OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage Criteria for H. pylori Testing

Adult (≥18 years) - Noninvasive testing to diagnose

Urea breath testing OR stool antigen testing may be reimbursable when ANY of the following indications apply:

Adult diagnostic indications: dyspeptic symptoms; active peptic ulcer disease; past peptic ulcer disease without H. pylori history; low-grade gastric mucosa-associated lymphoid tissue (MALT) lymphoma; history of endoscopic resection of early gastric cancer; gastric intestinal metaplasia; uninvestigated dyspepsia in individuals <60 years without alarm features; initiating chronic non-steroidal anti-inflammatory drug (NSAID) therapy; unexplained iron deficiency anemia; evaluation of chronic immune thrombocytopenic purpura with suspected H. pylori infection; family history of gastric cancer; first-generation immigrants from high-prevalence areas

Adult (≥18 years) - Test of eradication (follow-up)

Urea breath testing or stool antigen testing to measure eradication may be reimbursable when ANY of the following apply; testing must be at least 4 weeks post-treatment:

Eradication follow-up indications: H. pylori–associated ulcer; persistent dyspepsia after appropriate antibiotic treatment for H. pylori; gastric MALT lymphoma; post-resection of early gastric cancer>=4 weeks post-treatment

    Adult (≥18 years) - Biopsy-based testing

For individuals undergoing endoscopy or with alarm symptoms, biopsy-based histology plus rapid urease test or culture with susceptibility testing may be reimbursable.

Indication for biopsy-based testing: Undergoing endoscopic examination or having alarm symptoms

Includes histology with rapid urease test or culture with susceptibility testing

Pediatric (<18 years) - Diagnostic and eradication testing

Noninvasive testing and follow-up may be reimbursable in limited pediatric indications:

Pediatric noninvasive indications: Chronic immune thrombocytopenic purpura with suspected H. pylori infection; follow-up eradication testing after treatment>=4 weeks post-treatment for eradication

Pediatric (<18 years) - Biopsy-based testing (when reimbursable)

Biopsy-based histology plus rapid urease or culture with susceptibility may be reimbursable for:

Pediatric biopsy indications: Gastric or duodenal ulcers; refractory iron deficiency anemia when other causes have been ruled out

Urea breath testing or stool antigen testing to diagnose Helicobacter pylori infection is not reimbursable for asymptomatic individuals of all ages. This exclusion applies to diagnostic noninvasive testing regardless of patient age or setting. Additionally, serologic testing for H. pylori is not reimbursable for any age. Providers should confirm clinical indications before ordering testing to avoid denial of payment.

Concurrent testing using any combination of the urea breath test, stool antigen testing, and/or biopsy-based testing to diagnose H. pylori may not be reimbursable. Ordering multiple modalities for the same diagnostic episode is discouraged; select the single most appropriate test based on clinical presentation and prior procedures. Also note that testing performed after recent use of antibiotics, proton pump inhibitors, or bismuth may not be reimbursable, and nucleic acid testing for H. pylori may not be reimbursable.

    Note

    Verify member eligibility and benefits

Coverage is determined by date of service, member eligibility, benefit limits, and provider agreements. Confirm member eligibility and benefits prior to ordering or performing testing to ensure the service is a covered benefit for the date of service.

    • Confirm eligibility and benefits for the date of service
    • Verify any applicable benefit limits or frequency restrictions
    • Confirm provider network status and applicable provider agreement
    Billing Rule

    Provider billing action

Bill and submit claims according to standard Blue KC billing guidelines. Use appropriate CPT/HCPCS codes for the selected test and append modifiers as required by the claim context. Providers should contact the Blue KC Provider Hotline for billing questions or unusual claim edits.

    Definitions

Eradication testing — Follow-up testing (key facts)
DefinitionFollow-up urea breath test or stool antigen testing performed at least 4 weeks post-treatment to confirm H. pylori eradication.
Minimum timingTesting must be performed at least 4 weeks after completion of H. pylori eradication therapy.
Test modalitiesUrea breath test or stool antigen test are acceptable noninvasive options for eradication confirmation.
Exclusions affecting accuracyTesting may not be reimbursable if performed after recent use of antibiotics, proton pump inhibitors, or bismuth (may reduce accuracy).
Concurrent testingConcurrent use of multiple modalities (urea breath, stool antigen, and/or biopsy-based testing) to diagnose is not reimbursable; follow policy guidance when confirming eradication.

    Background

Helicobacter pylori is a spiral-shaped, gram-negative bacterium that survives in acidic gastric environments and is implicated in a range of upper gastrointestinal diseases. Infection with H. pylori is associated with chronic gastritis and is a major cause of peptic ulcer disease. Long-term infection increases risk for more serious outcomes including gastric adenocarcinoma and gastric mucosa-associated lymphoid tissue (MALT) lymphoma. Diagnostic options include noninvasive tests (urea breath test, stool antigen, serology) and invasive biopsy-based methods (histology, rapid urease, culture with susceptibility); timing of testing relative to antibiotics, proton pump inhibitors, or bismuth can affect diagnostic accuracy.

    Documentation Required

    Required clinical indication documentation

Document clinical indications thoroughly in the medical record to support the selected test. The record should state the presenting symptoms or diagnostic reason that matches the policy's covered indications.

    • Record the patient’s symptoms (e.g., dyspepsia) or specific indication (e.g., gastric MALT lymphoma)
    • For post-treatment eradication testing, document date of completion of therapy and that testing is ≥4 weeks after treatment
    • For NSAID initiators, document intent for chronic NSAID therapy
    Denial Risk

    Common denial triggers

Testing may be denied when clinical or pre-analytic conditions make results unreliable or when testing does not meet covered indications. Review common denial triggers before ordering tests.

    • Do not order testing for asymptomatic individuals of any age
    • Avoid urea breath, stool antigen, or biopsy testing within the window after recent antibiotics, proton pump inhibitors, or bismuth use
    • Do not perform concurrent combinations of urea breath test, stool antigen, and biopsy-based testing for diagnosis
    • Serologic testing and nucleic acid testing for H. pylori are not reimbursable per policy
