OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage Criteria — When Adjustable Cranial Orthoses Are Covered

    Medically necessary: positional plagiocephaly/brachycephaly

Covered when ALL of the following are met for positional plagiocephaly/brachycephaly:

Positional plagiocephaly/brachycephaly criteria: Individual is between 3 and 18 months old.age 3-18 months

Age range required to consider orthosis for deformational plagiocephaly/brachycephaly.

Positional plagiocephaly/brachycephaly criteria: Documented failure of conservative therapy (repositioning and physical therapy) of at least 2 months duration.conservative therapy >= 2 months

Conservative measures must be attempted and documented before orthosis fitting.

Positional plagiocephaly/brachycephaly criteria: Cephalic index at least two standard deviations above or below the mean for the appropriate gender and age.cephalic index >= 2 SD from mean

Cephalic index measured and compared to age- and gender-specific normative values per policy guidance.

    Medically necessary: postoperative craniosynostosis

Covered when the following condition is met for synostosis:

Post-cranial vault remodeling: Use of an adjustable cranial orthosis following cranial vault remodeling (open or endoscopic) surgery for craniosynostosis.postoperative status

Policy states evidence supports improvement in net health outcome after cranial vault remodeling; orthosis may facilitate remodeling and improve outcomes.

    Investigational indications

Investigational uses: 1) Use of an adjustable cranial orthosis for craniosynostosis in the absence of prior cranial vault remodeling surgery; OR 2) Use of an adjustable cranial orthosis for indications other than postoperative synostosis or persistent plagiocephaly/brachycephaly meeting the above criteria.investigational

Insufficient evidence to support general health benefit for these uses; claims for these indications risk denial.

Use of an adjustable cranial orthosis for craniosynostosis in the absence of prior cranial vault remodeling surgery is investigational. The policy states there is insufficient evidence to support a general conclusion concerning health outcomes or benefits for orthosis use for synostosis without prior surgical cranial vault remodeling, and claims for that indication may be denied.

Administrative policy history notes a minor review on 04/03/2025 that removed the asymmetry criteria and the criteria addressing subsequent cranial orthoses. This change is recorded in the policy history and is effective as of the 04/03/2025 update.

Use of an adjustable cranial orthosis is considered investigational for all indications not specifically described as medically necessary in this policy. The policy indicates insufficient evidence to support a general conclusion about benefits for orthosis use outside the listed medically necessary indications (postoperative use following cranial vault remodeling and positional plagiocephaly/brachycephaly meeting specified criteria).

Provider Actions — Authorization, Documentation, and Billing Guidance

    Prior Authorization

    Prior authorization may be product-dependent

Prior authorization requirements may vary by program or product; this policy applies only to certain Capital Blue Cross–administered programs and is subject to benefit variations. Check product-specific guidance (for example, FEP PPO refers to the FEP Medical Policy Manual) to determine prior authorization rules for the member's plan.

    • Policy applies only to certain programs and products administered by Capital Blue Cross and subject to benefit variations.
    • See product-specific guidance (e.g., FEP PPO refers to the FEP Medical Policy Manual).
    Prior Authorization

Coverage note for procedure code S1040

Procedure code S1040 is listed under “Covered when medically necessary,” but whether services are covered for a given member depends on that member’s benefit plan—identification of the code here does not guarantee coverage.

    • S1040 is listed in the policy’s covered procedure codes.
    • The identification of a code does not denote coverage; coverage is determined by member benefit information.

    Background

Cranial orthoses are adjustable helmets or bands designed to mold the infant cranium by applying corrective pressure to flattened areas while allowing growth in other regions. They are used postoperatively after cranial vault remodeling for synostosis and to treat positional (nonsynostotic) plagiocephaly or brachycephaly, which is associated with environmental risk factors such as supine sleeping and torticollis.

    Definitions and Measurement Metrics

    Cephalic index — inv-23

Age rangeIndividual must be between 3 and 18 months old
DocumentationMember age must be documented to confirm eligibility
ApplicationApplies to treatment of persistent positional plagiocephaly or brachycephaly with an adjustable cranial orthosis
    Step Therapy

Require ≥2 months conservative therapy before orthosis

Before an adjustable cranial orthosis is considered medically necessary for positional plagiocephaly or brachycephaly, document at least 2 months of failed conservative therapy consisting of repositioning and physical therapy.

    • Conservative therapy (repositioning and physical therapy) must be documented for a minimum of 2 months prior to orthosis consideration.
    Billing Rule

Verify coding and payer billing rules before submission

Confirm correct billing codes and payer requirements for the member’s plan prior to claim submission; presence of a procedure code in this policy does not ensure payment and not all covered services are eligible for separate reimbursement.

    • Codes listed may not be all-inclusive and are subject to change.
    • Not all covered services are eligible for separate reimbursement.
    Documentation Required

Required clinical documentation to support medical necessity

Include the member’s age to confirm eligibility within the 3–18 month range; document at least 2 months of unsuccessful conservative therapy (repositioning and physical therapy); and record objective measurements (e.g., cephalic index) demonstrating at least two standard deviations above or below the mean for age and gender.

    • Document age to confirm 3–18 months.
    • Document ≥2 months failed conservative therapy (repositioning and physical therapy).
    • Provide cephalic index measurement showing ≥2 standard deviations from the mean for appropriate gender and age.
    Billing Rule

    Coding and coverage determination note

The policy’s coding list may not be complete and codes can change; the identification of a code in this policy does not guarantee that the service is covered under the member’s benefits.

    • Code lists may not be all‑inclusive and are subject to change.
    • Identification of a code does not denote coverage; coverage is determined by member benefit information.
    Denial Risk

    Investigational—craniosynostosis without prior surgery

Use of an adjustable cranial orthosis for craniosynostosis in the absence of prior cranial vault remodeling surgery is considered investigational; claims for that indication may be denied.

    • Adjustable cranial orthosis is investigational for synostosis without prior cranial vault remodeling surgery.
    • Claims for that indication risk denial due to investigational status.
    Denial Risk

    Investigational for other indications

Use of an adjustable cranial orthosis for indications other than postoperative synostosis or persistent plagiocephaly/brachycephaly that meet policy criteria is considered investigational and may be denied.

    • Orthosis use for any indication not specifically outlined as medically necessary is considered investigational.
    • Such uses may be denied due to insufficient evidence of benefit.
    Note

    Coverage depends on member benefits

Coverage for services is determined by the member’s benefit information; identification of codes in this policy does not imply coverage and not all covered services are eligible for separate reimbursement—verify benefits before authorizing or billing.

    • Coverage is determined by member benefit information, not solely by code inclusion in this policy.
    • Not all covered services are payable as separate reimbursable items.
