    Coverage Criteria

    Covered indication

Covered when all applicable criteria are met.

Covered indication: Automated ambulatory blood pressure monitoring is performed over a 24-hour period one time for an individual with elevated office blood pressure to differentiate white coat hypertension from true hypertension.1 time; 24-hour period

Patients aged 13 years and older

For individuals aged 13 years and older, all of the following must be met.

    • Office BP threshold: The patient is aged 13 years or older, and office blood pressure elevation is systolic blood pressure at least 130 mm Hg or diastolic blood pressure at least 80 mm Hg and below 160/100 mm Hg.SBP ≥130 or DBP ≥80 mm Hg AND <160/100 mm Hg
    • No immediate treatment requirement: The clinical presentation does not require immediate initiation of antihypertensive medication based on the severity of blood pressure elevation or acute hypertensive end-organ injury.

    Patients younger than 13 years

For individuals younger than 13 years, all of the following must be met.

    • Pediatric office BP threshold: The patient is younger than 13 years, and office blood pressure is at or above the lower of: the 95th percentile or greater based on gender, age, and height using the American Academy of Pediatrics 2017 normative pediatric blood pressure tables, or the adult cutoff. At or above the applicable pediatric 95th percentile or adult cutoff
    • No immediate treatment requirement: The clinical presentation does not require immediate initiation of antihypertensive medication based on the severity of blood pressure elevation or acute hypertensive end-organ injury.

    Elevated office blood pressure

The evidence is sufficient to determine that 24-hour automated ABPM improves the net health outcome for individuals with elevated office blood pressure.

Use of automated ABPM for elevated office blood pressure: The individual has elevated office blood pressure and receives 24-hour automated ambulatory blood pressure monitoring to improve diagnostic accuracy and avoid unnecessary treatment associated with white coat hypertension.24-hour automated ABPM

Compared with office or home blood pressure measurements, 24-hour ABPM has higher sensitivity, specificity, and predictive value for diagnosing hypertension.

    Suspected white coat hypertension

Medicare supplemental coverage criteria for suspected white coat hypertension. ABPM is reasonable and necessary for diagnosis when all of the following are met.

    • Office measurements: Average office systolic blood pressure is greater than 130 mm Hg but less than 160 mm Hg, or average office diastolic blood pressure is greater than 80 mm Hg but less than 100 mm Hg.>130 and <160 mm Hg systolic, or >80 and <100 mm Hg diastolic
    • Repeated visits and measurements: The office blood pressure findings occur on two separate clinic or office visits, with at least two separate measurements made at each visit.2 visits; at least 2 measurements per visit
    • Outside-office measurements: At least two blood pressure measurements taken outside the office are below 130/80 mm Hg.<130/80 mm Hg

    Suspected masked hypertension

Medicare supplemental coverage criteria for suspected masked hypertension. ABPM is reasonable and necessary for diagnosis when all of the following are met.

    • Office measurements: Average office systolic blood pressure is between 120 mm Hg and 129 mm Hg, or average office diastolic blood pressure is between 75 mm Hg and 79 mm Hg.120–129 mm Hg systolic, or 75–79 mm Hg diastolic
    • Repeated visits and measurements: The office blood pressure findings occur on two separate clinic or office visits, with at least two separate measurements made at each visit.2 visits; at least 2 measurements per visit
    • Outside-office measurements: At least two blood pressure measurements taken outside the office are at least 130/80 mm Hg.≥130/80 mm Hg

    NICE supplemental confirmation criteria

NICE supplemental diagnostic approach for confirming hypertension with ABPM.

Confirming hypertension with ABPM: When clinic blood pressure is between 140/90 mmHg and 180/120 mmHg, offer ABPM to confirm hypertension. During usual waking hours, obtain at least 2 measurements per hour and use the average of at least 14 measurements taken during usual waking hours. Confirm hypertension when clinic blood pressure is at least 140/90 mmHg and the ABPM daytime average is at least 135/85 mmHg.Clinic BP 140/90–180/120 mmHg; at least 2 measurements per hour; at least 14 daytime measurements; daytime average ≥135/85 mmHg

If ABPM is unsuitable or not tolerated, NICE recommends home blood pressure monitoring as an alternative.

Repeated ambulatory blood pressure monitoring in individuals with persistently elevated office blood pressure is considered investigational.

Using ambulatory blood pressure monitoring to monitor treatment effectiveness is considered investigational.

All other uses of ambulatory blood pressure monitoring for individuals with elevated office blood pressure are considered investigational.

    Provider Documentation and Denial Risks

    Documentation Required

    Document clinical eligibility

Document the patient’s age, office blood pressure measurements, the applicable adult or pediatric blood pressure threshold, the clinical purpose of testing to differentiate white coat hypertension from true hypertension, and that immediate antihypertensive treatment is not required because of severe elevation or acute hypertensive end-organ injury.

    Documentation Required

    Document pediatric monitoring standards

For pediatric patients, document use of a pediatric-appropriate device and cuff size and application of gender- and height-specific normative blood pressure data.

    Documentation Required

Meet ABPM device and interpretation requirements

Use an ABPM device capable of producing standardized 24-hour blood pressure plots with daytime and nighttime windows and normal blood pressure bands demarcated. Provide oral and written instructions, perform a test run in the physician’s office, and ensure results are interpreted by the treating physician or treating non-physician practitioner.

    Clinical Background

Automated ambulatory blood pressure monitoring (ABPM) typically records blood pressure over a 24-hour period, providing repeated measurements during both daytime and nighttime. This broader measurement profile can help distinguish elevated office blood pressure from out-of-office readings.

    Denial Risk

    Avoid noncovered or repeat testing

    • Repeated testing in individuals with persistently elevated office blood pressure is investigational.
    • Monitoring treatment effectiveness is investigational.
    • All other uses for individuals with elevated office blood pressure are considered investigational.
    Denial Risk

Do not use ABPM when immediate treatment is required

The service may be denied when the patient requires immediate initiation of antihypertensive medication because of the severity of blood pressure elevation or acute hypertensive end-organ injury.

    Denial Risk

Verify the covered office BP range

For patients aged 13 years and older, coverage is not supported unless office systolic blood pressure is at least 130 mm Hg or diastolic blood pressure is at least 80 mm Hg and the elevation is below 160/100 mm Hg.

    • Covered range: SBP ≥130 or DBP ≥80 mm Hg and <160/100 mm Hg.
