OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage Criteria — Generally Accepted Standard of Dental Care

    Not meeting generally accepted standards

A service may be considered not meeting generally accepted standards of dental care when ALL of the following applicable conditions apply to the tooth or proposed treatment:

Structural and periodontal considerations: Includes crown-to-root ratio poorer than 1:1; significant coronal structural loss appearing unrestorable; root fracture/defects/perforation; caries into the furcation; unrestorable due to extent/location of root caries; and sub-gingival/sub-osseous carious lesions compromising supracrestal tissue attachment without addressing restorative and periodontal considerations.
Periodontal status requirement: Teeth demonstrating uncontrolled or untreated periodontal disease, evidenced by radiographic or periodontal charting, loss of supporting bone including furcation, or presence of tooth mobility may not be approved unless the treating dentist demonstrates definitive periodontal therapy and periodontal maintenance have been successfully performed, or the treatment plan includes periodontal therapy with success to be evaluated prior to approval. Current (within 12 months) dated 6-point periodontal charting and history of periodontal therapy may be requested.
Endodontic conditions: Endodontic concerns include untreated or unresolved periapical/periradicular pathology; unresolved lesion in close proximity to the pulp without planned endodontic therapy; overfill/underfill root canal obturation or poor condensation; and internal or external resorption — such teeth will not be considered for benefits other than endodontic therapy (see Dental Clinical Policy 03-001).

Dental care directed at salvaging non-functional or non-restorable teeth may be excluded from benefits. The plan reviews submitted treatment to determine whether proposed or rendered services meet the contractual requirement that dental services adhere to generally accepted standards of dental care and are appropriate in type, frequency, and extent. Services that are primarily cosmetic or provided for convenience are not considered medically/dentally necessary and may be excluded.

A tooth may be judged unrestorable for restorative procedures for any of the following reasons: significant coronal structural loss that appears unrestorable; presence of a root fracture, root defect, or root perforation; caries extending into the furcation; or extensive root caries where the extent and location of the lesion make restoration impractical.

Provider Requirements, Documentation, and Prior Authorization

    Prior Authorization

Prior authorization — submit requested diagnostic documentation

When requests are submitted by treating dentists, provide requested documentation (radiographs, charting, intra-oral photos, and periodontal history) so the plan can determine whether proposed services meet generally accepted standards of dental care; benefit determination may require demonstration of definitive periodontal or endodontic therapy prior to approval.

    • Include radiographs that show the radiographic apex (see required documentation).
    • Provide current periodontal history and any prior periodontal therapy performed.
    • Supply intra-oral photographs and clinical chart notes as requested.
    Step Therapy

Sequencing — perform less costly alternatives and required definitive therapy first

Obtain and document less costly appropriate alternatives and complete any prerequisite definitive periodontal or endodontic therapy (or include these treatments in the plan with evidence of successful therapy) before approval of indirect restorations or other services.

    Background and Policy Scope

Review determines whether dental services are provided in accordance with generally accepted standards of dental practice and whether the proposed treatment is effective and appropriate for the patient’s condition. The review considers clinical factors including periodontal health, endodontic status, structural integrity (for example crown-to-root ratio and significant coronal loss), and cost-effectiveness compared with alternative services. If these factors indicate the tooth or proposed treatment does not meet generally accepted standards, benefits may be denied or excluded.

    Use in benefit determination
Current radiographs and periodontal charting may be requested prior to approval to evaluate periodontal and endodontic status
    • Definitive periodontal therapy and maintenance must be successfully performed or included in the treatment plan for evaluation prior to approval of indirect restorations.
    • Endodontic conditions may require endodontic therapy per Dental Clinical Policy 03-001 before restorative benefits are considered.
    Documentation Required

Required documentation — radiographs, charting, photos, clinical notes

Include current (within 12 months), dated, properly identified pretreatment radiographic image(s) that show the radiographic apex, clinical chart notes, intra-oral photographs, current (within 12 months) dated 6-point periodontal charting, and history of periodontal therapy when requested.

    • Radiographs must be pretreatment, properly identified, dated, and include the radiographic apex.
    • Provide current (within 12 months) 6-point periodontal charting and documentation of periodontal therapy or maintenance.
    Denial Risk

Denial risk — failure to meet generally accepted standards may result in denial

Services may be denied if treatment is directed at salvaging non-functional or non-restorable teeth or otherwise does not meet generally accepted standards of dental care considering full‑mouth condition, periodontal and endodontic status, and cost‑effectiveness.

    • Teeth with crown-to-root ratio poorer than 1:1, significant coronal structural loss that appears unrestorable, root fracture/defects/perforation, furcation caries, or extensive/root‑location root caries may be considered not meeting standards.
    • Untreated or unresolved periapical/periradicular pathology, unresolved lesions near the pulp without planned endodontic therapy, poor root canal obturation, or internal/external resorption will not be considered for restorative benefits (see Endodontic Therapy policy).
Cost-effectiveness requirementServices should not be more costly than an appropriate alternative
