splint, physical therapy).

Presurgical dental therapy and orthodontia must have been attempted as appropriate.

Anatomic discrepancy - Anteroposterior: One or more of the following anteroposterior findings after necessary presurgical dental therapy and orthodontia: (a) maxillary/mandibular incisor horizontal overjet >= 5 mm or 0 to negative value; OR (b) anteroposterior molar relationship discrepancy >= 4 mm (norm = 0–1 mm).>= 5 mm incisor overjet OR >= 4 mm molar discrepancy

Two or more standard deviations from published norms where specified.

Anatomic discrepancy - Vertical: Vertical deformity criteria after necessary presurgical dental therapy and orthodontia: (a) vertical facial skeletal deformity two or more standard deviations from published norms for accepted skeletal landmarks; OR (b) open bite with: no anterior vertical overlap; OR unilateral/bilateral posterior open bite > 2 mm; OR deep overbite with impingement/irritation of opposing tissues; OR supraeruption of a dentoalveolar segment due to lack of occlusion.> = two standard deviations for landmarks or >2 mm posterior open bite where specified
Anatomic discrepancy - Transverse: Transverse deformity criteria after necessary presurgical dental therapy and orthodontia: (a) transverse skeletal discrepancy two or more standard deviations from published norms; OR (b) total bilateral maxillary palatal cusp to mandibular fossa discrepancy >= 4 mm, or unilateral discrepancy >= 3 mm, with normal axial inclination of posterior teeth.>=4 mm bilateral or >=3 mm unilateral
Anatomic discrepancy - Asymmetry: Anteroposterior, transverse, or lateral asymmetries greater than 3 mm with a concomitant occlusal asymmetry, following necessary presurgical dental therapy and orthodontia.> 3 mm
Functional impairment (one or more required): Clinical documentation must support one or more of the following as a direct result of the skeletal facial deformity after necessary presurgical dental therapy and orthodontia: (1) persistent difficulties with chewing and swallowing after neurologic and metabolic causes are ruled out, evaluated by a speech-language pathologist; (2) repeated damage to oral soft tissues during mastication; (3) malnutrition, failure to thrive, or significant weight loss (10 pounds or >=5% body weight over 6 months) secondary to the deformity; (4) SLP documentation that the skeletal deformity causes a speech impairment; (5) persistent pain attributable to the skeletal deformity that has persisted at least one year despite conservative treatment including diet modification, NSAIDs if not contraindicated, occlusal splint, and physical therapy.See functional thresholds above

Conservative treatments must have been tried for persistent pain for at least one year before surgery is considered.

Network Health considers orthognathic surgery to be cosmetic and not medically necessary when performed solely to correct unaesthetic facial features, regardless of any associated psychological disorder. Coverage determinations are governed by the member's coverage document and contract provisions, which take precedence over clinical policy.

Orthognathic surgery performed for impairments related to the production of speech involving muscular coordination is considered not medically necessary and is excluded from coverage under this policy.

Orthognathic surgery for the correction of temporomandibular joint (TMJ) disease or myofascial pain dysfunction is considered experimental/investigational and not medically necessary due to insufficient evidence in the medical literature to support effectiveness for these indications.

    Provider Actions & Documentation Requirements

    Prior Authorization

Obtain prior authorization and verify member coverage

Prior authorization is required per the member's plan contract; coverage determinations must reference the member's Certificate of Coverage/Evidence of Coverage/Summary Plan Description, and Medicare Advantage members follow CMS NCDs/LCDs as applicable.

    • Refer to the individual's coverage document first — the coverage document takes precedence over this clinical policy.
    • For Medicare Advantage members, NHIC follows Medicare National/Local Coverage Determinations.
    Step Therapy

Document trial of conservative treatments for ≥1 year

Conservative treatments must have been tried and failed for persistent pain for at least one year before surgery is considered; examples include diet modification, nonsteroidal anti-inflammatory medications (if not contraindicated), occlusal splint therapy, and physical therapy.

    • Diet modification (e.g., soft chew diet).

    Background

Orthognathic surgery is the surgical correction of abnormalities of the maxilla and/or mandible to restore normal body function when skeletal deformities cause significant functional impairment that cannot be adequately treated with dental or orthodontic therapy alone. Indications include congenital, developmental, traumatic, or acquired skeletal deformities that result in substantial masticatory dysfunction or other impairments.

Medical necessity is determined when documentation shows both relevant anatomic discrepancies (for example, predefined anteroposterior, transverse, vertical, or asymmetry thresholds) and functional impairments (such as persistent chewing/swallowing difficulty, repeated soft tissue injury during mastication, malnutrition/weight loss, speech impairment attributable to the skeletal deformity, or persistent pain despite conservative therapy). Presurgical dental therapy and orthodontics must be attempted as appropriate, and prior authorization is required per plan contract.

    • Pain history showing ≥1 year duration and failed conservative treatments.
    Denial Risk

Denial risk if criteria unmet or excluded indications present

Requests will be denied when the policy criteria are not met, or when surgery is cosmetic (correction of unaesthetic facial features), intended for speech production involving muscular coordination, or when orthognathic surgery is considered experimental/investigational for temporomandibular joint disease or myofascial pain; plan contract exclusions may also apply.

    • Orthognathic surgery for production of speech involving muscular coordination is not medically necessary.
    • Surgery to correct unaesthetic facial features is considered cosmetic and not covered.
