OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria

inv-01: Covered when ALL of the following are met

Covered when ALL of the following are met:

Primary coverage criteria: Non-surgical methods such as scaling and root planing have been attempted and been unsuccessful; periodontal charting after completion of non-surgical therapy or maintenance is required.
Pocket depth requirement: Current (within 12 months), dated, post initial therapy 6-point periodontal charting indicating pocket depth recordings of a minimum of 5 mm.
Radiograph requirement: Current (within 12 months), dated, pretreatment radiographs showing the periapical area and an undistorted image of the alveolar crest.
Anatomic indication: Procedure indicated for supra-bony pocket depths when increased access to root surfaces is needed and osseous recontouring or bone grafting are not required.
Documentation of surgical intent: Chart notes may be requested to demonstrate a soft tissue flap was reflected, resected, or planned to allow debridement of the root surface and removal of granulation tissue; osseous recontouring must not be accomplished with this procedure.
Tooth type: Performed for periodontal defects involving natural teeth only; procedures performed around implants are not considered under this policy.
Scope per date of service: Limited to two quadrants per date of service; exceptions may be allowed on a case-by-case basis.
Keratinized tissue consideration: May be performed in the presence of minimal amounts of attached keratinized tissue when clinically indicated.
Adjuncts and concurrent procedures: The use of lasers is considered an adjunct and is not eligible for an additional or separate benefit; when multiple similar procedures are performed on the same date of service, the less complex procedure is considered inclusive to the primary procedure.
Diagnostic and staging guidance: A periodontal diagnosis including staging and grading per current AAP and ADA guidelines is required.

Procedures performed around dental implants are not considered eligible under this policy. The policy explicitly states that gingival flap procedures are limited to periodontal defects involving natural teeth only and will not be considered when the procedure is performed around implants.

Gingival flap procedures are not considered appropriate when osseous recontouring is required — for example, treatment of infra‑bony defects that require bone manipulation. The policy specifies that osseous recontouring is not accomplished with a gingival flap and that the procedure is intended to provide soft tissue reflection for root debridement and removal of granulation tissue rather than bone modification.

    Prior Authorization

    Prior Authorization / Supporting Documentation

Prior authorization may be required. Submit supporting documentation with the prior authorization request or with the claim to demonstrate medical necessity.

    • Current (within 12 months), dated, post-initial therapy 6‑point periodontal charting with pocket depth recordings (minimum 5 mm)
    • Current (within 12 months), dated pretreatment radiographs showing the periapical area and an undistorted image of the alveolar crest
    • Chart notes documenting completion of non-surgical periodontal therapy (scaling and root planing) and rationale why non-surgical care was unsuccessful
    • Operative notes demonstrating a soft tissue flap was reflected or resected (or planned) to allow debridement of root surfaces and removal of granulation tissue when requested
    • Documentation supporting quadrant limits (no more than two quadrants per date of service) or justification for an exception
    • If procedure performed around implants: documentation will be reviewed and such procedures may be denied when submitted as gingival flap procedures for natural teeth

    Definitions

inv-14: Gingival flap procedure — Reflecting a soft tissue flap to allow debridement of the root surface and removal of granulation tissue.
DefinitionReflecting a soft tissue flap to allow debridement of the root surface and removal of granulation tissue.
Clinical intentIncrease access to root surfaces to remove subgingival plaque and calculus after nonsurgical therapy has failed.
Bone manipulationDoes not involve manipulation or recontouring of alveolar (osseous) bone.
ScopePerformed for periodontal defects of natural teeth only; not considered when performed around implants.
inv-15: Apically positioned flap — A surgical flap positioned apically to maintain an adequate zone of keratinized tissue with the goal of pocket elimination.
    Definition

    Background

Gingival flap procedures involve reflecting soft tissue to provide access for debridement of root surfaces and removal of granulation tissue. An apically positioned flap is used to maintain or increase the zone of keratinized tissue and to help eliminate supra‑bony pockets. These procedures are intended to treat supra‑bony pocketing where increased access to root surfaces is needed and do not include manipulation of alveolar bone or osseous recontouring.

    Step Therapy

    Stepwise Care Requirement

Gingival flap procedures are considered only after a stepwise course of care: initial non‑surgical therapy (scaling and root planing) and periodontal maintenance must be attempted and documented prior to surgical flap therapy.

    • Periodontal charting is required after completion of non‑surgical periodontal therapy or periodontal maintenance
    • Documentation must show non‑surgical methods were unsuccessful in removing subgingival plaque/biofilm and calculus before flap surgery is considered
    Documentation Required

Provider Actions — Periodontal Charting, Radiographs, and Chart Notes

Providers must supply current periodontal charting and dated pretreatment radiographs with prior authorization requests or upon request for claim review. Chart notes may be requested to confirm that a soft tissue flap was reflected/resected to allow debridement of the root surface and removal of granulation tissue.

    • Provide 6‑point periodontal charting (post‑initial therapy) within 12 months showing minimum 5 mm pocket depths
    • Provide dated pretreatment radiographs taken within 12 months showing periapical area and alveolar crest
    • Retain and provide chart/operative notes that document the surgical flap procedure (or planned flap) and that osseous recontouring was not performed
    • If lasers are used, note that laser use is considered adjunctive and is not eligible for a separate benefit
    Denial Risk

Provider Actions — Documentation Deficiencies May Lead to Denial

Failure to provide required current (within 12 months) dated post‑initial therapy 6‑point periodontal charting showing minimum 5 mm pocket depths, or failure to provide dated pretreatment radiographs and supporting chart/operative notes when requested, may result in denial of the procedure.

    • Missing or outdated periodontal charting (older than 12 months)
    • Absent or nondiagnostic pretreatment radiographs
    • Lack of chart notes documenting prior non‑surgical therapy and surgical flap reflection/resection
    Billing Rule

    Procedures Around Implants

Procedures performed around dental implants are not considered gingival flap procedures for natural teeth and may be denied when submitted or billed as such. Document clearly if treatment involves implants; separate implant‑specific procedures may have different coverage rules.

    • Gingival flap procedures billed for sites around implants are not considered for natural tooth gingival flap benefit
    • If treatment involves implants, provide clear documentation; expect review under implant‑related procedure rules
A surgical flap positioned apically to maintain an adequate zone of keratinized tissue with the goal of pocket elimination.
Clinical goalMaintain/increase keratinized tissue and help eliminate supra-bony pockets when access is needed to root surfaces.
Bone manipulationDoes not include osseous recontouring or bone grafting as part of the procedure.
IndicationsUsed after unsuccessful nonsurgical therapy and when minimal attached keratinized tissue is present; not performed around implants or for pockets below the mucogingival junction.
