A statement that informed consent has been obtained from the client

Background & Evidence

Gender incongruence is defined as a condition in which a person's gender identity does not align with the gender assigned at birth; gender dysphoria is the psychological distress that can result from that incongruence. Transgender and gender diverse (TGD) describes people whose gender identities or expressions differ from the gender socially attributed to their sex assigned at birth.

The goal of gender-affirming care is to partner with TGD individuals to holistically address social, mental, and medical health needs while affirming gender identity. Interventions may include puberty suppression, hormone therapy, and gender-affirming surgeries, and persons may undergo all, some, or none of these interventions.

Gender-affirming surgery is intended to be a permanent change to increase congruence between gender identity and physical appearance; detransition is proportionally rare but is addressed in the policy with criteria for when detransition-related surgical intervention may be considered medically necessary.

The policy references professional guidance supporting the benefit and safety of gender-affirming interventions, including WPATH Standards of Care Version 8 (2022) and ACOG Committee Opinion Number 823, March 2021; reaffirmed 2024, which note evidence of improvements in quality of life and well-being when interventions are properly indicated and performed.

Social transition considered: Social transition has been discussed and considered.
Lived in social role if recommended: The individual has lived in the social role for a prolonged period of time (if recommended by the multidisciplinary team).
Assessor documents best interest: The assessing health care professional, who is a member of the comprehensive multidisciplinary assessment team, documents how detransition is in the best interest of the individual.

A statement that the health care professional is available for coordination of care (including willingness to be contacted)

For this policy, a qualified health care professional is usually a licensed behavioral health care professional with a postgraduate degree.
