OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria

    Medical necessity criteria

Covered when ALL of the following are met

Timeliness: Treatment is generally rendered within seventy-two (72) hours of the onset of the injury, unless delayed due to hospitalization for other accident-related injuries.<=72 hours

    Exceptions allowed for hospitalization

Injury type: Injury results from extra-oral blunt force trauma to sound natural teeth and/or supporting hard and soft tissues (not due to chewing or biting forces).N/A

Sound natural teeth are teeth that were stable, functional, and free from decay, fracture, or advanced periodontal disease at the time of injury.

Pre-existing conditions: Services treating pre-existing or pre-accident dental conditions are not appropriate unless specified by group contract; related services performed in conjunction with non-covered services are also not covered.N/A

Coverage depends on group contract terms

Documentation: Required clinical documentation includes diagnostic pre- and post-trauma radiographs (properly oriented, labeled, and dated) that reveal all existing and missing teeth in both arches; chart notes; intra-oral and facial photographs when appropriate; accident/ER report; and a letter from the treating dentist describing the accident, injuries, and proposed treatment.N/A

Radiographs must be of diagnostic quality (full mouth, panoramic, or other as appropriate)

Initial services: Initial/emergency/palliative treatments (evaluation/diagnosis, radiographic evaluation, extraction, suturing, splinting, re-implantation/repositioning/stabilization of dislodged teeth) are permitted without prospective review.N/A

Retrospective claim review will be conducted after services are provided to evaluate documentation, coding, and necessity.

Definitive care timeframe: Definitive restorative and reconstructive treatment related to the accidental dental injury is considered when performed within 12 months of the date of injury.<=12 months

May include extraction, endodontic, periodontic (soft tissue grafting), restorative, implant, fixed/removable prosthodontic services, and bone grafting.

Dental services are not considered appropriate for treatment of accidental dental injuries when they address conditions that existed before the accident. Specifically, services rendered to treat pre-existing or pre-accident dental conditions are not considered appropriate unless the group contract explicitly states otherwise. Additionally, when a service is performed in conjunction with or in preparation for a non-covered or denied service, related services are also subject to non-coverage or denial.

Services that address pre-existing or pre-accident dental conditions rather than injuries caused by extra-oral blunt force trauma are not considered medically necessary. Sound natural teeth are defined as teeth that were stable, functional, and free from decay, fracture, and advanced periodontal disease at the time of the accident; treatments directed at problems present before the accident fall outside the policy’s medical necessity criteria. When performed with or to prepare for a non-covered service, related procedures will also be treated as not medically necessary.

    Provider Actions & Documentation Requirements

    Prior Authorization

Prior authorization not required for initial/emergency/palliative care

Prospective treatment review is not required for initial, emergency, or palliative services. After these services are provided, Blue Cross Blue Shield - Wisconsin may perform a retrospective claim review to evaluate reimbursement, documentation accuracy, coding, and medical/dental necessity.

    Note

Retrospective claim review and potential payment adjustments

Retrospective claim review may include evaluation of reimbursement levels, documentation accuracy, coding accuracy, and adjudication of payment; appropriateness and medical/dental necessity will be judged against prevailing standards or current dental practice.

    • Expect retrospective review of claims after initial/emergency care is provided.
    • Denial or payment adjustment may follow based on documentation, coding, or medical necessity findings.

    Background

Accidental dental injuries are physical damage to teeth and/or supporting hard and soft tissues resulting from extra-oral blunt force trauma and not from chewing or biting forces. These policies apply to injuries to sound natural teeth—teeth that were stable, functional, and free from decay, fracture, or advanced periodontal disease at the time of the accident. Timely evaluation and treatment are required to preserve tooth structure and function; initial emergency care focuses on stabilization, pain control, radiographic assessment, and re-implantation or repositioning when indicated, with definitive restorative care addressed according to the policy’s timeframes and documentation requirements.

    Documentation Required

Required clinical documentation for accidental dental injury claims

Provide full clinical documentation with every accidental dental injury claim: diagnostic pre- and post-trauma radiographs (properly oriented, labeled, and dated) that reveal all existing and missing teeth in both upper and lower arches, chart notes, intra-oral and facial photographs when appropriate, the accident/ER report, and a letter from the treating dentist describing the accident, injuries, and proposed treatment.

    • Radiographs may include full mouth, panoramic, and/or other appropriate images of diagnostic quality.
