OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage Criteria — When Gingivectomy/Gingivoplasty Are Covered

Covered when ALL of the following are met

Gingivectomy or gingivoplasty is considered appropriate when treating mild to moderate periodontal disease and when the following criteria are satisfied:

Primary indications: Performed to eliminate suprabony pockets or restore normal architecture for gingival enlargements or unaesthetic topography with normal bony configuration; indicated for pocket elimination and recontouring in presence of supra-bony pockets with normal bony configuration.
Failed nonsurgical therapy: Used when nonsurgical methods (eg, root planing and scaling) have been unsuccessful in removing subgingival deposits of plaque and calculus.
Diagnostic documentation: Current (within 12 months) dated 6-point periodontal charting showing pocket depth recordings (minimum 5 mm); charting after completion of nonsurgical periodontal therapy and/or periodontal maintenance; current (within 12 months) diagnostic quality pretreatment radiographs showing periapical area and the alveolar crest.>= 5 mm
Therapy timing: Completion of initial periodontal therapy (eg scaling and root planing) with a minimum 4-week healing interval prior to surgical treatment to assess success of nonsurgical therapy.>= 4 weeks
Frequency and quadrant limits: Benefits generally limited to one periodontal surgical procedure in a [36/60] month period per single tooth or multiple teeth in the same quadrant; benefits limited to two quadrants per date of service (exceptions case-by-case).[36/60] months

Other appropriate uses — single top-level node

Additional indications: May be performed to allow access for restorative procedures; may be used to remove loose or diseased tissue and to re-sculpt excess tissue from drug-induced gingival hyperplasia (eg, Dilantin therapy).

Gingivectomy or gingivoplasty are contraindicated for treatment of infra-bony pockets and for pockets that extend below the mucogingival junction. Procedures that require access to the alveolar bone or where only a minimal amount of attached keratinized tissue is present are also excluded and may be denied.

When gingivectomy is proposed to provide restorative access, benefits are group contract dependent and the procedure may be considered inclusive to the primary restorative procedure or incidental if diagnostic documentation does not support medical necessity; evaluate submissions with D4212 per guidance. These anatomic and documentation issues increase the risk of denial if not clearly supported.

Gingivectomy performed for the sole purpose of correcting altered passive eruption is not benefited and is excluded from coverage.

Gingivectomy is considered cosmetic when performed within six months of orthodontic treatment and is therefore not a covered medical necessity indication.

When gingivectomy is performed to allow access for restorative procedures, benefits are group contract dependent; the procedure may be considered inclusive to the restorative work or incidental if diagnostic documentation (bone level, gingival level, recession) indicates adequate access without surgery. Document restorative need and whether the gingival procedure is inclusive to the primary procedure to support benefit determination.

Provider Actions — Authorization, Documentation, and Denial Risks

    Prior Authorization

    Prior Authorization / Benefit Determination

Prior authorization and benefit determination may be required; verify group contract–dependent benefits and any frequency limits before scheduling.

    • Affected codes: D4210, D4211, D4212, D4999
    • Benefits may be limited to two quadrants per date of service; one periodontal surgical procedure per 36/60 months per tooth or quadrant depending on group contract
    Documentation Required

    Nonsurgical Therapy Prerequisite

Completion of initial nonsurgical periodontal therapy (e.g., scaling and root planing) and a minimum healing/assessment period are required prior to surgical intervention.

    • Document completion of initial periodontal therapy
    • Allow ≥4 weeks healing prior to surgical treatment

    Background

Gingivectomy is the surgical removal of diseased gingival tissue performed to eliminate suprabony pockets and restore normal gingival architecture in the presence of periodontal disease. Gingivoplasty is the reshaping or recontouring of otherwise healthy gingival tissues when periodontal pockets are absent.

Both procedures may be used to improve access for restorative treatment or to recontour excess tissue (for example, drug-induced gingival hyperplasia). Use of lasers or electrosurgery is considered an adjunct and is not eligible for an additional benefit separate from the primary procedure.

Gingivectomy — surgical removal of diseased gum tissue
DefinitionSurgical removal of diseased gum tissue in the presence of periodontal disease.
IndicationsEliminate suprabony pockets, restore normal gingival architecture for enlargements, improve access for restorative procedures, or recontour drug-induced gingival hyperplasia.
PrerequisitesCompletion of initial nonsurgical periodontal therapy with a minimum 4‑week healing interval prior to surgery and diagnostic documentation (6‑point charting and radiographs).
Exclusions/contraindicationsNot indicated for infra‑bony pockets, pockets extending below the mucogingival junction, procedures requiring access to alveolar bone, or when minimal attached keratinized tissue is present.

    Documentation Required

    Required Clinical Documentation

Provide current (within 12 months) diagnostic documentation after nonsurgical therapy: a dated 6‑point periodontal charting and diagnostic-quality radiographs.

    • 6‑point periodontal charting showing pocket depths (minimum 5mm when applicable)
    • Diagnostic-quality radiographs showing periapical area and alveolar crest
    Documentation Required

    Restorative Access Documentation

When gingivectomy is performed to allow restorative access, document that the procedure is for access to crown(s)/restoration(s) and whether benefits for the gingivectomy are inclusive per the member's group contract.

    • If billed with restorations, indicate associated restorative procedure and supporting diagnostics
    • If billed without an associated restorative procedure, include clinical justification per D4211/D4212 guidance
    Denial Risk

    Contraindicated Anatomy

Gingivectomy is contraindicated for infra‑bony pockets or pockets extending below the mucogingival junction; procedures requiring access to alveolar bone or treatment of pockets below the mucogingival junction may be denied.

    • Do not use gingivectomy for infra‑bony pocket treatment or pockets extending below the mucogingival junction
    • Alternative codes (e.g., osseous procedures) may be appropriate when bone access is required
    Denial Risk

D4212 — Diagnostic Insufficiency and Inclusion Rules

D4212 (gingivectomy to allow access for restorative procedure) may be considered inclusive with restorations or incidental to crown placement; if diagnostics (bone level, gingival level, recession) indicate adequate access, the procedure may not be necessary and may be denied. When submitted without an associated restorative procedure, evaluate per D4211 guidelines.

    • When billed with crown/restoration, submit supporting restorative documentation and diagnostics
    • If no restorative procedure is present, supply clinical justification per D4211 or consider denial
Reshaping of otherwise healthy gingival tissues in the absence of periodontal pockets.
UsesRecontouring excess tissue for aesthetic reasons or to improve gingival topography; may be used to allow access for restorative procedures when pockets are not present.
Adjunct techniquesUse of lasers/electrosurgery is considered an adjunct and is not eligible for an additional separate benefit.
LimitationsContraindicated for treatment of infra‑bony pockets or where pockets extend below the mucogingival junction; benefits limited by group contract and frequency rules.
