Core indications: Replacement of missing teeth with retainer crowns and fixed partial dentures may be considered appropriate for teeth lost due to accidental traumatic injury, pathologic disorder resulting in extraction, congenitally missing teeth, or congenital disorders resulting in extraction.
See chunks 3,5.
Documentation: Current (within 12 months), dated diagnostic pretreatment radiographic images that include the radiographic apex are required; when radiographs are insufficient, intra-oral photographs and clinical chart notes stating the rationale, and a current dated 6‑point periodontal chart may be requested.12 months

Tooth-specific structural criteria: Anterior retainer: significant coronal tooth structure loss (caries, fracture, defective restoration or involvement of both incisal angles). Posterior retainer: significant missing tooth structure due to caries, fracture, or defective restoration; large restorations compromising function; undermined cusps (one or more) with compromised mesial or distal marginal ridges.
See chunk 3.
Periodontal and endodontic requirements: Teeth must have controlled periodontal health or definitive periodontal therapy and maintenance documented; placement is not covered for teeth with untreated or unresolved periapical/periradicular pathology, unresolved caries near the pulp without planned endodontic therapy, internal or external resorption, subgingival/sub‑osseous caries compromising supracrestal tissue attachment, or caries into the furcation. Endodontic obturation should extend close to the apical constriction (ideal 0.5–1.2 mm) with appropriate fill density; gross overextension or significant underfill is unacceptable.
See chunks 4,6,7.
Exclusions specific to indications: Fixed partial dentures placed for occlusal alterations or vertical dimension changes, for treatment of temporomandibular joint disorder/craniomandibular disorder, for repair of complications from wear/attrition/abrasion/erosion/abfraction, for splinting (contract dependent), or to close spaces where there is no missing natural tooth are not covered or are contract dependent.
See chunks 4,3,5.
Prosthesis type and cantilever restrictions: Tooth‑implant supported fixed partial dentures and fixed partial dentures supported by inlay retainers are not considered benefits. Cantilever rules: anterior cantilevers where cuspids retain lateral incisors may be allowed; posterior cantilevers are limited to a single pontic supported by two retainers. If one retainer is denied, the entire fixed partial denture may be denied.
See chunk 6.
Replacement rules: Replacement of an existing retainer crown or fixed partial denture is covered only when supported by recurrent caries, fracture, or defective restoration; discoloration, misshapenness or cosmetic compromise alone do not qualify.
See chunks 6,3.
Missing tooth clause handling: When the plan includes a missing tooth clause, the pontic portion will be denied for teeth extracted and not replaced prior to plan coverage and retainers will be reviewed on their own merit. For plans without a missing tooth clause: if fewer than three teeth are missing in the arch, appropriateness of the fixed partial denture as a whole will be determined; if three or more teeth are missing, a removable partial denture alternate benefit may be considered (contract dependent) and retainers are reviewed on their own merit.3 teeth
