OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage Criteria for Local Delivery Agents

    Coverage criteria for LDCA/LDAA

Covered when ALL of the following are met

Prerequisite therapy: Definitive non-surgical (scaling and root planing) and/or surgical periodontal treatment has been completed and adequate time for healing has been allowed.6 weeks to 6 months post-therapy

Per policy: allow 6 weeks to 6 months after scaling/root planing or periodontal surgery before local delivery.

Site characteristics: Target tooth/site demonstrates a periodontal pocket measuring 5 mm or greater with persistent signs of inflammation (spontaneous bleeding, bleeding on probing, or suppuration) and a history of increasing pocket depth with associated clinical attachment loss.>= 5 mm

Defines refractory disease site and minimum probing depth requirement.

Extent limitation: Indication limited to isolated, localized refractory or recurrent/residual sites (including during periodontal maintenance); not appropriate for generalized periodontitis where comprehensive or surgical therapy is indicated.localized

Policy excludes generalized periodontitis as an indication and notes surgical therapies may be necessary.

Documentation: A current (within 12 months), dated, post-periodontal maintenance and/or post-periodontal surgery 6-point periodontal charting is required; medically compromised patients require physician documentation and will be reviewed case-by-case.charting within 12 months

Documentation and case-by-case review requirements per policy.

Tooth selection: Only natural teeth with favorable periodontal outcomes will be considered for benefit.

Policy limits benefit to natural teeth with favorable outcomes.

The use of locally delivered chemotherapeutic or antimicrobial agents (LDCA/LDAA) for the purpose of regeneration of alveolar bone is considered experimental and investigational and is excluded from coverage.

LDCA/LDAA modalities are not appropriate for periodontal probing depths of less than 5 mm. When a generalized pattern of periodontitis is diagnosed, more comprehensive intervention—such as periodontal surgical therapies—may be necessary and isolated local delivery is not indicated.

These agents are intended for isolated, refractory or localized recurrent/residual sites encountered during periodontal maintenance after definitive therapy, not for generalized disease. Providers should confirm that root planing or other definitive non‑surgical therapy has been completed and allow an adequate healing interval before local delivery is performed.

Provider Requirements, Sequencing, and Denial Triggers

    Prior Authorization

    Prior authorization / coding note — D4381

CDT code D4381 (Localized delivery of antimicrobial agents via a controlled release vehicle into diseased crevicular tissue per tooth) is listed as an applicable procedure code; verify member contract benefits and prior authorization requirements for D4381 with the payer before billing or performing the service.

    • Inclusion of a code in the policy does not guarantee coverage or reimbursement; check member contract benefits.
    • Prior authorization requirements are not explicitly stated in this document and must be confirmed with the plan.
    Step Therapy

Therapy sequencing — adjunctive use after definitive periodontal therapy

LDCA/LDAA are considered adjunctive therapies and should only be used after definitive non-surgical (scaling and root planing) and/or surgical periodontal treatment has been completed and an adequate healing interval allowed.

    • Allow a healing period of six weeks to six months after scaling and root planing or periodontal surgery before performing local delivery.

    Background and Rationale

Scaling and root planing is the standard non‑surgical therapy for chronic periodontitis and is highly effective in the majority of diseased sites; most will respond and will not require adjunctive local antimicrobial delivery.

Local delivery of antimicrobial agents is considered an adjunctive therapy after definitive non‑surgical (scaling and root planing) and/or surgical periodontal treatment. An adequate healing period of 6 weeks to 6 months post‑therapy should be allowed before performing local delivery, and treated teeth must demonstrate an active/refractory site (typically a periodontal pocket of >= 5 mm with persistent signs of inflammation).

    Key Definitions

Refractory disease site — definition including pocketing >= 5 mm with persistent signs of inflammation/bleeding
DefinitionAn area with periodontal pocketing equal to or greater than 5 millimeters (mm) with persistent signs of inflammation, spontaneous bleeding or bleeding on probing, suppuration and/or increasing loss of clinical attachment.
Clinical contextEvaluation for LDCA/LDAA is made after completion of definitive non-surgical and/or surgical periodontal treatment; may also be indicated for isolated refractory sites at periodontal maintenance visits.
Site characteristicsUsually described as an active/refractory site with history of increasing pocket depth and associated attachment loss.

  1. Appropriate for isolated/refractory sites or localized recurrent/residual sites encountered during periodontal maintenance, not for generalized periodontitis.
  2. Documentation Required

Required documentation — dated 6‑point periodontal charting within 12 months

A current, dated 6‑point periodontal charting performed within the prior 12 months is required and must be included in the patient record; medically compromised patients require physician documentation for case‑by‑case review.

    • Charting must be post-periodontal maintenance and/or post-periodontal surgery and dated within 12 months.
    • Only natural teeth with favorable periodontal outcomes will be considered for benefit.
    Denial Risk

Denial triggers — non-covered concurrent services and clinical exclusions

Services performed in conjunction with or in preparation for a non-covered or denied service will also be not covered or denied; LDCA/LDAA are not appropriate for sites with probing depths less than 5 mm and are not indicated for generalized periodontitis where more comprehensive therapy is required.

    • LDCA/LDAA modalities are not appropriate for periodontal probing depths < 5 mm.
    • When generalized periodontitis is diagnosed, surgical therapies or more comprehensive interventions should be considered; isolated local delivery may be denied in that context.
